Project description:BackgroundDosimetry in ultra-high dose rate (UHDR) electron beamlines poses a significant challenge owing to the limited usability of standard dosimeters in high dose and high dose-per-pulse (DPP) applications.PurposeIn this study, Al2 O3 :C nanoDot optically stimulated luminescent dosimeters (OSLDs), single-use powder-based LiF:Mg,Ti thermoluminescent dosimeters (TLDs), and Gafchromic EBT3 film were evaluated at extended dose ranges (up to 40 Gy) in conventional dose rate (CONV) and UHDR beamlines to determine their usability for calibration and dose verification in the setting of FLASH radiation therapy.MethodsOSLDs and TLDs were evaluated against established dose-rate-independent Gafchromic EBT3 film with regard to the potential influence of mean dose rate, instantaneous dose rate, and DPP on signal response. The dosimeters were irradiated at CONV or UHDR conditions on a 9-MeV electron beam. Under UHDR conditions, different settings of pulse repetition frequency (PRF), pulse width (PW), and pulse amplitude were used to characterize the individual dosimeters' response in order to isolate their potential dependencies on dose, dose rate, and DPP.ResultsThe OSLDs, TLDs, and Gafchromic EBT3 film were found to be suitable at a dose range of up to 40 Gy without any indication of saturation in signal. The response of OSLDs and TLDs in UHDR conditions were found to be independent of mean dose rate (up to 1440 Gy/s), instantaneous dose rate (up to 2 MGy/s), and DPP (up to 7 Gy), with uncertainties on par with nominal values established in CONV beamlines (± 4%). In cross-comparing the response of OSLDs, TLDs and Gafchromic film at dose rates of 0.18-245 Gy/s, the coefficient of variation or relative standard deviation in the measured dose between the three dosimeters (inter-dosimeter comparison) was found to be within 2%.ConclusionsWe demonstrated the dynamic range of OSLDs, TLDs, and Gafchromic film to be suitable up to 40 Gy, and we developed a protocol that can be used to accurately translate the measured signal in each respective dosimeter to dose. OSLDs and powdered TLDs were shown to be viable for dosimetric measurement in UHDR beamlines, providing dose measurements with accuracies on par with Gafchromic EBT3 film and their concurrent use demonstrating a means for redundant dosimetry in UHDR conditions.

Project description:IntroductionStudies have suggested that optically stimulated luminescent dosimeters (OSLDs) can be used for in vivo dosimetry of intensity-modulated radiation therapy (IMRT) and volumetric-modulated arc therapy (VMAT). Clinical uncertainties such as placement error have not been thoroughly investigated. The purpose of this work was to measure OSLD placement error in a clinical sample and analyze its dosimetric impact.Methods and materialsThe analysis consisted of three parts: first, quantification of placement error in a clinical sample of 128 patients yielding 293 cone-beam CT (CBCT) with visible OSLDs registered to the treatment plan; Second, correlation of placement error and clinical OSLD measurements; third, simulation of dosimeter placement in the treatment plan and correlation of recalculated dose with placement error.ResultsIn the first analysis, average placement error was 9.7 ± 9.5 mm. In the second analysis, placement error and measured-to-planned dose agreement yielded no correlation (R2 = 0.02) for a subsample of 77 CBCTs of 55 head-and-neck patients. Average placement error was 7.0 ± 6.0 mm. Several factors, including image-guided shifts, introduced uncharacterized uncertainty to the measured-to-planned dose agreement. The third analysis isolated placement error from these other effects. Average dosimetric error was -2.4 ± 19.3%. Simulated dosimetric impact was weakly correlated with placement error (R2 = 0.39). Removing outliers reduced the average dosimetric error to -2.1 ± 10.9%, marginally improving the correlation (R2 = 0.44).ConclusionPlacement error can substantially impact measured-to-planned dose agreement of OSLDs in high gradient regions, demonstrating the criticality of accurate dosimeter placement for IMRT and VMAT treatments.

