Elizabethkingia bruuniana Infections in Humans, Taiwan, 2005-2017.

ABSTRACT: Using 16S rRNA and rpoB gene sequencing, we identified 6 patients infected with Elizabethkingia bruuniana treated at E-Da Hospital (Kaohsiung, Taiwan) during 2005-2017. We describe patient characteristics and the molecular characteristics of the E. bruuniana isolates, including their MICs. Larger-scale studies are needed for more robust characterization of this pathogen.

The major clinical manifestations of monoinfection were fever, asthenia, myalgia, rash, and anorexia; laboratory findings included thrombocytopenia and elevated hepatic aminotransferase concentrations. Physicians in China should consider R. japonica infection in at-risk patients.

The trend of DALY of breast, colorectal, oral, and cervical cancers in Taiwan in 2005-2017.

Project description:ObjectivesThis study used the Disability-Adjusted Life Years (DALYs) to quantify the long-term trends for four cancers (oral cancer, colorectal cancer, breast cancer, and cervical cancer) that have undergone cancer screening in Taiwan.MethodsDALYs were calculated as the sum of Years of Life Lost (YLL) due to premature mortality and Years Lived with Disability (YLD). YLLs were determined using cancer-specific mortality data from the Health Promotion Administration (HPA), Ministry of Health and Welfare, based on age-specific life expectancy. YLDs were estimated by combining the incidence rates of the cancers, average disability durations, and disability weights, with data sourced from the Taiwan Cancer Registry. Results were expressed as DALYs per 100,000 population.ResultsThe disease burden has significantly increased over the past 12 years. Oral cancer rose from 263 to 368 DALYs per 100,000 population (40 % increase), colorectal cancer from 343 to 563 DALYs (64 % increase), and breast cancer from 446 to 782 DALYs (75 % increase), while the burden of cervical cancer decreased from 168 to 147 DALYs per 100,000 population from 2010 to 2017, showing a 13 % reduction. At the cancer stages, the impact of YLDs was mostly at cancer stage IV (oral cancer), cancer stage 0 (colorectal and cervical cancer), and stage I (breast cancer).ConclusionOral cancer increased by 40 %, colorectal cancer by 64 %, and breast cancer by 75 % from 2005 to 2017, while cervical cancer decreased by 13 % between 2010 and 2017. YLD contributions were highest in stage IV for oral cancer, stage 0 for colorectal and cervical cancers, and stage I for breast cancer. The highest DALYs consistently occurred in the 50-69 age group across all cancer types, highlighting the significant burden on middle-aged populations.

Clonality of Fluconazole-Nonsusceptible Candida tropicalis in Bloodstream Infections, Taiwan, 2011-2017.

Project description:Candida tropicalis is the leading cause of non-C. albicans candidemia in tropical Asia and Latin America. We evaluated isolates from 344 patients with an initial episode of C. tropicalis candidemia. We found that 58 (16.9%) patients were infected by fluconazole-nonsusceptible (FNS) C. tropicalis with cross resistance to itraconazole, voriconazole, and posaconazole; 55.2% (32/58) of patients were azole-naive. Multilocus sequence typing analysis revealed FNS isolates were genetically closely related, but we did not see time- or place-clustering. Among the diploid sequence types (DSTs), we noted DST225, which has been reported from fruit in Taiwan and hospitals in Beijing, China, as well as DST376 and DST505-7, which also were reported from hospitals in Shanghai, China. Our findings suggest cross-boundary expansion of FNS C. tropicalis and highlight the importance of active surveillance of clinical isolates to detect dissemination of this pathogen and explore potential sources in the community.

Multicenter Study of Cronobacter sakazakii Infections in Humans, Europe, 2017.

Project description:Cronobacter sakazakii has been documented as a cause of life-threating infections, predominantly in neonates. We conducted a multicenter study to assess the occurrence of C. sakazakii across Europe and the extent of clonality for outbreak detection. National coordinators representing 24 countries in Europe were requested to submit all human C. sakazakii isolates collected during 2017 to a study center in Austria. Testing at the center included species identification by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry, subtyping by whole-genome sequencing (WGS), and determination of antimicrobial resistance. Eleven countries sent 77 isolates, including 36 isolates from 2017 and 41 historical isolates. Fifty-nine isolates were confirmed as C. sakazakii by WGS, highlighting the challenge of correctly identifying Cronobacter spp. WGS-based typing revealed high strain diversity, indicating absence of multinational outbreaks in 2017, but identified 4 previously unpublished historical outbreaks. WGS is the recommended method for accurate identification, typing, and detection of this pathogen.

Transfusion-transmissible infections among Serbian blood donors: declining trends over the period 2005-2017.

