Novel Structured Reporting Systems for Theranostic Radiotracers.

ABSTRACT: Standardized reporting is more and more routinely implemented in clinical practice, and such structured reports have a major impact on a large variety of medical fields, such as laboratory medicine, pathology, and, recently, radiology. Notably, the field of nuclear medicine is constantly evolving as novel radiotracers for numerous clinical applications are developed. Thus, framework systems for standardized reporting in this field may increase clinical acceptance of new radiotracers, allow for inter- and intracenter comparisons for quality assurance, and be used in global multicenter studies to ensure comparable results and enable efficient data abstraction. In the last couple of years, several standardized framework systems for PET radiotracers with potential theranostic applications have been proposed. These include systems for prostate-specific membrane antigen-targeted PET agents to diagnose and treat prostate cancer, and systems for somatostatin receptor-targeted PET agents to diagnose and treat neuroendocrine neoplasia. In the present review, the framework systems for these 2 types of cancer will be briefly introduced, followed by an overview of their advantages and limitations. In addition, potential applications will be defined, approaches to validate such concepts will be proposed, and future perspectives will be discussed.

Project description:In recent years, there has been a large-scale dissemination of guidelines in radiology in the form of Reporting & Data Systems (RADS). The use of iodinated contrast media (ICM) has a fundamental role in enhancing the diagnostic capabilities of computed tomography (CT) but poses certain risks. The scope of the present review is to summarize the current role of ICM only in clinical reporting guidelines for CT that have adopted the "RADS" approach, focusing on three specific questions per each RADS: (I) what is the scope of the scoring system; (II) how is ICM used in the scoring system; (III) what is the impact of ICM enhancement on the scoring. We analyzed the original articles for each of the latest versions of RADS that can be used in CT [PubMed articles between January, 2005 and March, 2023 in English and American College of Radiology (ACR) official website]. We found 14 RADS suitable for use in CT out of 28 RADS described in the literature. Four RADS were validated by the ACR: Colonography-RADS (C-RADS), Liver Imaging-RADS (LI-RADS), Lung CT Screening-RADS (Lung-RADS), and Neck Imaging-RADS (NI-RADS). One RADS was validated by the ACR in collaboration with other cardiovascular scientific societies: Coronary Artery Disease-RADS 2.0 (CAD-RADS). Nine RADS were proposed by other scientific groups: Bone Tumor Imaging-RADS (BTI-RADS), Bone‑RADS, Coronary Artery Calcium Data & Reporting System (CAC-DRS), Coronavirus Disease 2019 Imaging-RADS (COVID-RADS), COVID-19-RADS (CO-RADS), Interstitial Lung Fibrosis Imaging-RADS (ILF-RADS), Lung-RADS (LU-RADS), Node-RADS, and Viral Pneumonia Imaging-RADS (VP-RADS). This overview suggests that ICM is not strictly necessary for the study of bones and calcifications (CAC-DRS, BTI-RADS, Bone-RADS), lung parenchyma (Lung-RADS, LU-RADS, COVID-RADS, CO-RADS, VP-RADS and ILF-RADS), and in CT colonography (C-RADS). On the other hand, ICM plays a key role in CT angiography (CAD-RADS), in the study of liver parenchyma (LI-RADS), and in the evaluation of soft tissues and lymph nodes (NI-RADS, Node-RADS). Future studies are needed in order to evaluate the impact of the new iodinated and non-iodinate contrast media, artificial intelligence tools and dual energy CT in the assignment of RADS scores.

Project description:BackgroundEvent reporting systems are an integral part of patient safety programs that continue to remain a challenge primarily due to systems-based barriers. Smartphone use in patient care continues to be an ever-growing facet of medicine and patient care. Combining the problem of event reporting challenges with the modern resource of mobile phones may be used to promote a culture of safety and increase event reporting in the emergency department (ED) and the hospital among residents.MethodsWe introduced a new method of event reporting for emergency medicine (EM) residents in the ED using a mobile messaging application widely used throughout the hospital to facilitate physician communication to report events. Implementation of the intervention occurred in three phases. During the preintervention phase, we retrospectively reviewed EM resident rates of event reporting and administered a survey regarding their attitudes toward the traditional system of event reporting. We then introduced the mobile messaging application-based event reporting system and tracked the number of events recorded during the first 8 months of implementation. Following the intervention, we administered a postintervention survey to the EM residents inquiring about the same metrics that were used in the preintervention survey.ResultsForty EM residents reported a total of 147 events during the 8 months of the intervention phase compared to 12 reports during the prior year as whole, resulting in a 12-fold increase. The postintervention scores ranged from 55 to 73 with a mean (±SD) of 65 (±9). EM resident satisfaction rates and comfort level with the new reporting system increased by 232 and 104%, respectively, and the likelihood of reporting an event increased by 127% (p < 0.02). The time required by EM residents to submit a report also decreased significantly.ConclusionThe implementation of a mobile application to target systems barriers associated with event reporting significantly increased event reporting by EM residents, improved EM resident attitudes about event reporting, and reduced the time required to submit an event, ultimately promoting a culture of safety.
