Stent treatment for huge aneurysm of the common hepatic artery: A case report.

ABSTRACT:

Objective/background

Huge aneurysm of the visceral artery is rare and a treatment strategy for such cases has not yet been established. Here, we report a case of huge aneurysm of the common hepatic artery (44-mm diameter) successfully treated by stent placement.

Methods

A 77-year-old female patient was referred to our department due to growth of the common hepatic artery aneurysm. The cause of the aneurysm was suspected to be segmental arterial mediolysis. Due to the possibility of a spontaneous rupture, we decided to stent the common hepatic artery.

Result

We had some difficulties during the procedure, such as thrombosis of the stent, and it was necessary to insert an additional stent. The procedure was effective and the patient has been doing well without any complications at the 6-year follow-up.

Conclusion

Stenting is possible and effective in cases of huge aneurysm of the common hepatic artery.

Project description:Patient is a 72-year-old male who presented with abdominal pain and hypertension. Through bedside point-of-care ultrasound (POCUS), patient was found to have markedly enlarged bilateral iliac artery aneurysms with signs of impending rupture, which was confirmed on computed tomography (CT) angiogram of the abdomen/pelvis. Patient was immediately taken to the operating room for emergent repair of aneurysm, and was discharged from the hospital the next day. A huge coronary artery aneurysm with ST-elevation myocardial infarction: A case report and review of literature.

Project description:BackgroundGiant coronary artery aneurysms (GCAA), are rare findings often discovered incidentally in adults. GCAAs are defined by a significant enlargement of coronary arteries, posing a heightened risk of myocardial infarction and thrombosis.Case presentationA 52-year-old male known case of ischemic heart disease (IHD), presented with chest pain and signs of anterior ST-elevation MI (STEMI). He had a history of MI six years ago and was on a medical regimen. Coronary angiography revealed a huge aneurysm (4.8∗8.2mm) in the left anterior descending artery (LAD) and ectasia in other arteries. Surgical management via coronary artery bypass graft (CABG) was pursued, successfully addressing the aneurysm.ConclusionRecent advancements have improved our understanding and imaging capabilities for coronary artery aneurysms (CAAs). Treatment options include medical therapy, percutaneous coronary intervention, or surgery, with decisions tailored to individual cases. Standardized treatment protocols await clarification through further research, including randomized controlled trials.

Giant coronary artery aneurysm of the left main treated with a covered stent: a case report

Project description:Abstract Background Coronary artery aneurysms (CAAs) of the left main represent a small subset of coronary artery disease and are associated with cardiovascular death. Because of its rare entity, large data are lacking and therefore treatment guidelines are missing. Case summary We describe a case of a 56-year-old female with a past medical history of spontaneous dissection of the distal descending left artery (LAD) 6 years before. She presented to our hospital with a non-ST elevation myocardial infarction and a coronary angiogram showed a giant saccular aneurysm of the shaft of the left main coronary artery (LMCA). Given the risk of rupture and distal embolization, the heart team decided to go for a percutaneous approach. Based on a pre-interventional 3D reconstructed CT scan and guided by intravascular ultrasound, the aneurysm was successfully excluded with a 5 mm papyrus-covered stent. At 3-month and 1-year follow-up, the patient is still asymptomatic and repeat angiographies showed full exclusion of the aneurysm and the absence of restenosis in the covered stent. Discussion We describe the successful percutaneous IVUS-guided treatment of a giant LMCA shaft coronary aneurysm with a papyrus-covered stent with an excellent 1-year angiographic follow-up showing no residual filling of the aneurysm and no stent restenosis.

Coronary artery bypass as a treatment of stent dislodgement: A case report.

Project description:IntroductionStent dislodgement is a known complication during coronary angiography. Different methods are used to retrieve it including open heart surgery.Case presentationA 71 year-old male with stable angina was scheduled for elective coronary angiography. Angiography showed two significant stenosis: one in the proximal right coronary artery (RCA) and one in the left anterior descending artery (LAD). Upon deployment of the right coronary stent, it got lodged and the cardiologist was unable to retrieve it. The patient started to experience angina and his ECG showed ST segment elevation in the inferior leads. Emergency CABG was performed.ConclusionStent dislodgement is a rare but serious complication. Most cases are treated by interventional methods; however, CABG is still needed in some cases.

Middle meningeal artery aneurysm: Case report.

Project description:Aneurysms of meningeal middle artery (MMA) are extremely rare. These aneurysms are of two types: true aneurysm and pseudoaneurysm. The true type is usually seen with pathologic conditions. Pseudoaneurysms, on the other hand, are associated with a skull fracture. Epilepsy caused by MMA aneurysm has never been described to our knowledge. We report a case of true aneurysm isolated from MMA revealed by epilepsy.A 57-year-old patient with a history of high blood pressure developed epilepsy which was treated by valproic acid. Initial scalp electroencephalography (EEG) showed seizure activity arising from the right temporal area. Epilepsy had become drug-resistant. Cerebral angiography revealed an aneurysm of the right middle meningeal artery without any other intraparenchymal anomaly. The interrogation did not reveal any history of family aneurysm. The patient underwent surgery with coagulation of the aneurysm and the MMA. The aneurysm was intradural in contact with the temporal cortex, and the surrounding brain tissues were preserved. The operative follow-up was favorable with amelioration of convulsions with a single antiepileptic. We planned to stop antiepileptic treatment according to electroencephalograms.Aneurysms of the MMA are rare. Their mode of revelation by seizures is unusual. The factors of rupture are not known. When isolated, their physiopathology is identical to that of the aneurysms of the Willis polygon. Their management uses the same techniques as for other cerebral aneurysms.

Right hemihepatectomy combined with ligation of the common hepatic artery and gastroduodenal artery for the treatment of intrahepatic HHT: A case report.