Project description:BackgroundThe mass migrations experienced by the Western Balkans in the past decades have significantly changed the demographic structures and have probably altered the prevalence of transfusion-transmitted infections (TTIs) among blood donors. However, data on the prevalence of TTIs in the Western Balkans countries remain incomplete. This study reports the prevalence of TTIs among blood donors in Serbia in the period 2005-2017.Materials and methodsBetween January 2005 and December 2017, in the four largest Serbian transfusion centres, mandatory serology tests for screening HBV, HCV, HIV and syphilis infection were used for all blood donations.ResultsOf the total of 1,660,019 blood donations made, 3,377 (0.203%) were positive for one of the TTIs: 1,440 (0.087%), 1,055 (0.064%), 215 (0.013%), and 667 (0.040%) were positive for HBV, HCV, HIV and syphilis, respectively. Serbia showed a declining trend of prevalence of HBV and HCV infection, while prevalence of HIV and syphilis remained unchanged. Prevalence of TTIs varied between different transfusion centres and showed a north-to-south upward trend.DiscussionThe reported prevalence of TTIs among blood donors in Serbia was low and continued to follow a declining trend over the period of study.

Candidatus Dirofilaria Hongkongensis Infections in Humans During 2005 to 2020, in Kerala, India.

Project description:We tried to determine the epidemiology and species of human dirofilariasis observed at two tertiary care hospitals in Kerala. We searched the hospital database to identify cases of dirofilariosis from January 2005 to March 2020. Along with human isolates, one dog Dirofilaria isolate was also subjected to PCR and sequencing of pan filarial primers cytochrome oxidase subunits 1 and 12S rDNA. We documented 78 cases of human dirofilariosis. The orbit, eyelid, and conjunctiva were the most commonly affected sites. Molecular characterization identified one dog and five human isolates as Candidatus Dirofilaria Hongkongensis. A rare case of subconjunctival infestation by B. malayi was also documented. Human dirofilariosis is a public health problem in the state of Kerala in India, and it is mostly caused by Candidatus Dirofilaria Hongkongensis. We propose that all diroifilaria isolates are subjected to sequencing for identification.

Self-reported myopia in Taiwan: 2005 Taiwan National Health Interview Survey.

Project description:PURPOSE: To determine the prevalence of self-reported myopia nationwide in Taiwan and its association with degrees of urbanization and education levels. METHODS: Data were obtained from the 2005 Taiwan National Health Interview Survey, a nationwide survey using multistage stratified systematic sampling. The presence of myopia, current residential areas, and education levels were ascertained by a structured questionnaire in participants ≥ 12 years of age. RESULTS: A total of 20,609 eligible persons were included in this study. The overall weighted prevalence of myopia in Taiwan was 46.7% (95% confidence interval: 45.9, 47.5%). The prevalence of myopia for persons aged 12-19, 20-39, 40-64, and ≥ 65 years was 70.3%, 65.4%, 30.4%, and 5.6%, respectively. Women had significantly higher rates of myopia than men for persons younger than 40 years of age (P<0.001). Myopia was significantly associated with both higher degrees of urbanization of current residential areas and higher education levels (both P<0.001). In young adult and adult groups, the effect of education levels on myopia was stronger than that of degrees of urbanization. CONCLUSION: The study provides a nation-wide prevalence data on myopia in Taiwan. Both degrees of urbanization and education levels are risk factors for myopia.

Macrolide-Resistant Mycoplasma pneumoniae Infections among Children before and during COVID-19 Pandemic, Taiwan, 2017-2023.

Project description:Before the COVID-19 pandemic, Mycoplasma pneumoniae infections emerged during spring to summer yearly in Taiwan, but infections were few during the pandemic. M. pneumoniae macrolide resistance soared to 85.7% in 2020 but declined to 0% during 2022-2023. Continued molecular surveillance is necessary to monitor trends in macrolide-resistant M. pneumoniae.

Comparison of Clinical Manifestations, Antimicrobial Susceptibility Patterns, and Mutations of Fluoroquinolone Target Genes between Elizabethkingia meningoseptica and Elizabethkingia anophelis Isolated in Taiwan.

Project description:Elizabethkingiameningoseptica and Elizabethkingia anophelis are two major pathogens in the genus Elizabethkingia. Studies have revealed that Elizabethkingia anophelis is frequently misidentified as E. meningoseptica. Therefore, our aim was to explore the clinical and molecular differences between these two species. The database of a clinical microbiology laboratory in a university-affiliated hospital of Taiwan was searched to identify patients with Elizabethkingia infections between January 2005 and June 2018. Species were reidentified using 16S ribosomal RNA gene sequencing. Twenty E. meningoseptica and 72 E. anophelis samples were collected from consecutive patients. E. meningoseptica was significantly more frequently isolated from the cerebrospinal fluid than was E. anophelis. The most susceptible antibiotic for all Elizabethkingia isolates was minocycline (91.3%), followed by levofloxacin (52.2%), tigecycline (23.9%), and piperacillin tazobactam (23.9%). Compared with E. anophelis, E. meningoseptica was significantly less susceptible to piperacillin tazobactam, minocycline, and levofloxacin. Regarding nonsynonymous substitutions in the quinolone-resistance determining regions of DNA gyrase, six sites were recognized in E. meningoseptica and one site was recognized in E. anophelis. E. meningoseptica had a significantly higher rate of fluoroquinolone target gene mutations than did E. anophelis. Because of less susceptibility to multiple antibiotics than E. anophelis, empirical antimicrobial therapy of E. meningoseptica should be more rigorous.
