Construction and preliminary evaluation of the inpatient glycemic control questionnaire (IGCQ): a survey tool assessing perceptions and knowledge of resident physicians.

ABSTRACT:

Background

Uncontrolled hyperglycemia in hospitalized patients, with or without diabetes mellitus, is associated with many adverse outcomes. Resident physicians are the primary managers of inpatient glycemic control (IGC) in many academic and community medical centers; however, no validated survey tools related to their perceptions and knowledge of IGC are currently available. As identification of common barriers to successful IGC amongst resident physicians may help foster better educational interventions (ultimately leading to improvements in IGC and patient care), we sought to construct and preliminarily evaluate such a survey tool.

Methods

We developed the IGC questionnaire (IGCQ) by using previously published but unvalidated survey tools related to physician perspectives on inpatient glycemic control as a framework. We administered the IGCQ to a cohort of resident physicians from the University of Mississippi Medical Center, University of Louisville, Emory University, and the University of Virginia. We then used classical test theory and Rasch Partial Credit Model analyses to preliminarily evaluate and revise the IGCQ. The final survey tool contains 16 total items and three answer-choice categories for most items.

Results

Two hundred forty-six of 438 (56.2%) eligible resident physicians completed the IGCQ during various phases of development.

Conclusions

We constructed and preliminarily evaluated the IGCQ, a survey tool that may be useful for future research into resident physician perceptions and knowledge of IGC. Future studies could seek to externally validate the IGCQ and then utilize the survey tool in pre- and post-intervention assessments.

Project description:BackgroundResidency training, a crucial part of postgraduate medical education, allows doctors to engage in research, enhancing their critical thinking and evidence-based practice skills. However, various barriers often impede this process. This study aims to evaluate Palestinian resident doctors' perceptions, practices, and obstacles regarding medical research.MethodologyA cross-sectional study was conducted from October to December 2023, focusing on resident doctors in West Bank training centers. A sample size of 250 residents was selected. Data were collected via a self-administered questionnaire that covered demographics, attitudes toward research, barriers to conducting research, and research practices. Data analysis included descriptive statistics, chi-squared tests, and multivariate analysis using binary logistic regression.ResultsOut of 290 residents invited, 256 responded, resulting in an 88.3% response rate. Positive attitudes towards research were common, with 79.7% supporting mandatory teaching of research methodology and 81.3% recognizing the importance of publishing research findings. Nevertheless, 53.9% of residents expressed concerns about the additional burden imposed by research. Research engagement was moderate, with 52.0% involved in research projects, although only 40.6% had published in peer-reviewed journals. Significant barriers identified were inadequate funding (71.9%) and insufficient time for research (71.5%). The presence of a research mentor significantly increased the likelihood of research engagement (aOR: 11.8, 95% CI: 6.2-22.5).ConclusionPalestinian resident doctors exhibit a positive attitude towards medical research, yet significant barriers hinder their participation. Mentoring, integrating research training into residency programs, and allocating the time and resources needed for research activities are all essential strategies to increase resident research engagement and productivity. These efforts can advance healthcare by promoting evidence-based practices, addressing local health challenges, and strengthening systems through tailored interventions and collaborative efforts.

Project description:We created a serious game to teach first year anesthesiology (CA-1) residents to perform general anesthesia for cesarean delivery. We aimed to investigate resident knowledge gains after playing the game and having received one of 2 modalities of debriefing. We report on the development and validation of scores from parallel test forms for criterion-referenced interpretations of resident knowledge. The test forms were intended for use as pre- and posttests for the experiment. Validation of instruments measuring the study's primary outcome was considered essential for adding rigor to the planned experiment, to be able to trust the study's results. Parallel, multiple-choice test forms development steps included: (1) assessment purpose and population specification; (2) content domain specification and writing/selection of items; (3) content validation by experts of paired items by topic and cognitive level; and (4) empirical validation of scores from the parallel test forms using Classical Test Theory (CTT) techniques. Field testing involved online administration of 52 shuffled items from both test forms to 24 CA-1's, 21 second-year anesthesiology (CA-2) residents, 2 fellows, 1 attending anesthesiologist, and 1 of unknown rank at 3 US institutions. Items from each form yielded near-normal score distributions, with similar medians, ranges, and standard deviations. Evaluations of CTT item difficulty (item p values) and discrimination (D) indices indicated that most items met assumptions of criterion-referenced test design, separating experienced from novice residents. Experienced residents performed better on overall domain scores than novices (P < .05). Kuder-Richardson Formula 20 (KR-20) reliability estimates of both test forms were above the acceptability cut of .70, and parallel forms reliability estimate was high at .86, indicating results were consistent with theoretical expectations. Total scores of parallel test forms demonstrated item-level validity, strong internal consistency and parallel forms reliability, suggesting sufficient robustness for knowledge outcomes assessments of CA-1 residents.