Project description:To determine the precision and accuracy of CTDI(100) measurements made using commercially available optically stimulated luminescent (OSL) dosimeters (Landaur, Inc.) as beam width, tube potential, and attenuating material were varied.One hundred forty OSL dosimeters were individually exposed to a single axial CT scan, either in air, a 16-cm (head), or 32-cm (body) CTDI phantom at both center and peripheral positions. Scans were performed using nominal total beam widths of 3.6, 6, 19.2, and 28.8 mm at 120 kV and 28.8 mm at 80 kV. Five measurements were made for each of 28 parameter combinations. Measurements were made under the same conditions using a 100-mm long CTDI ion chamber. Exposed OSL dosimeters were returned to the manufacturer, who reported dose to air (in mGy) as a function of distance along the probe, integrated dose, and CTDI(100).The mean precision averaged over 28 datasets containing five measurements each was 1.4% ± 0.6%, range = 0.6%-2.7% for OSL and 0.08% ± 0.06%, range = 0.02%-0.3% for ion chamber. The root mean square (RMS) percent differences between OSL and ion chamber CTDI(100) values were 13.8%, 6.4%, and 8.7% for in-air, head, and body measurements, respectively, with an overall RMS percent difference of 10.1%. OSL underestimated CTDI(100) relative to the ion chamber 21∕28 times (75%). After manual correction of the 80 kV measurements, the RMS percent differences between OSL and ion chamber measurements were 9.9% and 10.0% for 80 and 120 kV, respectively.Measurements of CTDI(100) with commercially available CT OSL dosimeters had a percent standard deviation of 1.4%. After energy-dependent correction factors were applied, the RMS percent difference in the measured CTDI(100) values was about 10%, with a tendency of OSL to underestimate CTDI relative to the ion chamber. Unlike ion chamber methods, however, OSL dosimeters allow measurement of the radiation dose profile.

Project description:Chest wall radiation therapy treatment delivery was monitored using a 5 mm thick radiochromic poly(vinyl alcohol) cryogel that also provided buildup material. The cryogels were used to detect positioning errors and measure the impact of shifts for a chest wall treatment that was delivered to a RANDO phantom. The phantom was shifted by ± 2, ± 3, and ± 5 mm from the planned position in the anterior/posterior (A/P) direction; these shifts represent setup errors and the uncertainty associated with lung filling during breath-hold. The two-dimensional absolute dose distributions measured in the cryogel at the planned position were compared with the distributions at all shifts from this position using gamma analysis (3%/3 mm, 10% threshold). For shifts of ± 2, ± 3, and ± 5 mm the passing rates ranged from 94.3% to 95.6%, 74.0% to 78.8%, and 17.5% to 22.5%, respectively. These results are consistent with the same gamma analysis performed on dose planes calculated in the middle of the cryogel and on the phantom surface using our treatment plan-ning system, which ranged from 94.3% to 95.0%, 76.8% to 77.9%, and 23.5% to 24.3%, respectively. The Pinnacle dose planes were then scaled empirically and compared to the cryogel measurements. Using the same gamma metric, the pass rates ranged from 97.0% to 98.4%. The results of this study suggest that cryogels may be used as both a buildup material and to evaluate errors in chest wall treat-ment positioning during deep-inspiration breath-hold delivery. The cryogels are sensitive to A/P chest wall shifts of less than 3 mm, which potentially allows for the detection of clinically relevant errors.

Project description:The risk of exposure of the general public or military personnel to high levels of ionizing radiation from nuclear weapons or radiological accidents is a dire national security matter. The development of advanced molecular biodosimetry methods, those that measure biological response, such as transcriptomics, to screen large populations of radiation-exposed victims is key to improving survival outcomes during radiological mass casualty scenarios. In this study, nonhuman primates were exposed to either 12.0 Gy cobalt-60 gamma (total-body irradiation, TBI) or X-ray (partial-body irradiation, PBI) 24 h after administration of a potential radiation medical countermeasure, gamma-tocotrienol (GT3). Changes in the jejunal transcriptomic profiles in GT3-treated and irradiated animals were compared to healthy controls to assess the extent of radiation damage. No major effect of GT3 on radiation-induced transcriptome at this radiation dose was identified. About 80% of the pathways with a known activation or repression state were commonly observed between both exposures. Several common pathways activated due to irradiation include FAK signaling, CREB signaling in the neurons, phagosome formation, and G-protein coupled signaling pathway. Sex-specific differences associated with excessive mortality among irradiated females were identified in this study, including Estrogen receptor signaling. Differential pathway activation was also identified across PBI and TBI, pointing towards altered molecular response for different degrees of bone marrow sparing and radiation doses. This study provides insight into radiation-induced changes in jejunal transcriptional profiles, supporting the investigation for the identification of biomarkers for radiation injury and countermeasure efficacy.