Project description:Hereditary haemorrhagic telangiectasia (HHT) is a rare disease that lacks effective treatment. Here, the authors report the case of a 30-year-old woman presenting with abdominal pain accompanied by severe malnutrition. After a definite diagnosis of HHT involvement in the liver, liver transplantation was the first-choice treatment according to the guidelines of HHT. However, the patient firmly refused liver transplantation. Finally, a new type of surgery, right hemihepatectomy combined with ligation of the common hepatic artery and gastroduodenal artery, was performed based on careful study of the case and with the maximum benefit of the patient in mind. Although the patient developed transient liver dysfunction after surgery, she eventually recovered well and continued to be followed up. As far as we know, this is the first report of this kind of surgery for the treatment of intrahepatic HHT.

Case report: a troublesome ophthalmic artery aneurysm.

Project description:Objective and Importance When treating large unruptured ophthalmic artery (OA) aneurysms causing progressive blindness, surgical clipping is still the preferred method because aneurysm sac decompression may relieve optic nerve compression. However, endovascular treatment of OA aneurysms has made important progress with the introduction of stents. Although this development is welcomed, it also makes the choice of treatment strategy less straightforward than in the past, with the potential of missteps. Clinical Presentation A 56-year-old woman presented with a long history of progressive unilateral visual loss and magnetic resonance imaging showing a 20-mm left-sided OA aneurysm. Intervention Because of her long history of very poor visual acuity, we considered her left eye to be irredeemable and opted for endovascular therapy. The OA aneurysms was treated with stent and coils but continued to grow, threatening the contralateral eye. Because she failed internal carotid artery (ICA) balloon test occlusion, we performed a high-flow extracranial-intracranial bypass with proximal ICA occlusion in the neck. However, aneurysm growth continued due to persistent circulation through reversed blood flow in distal ICA down to the OA and the cavernous portion of the ICA. Due to progressive loss of her right eye vision, we surgically occluded the ICA proximal to the posterior communicating artery and excised the coiled, now giant, OA aneurysm. This improved her right eye vision, but her left eye was permanently blind. Conclusion This case report illustrates complications of the endovascular and surgical treatment of a large unruptured OA aneurysm.

Ruptured Aneurysm of the Common Iliac Artery Caused by <i>Brucella melitensis</i>: A Case Report.

Project description:IntroductionBrucella is a genus of aerobic Gram negative bacteria that causes the disease brucellosis. It is considered a zoonotic infection transmitted to humans by ingestion of unpasteurised dairy products. Although aortic involvement is rarely seen, it can be a life threatening complication of this disease. This case report describes a ruptured aneurysm of the common iliac artery (CIA) due to secondary infection by Brucella melitensis.ReportA 79 year old man with a known isolated aneurysm of the CIA presented with acute abdominal pain. Contrast enhanced computed tomography (CT) revealed rupture of the aneurysm. The patient underwent prompt endovascular repair. Several weeks after an uneventful recovery, the patient presented with spiking fever and abdominal discomfort. CT revealed an abscess anterior to the CIA. Blood and pus cultures grew B. melitensis. In recurrent re-admissions, conservative antibiotic therapy proved to be insufficient. Eventually, neo-aorto-iliac system (NAIS) reconstruction using bilateral femoral veins was performed to provide definitive treatment four months after initial presentation.ConclusionAlthough Brucella infected aneurysms are rare, they are associated with life threatening disease. Diagnosing this type of brucellar infection can be challenging owing to the long incubation time needed for blood and tissue cultures. Definitive treatment of these aneurysms often needs open surgery and antibiotics for complete treatment. Vigilant surveillance is required to monitor for post-operative complications such as graft infection, recurrent (false) aneurysm, and abscess formation.

Extrinsic compression of the right coronary artery by a huge aortic wall aneurysm of an aortico-left ventricular tunnel: A case report.

Project description:Aortico-left ventricular tunnel (ALVT) is a rare congenital disease characterised by extracardiac channel communication between the ascending aorta and the ventricle. However, there have been no reports of ALVT compressing the coronary arteries. Here, we report the case of a 57-year-old woman who presented with unstable angina due to right coronary artery compression caused by a giant aneurysmal ALVT. Preoperative imaging failed to accurately diagnose this rare anomaly. Finally, we combined surgical exploration and a three-dimensional reconstruction technique to diagnose ALVT. After surgical resection of the aneurysm and repair of the tunnel, the patient's angina resolved. This case illustrates the importance of recognising extrinsic compression of the coronary artery as a potential cause of angina pectoris. It also highlights that ALVT with atypical anatomical features may require multiple imaging techniques and even surgical exploration to confirm the diagnosis.

Traumatic internal carotid artery aneurysm distant from facial bone fractures treated with a flow diverter stent: A case report.

Project description:BackgroundTraumatic intracranial aneurysms (TICAs) can be fatal if ruptured. We report a case of a TICA, distant from facial bone fractures, successfully treated with flow diverter (FD) before rupture.Case presentationA 20-year-old woman was admitted following a car accident. Initial contrast-enhanced computed tomography revealed traumatic subarachnoid hemorrhage, a 5-mm internal carotid artery aneurysm, LeFort type I + II, and mandibular fracture. On Day 8, she developed oculomotor palsy, and digital subtraction angiography revealed the aneurysm had increased to 12 mm. She started dual antiplatelet therapy (DAPT) following facial fracture repair. On Day 17, FD was placed, resulting in contrast agent stagnation within the aneurysm. Three months postinjury, her neurological symptoms improved.ConclusionScreening for cerebrovascular injury is crucial several days following severe head trauma, even without skull base fractures. Unruptured TICA can be safely treated with FD following DAPT administration.