Project description:Antecubital access for right heart catheterization (RHC) is a widespread technique, even though there is a need to clarify if there are differences and significant advantages compared to proximal vein access. To pursue this issue, we retrospectively identified patients who underwent RHC in our clinic over a 7 year period (between January 2015 and December 2022). We revised demographic, anthropometric, and procedural data, including the fluoroscopy time, the radiation exposure, and the use of guidewires. The presence of any complications was also assessed. In patients with antecubital access, the fluoroscopy time and the radiation exposure were lower compared to proximal vein access (6 vs. 3 min, mean difference of 2 min, CI 95% 1-4 min, p < 0.001 and 61 vs. 30 cGy/m2, mean difference 64 cGy/m2, CI 95% 50-77, p < 0.001). The number of patients requiring the use of at least one guidewire was lower in the group undergoing RHC through antecubital access compared to proximal vein access (55% vs. 43%, p = 0.01). The feasibility was optimal, as just 0.9% of procedures switched from antecubital to femoral access, with a negligible rate of complications. The choice of the antecubital site exhibits advantages, e.g., a shorter fluoroscopy time, a reduced radiation dose, and a lower average number of guidewires used compared to proximal vein access.

Project description:IntroductionImaging fusion technology is promising as it is radiation and contrast sparing. Herein, we compare conventional biplane angiography to multimodality image fusion with live fluoroscopy using two-dimensional (2D)-three-dimensional (3D) registration (MMIF2D-3D) and assess MMIF2D-3D impact on radiation exposure and contrast volume during cardiac catheterization of patients with congenital heart disease (CHD).MethodsWe matched institutional MMIF2D-3D procedures and controls according to patient characteristics (body mass index, age, and gender) and the seven procedure-type subgroups. Then, we matched the number of tests and controls per subgroup using chronological ordering or propensity score matching. Subsequently, we combined the matched subgroups into larger subgroups of similar procedure type, keeping subgroups with at least 10 test and 10 control cases. Air kerma (AK) and dose area product (DAP) were normalized by body weight (BW), product of body weight and fluoroscopy time (BW × FT), or product of body weight and number of frames (BW × FR), and stratified by acquisition plane and irradiation event type (fluoroscopy or acquisition). Three senior interventionists evaluated the relevance of MMIF2D-3D (5-point Likert scale).ResultsThe Overall group consisted of 54 MMIF2D-3D cases. The combined and matched subgroups were pulmonary artery stenting (StentPUL), aorta angioplasty (PlastyAO), pulmonary artery angioplasty (PlastyPUL), or a combination of the latter two (Plasty). The FT of the lateral plane reduced significantly by 69.6% for the Overall MMIF2D-3D population. AKBW and DAPBW decreased, respectively, by 43.9% and 39.3% (Overall group), 49.3% and 54.9% (PlastyAO), and 36.7% and 44.4% for the Plasty subgroup. All the aforementioned reductions were statistically significant except for DAPBW in the Overall and Plasty (sub)groups. The decrease of AKBW and DAPBW in the StentPUL and PlastyPUL subgroups was not statistically significant. The decrease in the median values of the weight-normalized contrast volume (CMCBW) in all five subgroups was not significant. Cardiologists considered MMIF2D-3D very useful with a median score of 4.ConclusionIn our institution, MMIF2D-3D overall enabled significant AKBW reduction during the catheterization of CHD patients and was mainly driven by reduced FT in the lateral plane. We observed significant AKBW reduction in the Plasty and PlastyAO subgroups and DAPBW reduction in the PlastyAO subgroup. However, the decrease in CMCBW was not significant.

Project description:Although image quality (IQ) is the ultimate goal for accurate diagnosis and treatment, minimizing radiation dose is equally important. This is especially true when pediatric patients are examined, because their sensitivity to radiation-induced cancer is two to three times greater than that of adults. DoseWise is an ALARA-based philosophy within Philips Medical Systems that is active at every level of product design. It encompasses a set of techniques, programs and practices that ensures optimal IQ while protecting people in the X-ray environments. DoseWise methods include management of the X-ray beam, less radiation-on time and more dose information for the operator. Smart beam management provides automatic customization of the X-ray beam spectrum, shape, and pulse frequency. The Philips-patented grid-controlled fluoroscopy (GCF) provides grid switching of the X-ray beam in the X-ray tube instead of the traditional generator switching method. In the examination of pediatric patients, DoseWise technology has been scientifically documented to reduce radiation dose to <10% of the dose of traditional continuous fluoroscopy systems. The result is improved IQ at a significantly lower effective dose, which contributes to the safety of patients and staff.

Project description:BackgroundAs more total hip arthroplasty (THA) procedures continue to be performed and concomitant intraoperative fluoroscopic utilization continues to increase, it is important for surgeons to critically analyze this practice during their procedures. The direct anterior approach (DAA) is a popular technique, but often requires the use of fluoroscopy for evaluation of component positioning and implantation. Computed tomography (CT)-based robotic-assisted THA (RA-THA) may represent an alternative to conventional procedures, which may allow the avoidance of the use of intraoperative fluoroscopy to be avoided and instead rely on a preoperative CT scan of the patient and intraoperative mapping. The purpose of this study was to evaluate the difference in radiation exposure from CT-based RA-THA DAA vs fluoroscopy-guided conventional THA DAA based on values from a cadaver study.MethodsThere were 2 surgeons who had previous extensive experience with the DAA, as well as conventional and RA-THA. They completed bilateral direct anterior THAs on 6 cadaver specimens. For each cadaver, a conventional THA with fluoroscopy was performed on the first hip, and a robotically assisted THA without fluoroscopy was performed on the contralateral hip. For all robotic-assisted cases, a preoperative 3-dimensional CT was performed. The radiation dosage from the preoperative scan was acquired through the radiation dose structured reports. Radiation exposure dosage for intraoperative fluoroscopy required tabulation of the number of fluoroscopy shots performed for each case as well as the length of time of exposure. This time was then converted to an effective radiation dose based on accepted standards, and this was multiplied by the average number of fluoroscopy shots per case to determine the average dosage per case.ResultsNo fluoroscopic images were taken during the RA-THA using the DAA. There was a mean of 21 ± 8.9 fluoroscopic shots taken during each fluoroscopy-guided conventional THA using the DAA. The minimum number of fluoroscopic shots was 9, with a maximum of 31. Total radiation exposure ranged from 300 to 1033 mrem, with an average of 700 mrem. The average radiation exposure for a hip CT scan was 289 mrem.ConclusionsIn this cadaver-based study, the use of a CT-based RA-THA DAA eliminated the need for intraoperative fluoroscopy, removing the concern of radiation exposure to the surgeon and surgical staff. The CT-based RA-THA DAA does require a preoperative CT scan of the patient; however, the radiation dosage to the patient was equivalent to 8.7 intraoperative fluoroscopy shots. As more surgeons, patients, and surgical staff are exposed to an increasing number of orthopaedic procedures that require ionizing radiation to generate images, such as fluoroscopy and CT, it is important to consider the radiation dosage for all individuals and understand the positives and negatives of each treatment. We must consider the overall long-term health impacts of continuous fluoroscopy vs a one-time CT scan. This is of particular importance to the surgeon and surgical staff who may be exposed to cumulative doses during multiple procedures.

Project description:Exposure to electromagnetic radiation (EMR) from the sun and from artificial lighting systems represents a modifiable risk factor for a broad range of health conditions including skin cancer, skin aging, sleep and mood disorders, and retinal damage. Technologies for personalized EMR dosimetry could guide lifestyles toward behaviors that ensure healthy levels of exposure. Here, we report a millimeter-scale, ultralow-power digital dosimeter platform that provides continuous EMR dosimetry in an autonomous mode at one or multiple wavelengths simultaneously, with time-managed wireless, long-range communication to standard consumer devices. A single, small button cell battery supports a multiyear life span, enabled by the combined use of a light-powered, accumulation mode of detection and a light-adaptive, ultralow-power circuit design. Field studies demonstrate single- and multimodal dosimetry platforms of this type, with a focus on monitoring short-wavelength blue light from indoor lighting and display systems and ultraviolet/visible/infrared radiation from the sun.
