Plasma microRNA Profiling Reveals Novel Biomarkers of Epicardial Adipose Tissue: A Multidetector Computed Tomography Study.

ABSTRACT: Epicardial adipose tissue (EAT) constitutes a novel parameter for cardiometabolic risk assessment and a target for therapy. Here, we evaluated for the first time the plasma microRNA (miRNA) profile as a source of biomarkers for epicardial fat volume (EFV). miRNAs were profiled in plasma samples from 180 patients whose EFV was quantified using multidetector computed tomography. In the screening study, 54 deregulated miRNAs were identified in patients with high EFV levels (highest tertile) compared with matched patients with low EFV levels (lowest tertile). After filtering, 12 miRNAs were selected for subsequent validation. In the validation study, miR-15b-3p, miR-22-3p, miR-148a-3p miR-148b-3p and miR-590-5p were directly associated with EFV, even after adjustment for confounding factors (p value < 0.05 for all models). The addition of miRNA combinations to a model based on clinical variables improved the discrimination (area under the receiver-operating-characteristic curve (AUC) from 0.721 to 0.787). miRNAs correctly reclassified a significant proportion of patients with an integrated discrimination improvement (IDI) index of 0.101 and a net reclassification improvement (NRI) index of 0.650. Decision tree models used miRNA combinations to improve their classification accuracy. These results were reproduced using two proposed clinical cutoffs for epicardial fat burden. Internal validation corroborated the robustness of the models. In conclusion, plasma miRNAs constitute novel biomarkers of epicardial fat burden.

Project description:BackgroundEpicardial adipose tissue (EAT) volume is a marker of visceral obesity that can be measured in coronary computed tomography angiograms (CCTA). The clinical value of integrating this measurement in routine CCTA interpretation has not been documented.ObjectivesThis study sought to develop a deep-learning network for automated quantification of EAT volume from CCTA, test it in patients who are technically challenging, and validate its prognostic value in routine clinical care.MethodsThe deep-learning network was trained and validated to autosegment EAT volume in 3,720 CCTA scans from the ORFAN (Oxford Risk Factors and Noninvasive Imaging Study) cohort. The model was tested in patients with challenging anatomy and scan artifacts and applied to a longitudinal cohort of 253 patients post-cardiac surgery and 1,558 patients from the SCOT-HEART (Scottish Computed Tomography of the Heart) Trial, to investigate its prognostic value.ResultsExternal validation of the deep-learning network yielded a concordance correlation coefficient of 0.970 for machine vs human. EAT volume was associated with coronary artery disease (odds ratio [OR] per SD increase in EAT volume: 1.13 [95% CI: 1.04-1.30]; P = 0.01), and atrial fibrillation (OR: 1.25 [95% CI: 1.08-1.40]; P = 0.03), after correction for risk factors (including body mass index). EAT volume predicted all-cause mortality (HR per SD: 1.28 [95% CI: 1.10-1.37]; P = 0.02), myocardial infarction (HR: 1.26 [95% CI:1.09-1.38]; P = 0.001), and stroke (HR: 1.20 [95% CI: 1.09-1.38]; P = 0.02) independently of risk factors in SCOT-HEART (5-year follow-up). It also predicted in-hospital (HR: 2.67 [95% CI: 1.26-3.73]; P ≤ 0.01) and long-term post-cardiac surgery atrial fibrillation (7-year follow-up; HR: 2.14 [95% CI: 1.19-2.97]; P ≤ 0.01).ConclusionsAutomated assessment of EAT volume is possible in CCTA, including in patients who are technically challenging; it forms a powerful marker of metabolically unhealthy visceral obesity, which could be used for cardiovascular risk stratification.

Project description:ObjectivesRecently, epicardial adipose tissue (EAT) assessed by CT was identified as an independent mortality predictor in patients with various cardiac diseases. Our goal was to develop a deep learning pipeline for robust automatic EAT assessment in CT.MethodsContrast-enhanced ECG-gated cardiac and thoraco-abdominal spiral CT imaging from 1502 patients undergoing transcatheter aortic valve replacement (TAVR) was included. Slice selection at aortic valve (AV)-level and EAT segmentation were performed manually as ground truth. For slice extraction, two approaches were compared: A regression model with a 2D convolutional neural network (CNN) and a 3D CNN utilizing reinforcement learning (RL). Performance evaluation was based on mean absolute z-deviation to the manually selected AV-level (Δz). For tissue segmentation, a 2D U-Net was trained on single-slice images at AV-level and compared to the open-source body and organ analysis (BOA) framework using Dice score. Superior methods were selected for end-to-end evaluation, where mean absolute difference (MAD) of EAT area and tissue density were compared. 95% confidence intervals (CI) were assessed for all metrics.ResultsSlice extraction using RL was slightly more precise (Δz: RL 1.8 mm (95% CI: [1.6, 2.0]), 2D CNN 2.0 mm (95% CI: [1.8, 2.3])). For EAT segmentation at AV-level, the 2D U-Net outperformed BOA significantly (Dice score: 2D U-Net 91.3% (95% CI: [90.7, 91.8]), BOA 85.6% (95% CI: [84.7, 86.5])). The end-to-end evaluation revealed high agreement between automatic and manual measurements of EAT (MAD area: 1.1 cm2 (95% CI: [1.0, 1.3]), MAD density: 2.2 Hounsfield units (95% CI: [2.0, 2.5])).ConclusionsWe propose a method for robust automatic EAT assessment in spiral CT scans enabling opportunistic evaluation in clinical routine.Critical relevance statementSince inflammatory changes in epicardial adipose tissue (EAT) are associated with an increased risk of cardiac diseases, automated evaluation can serve as a basis for developing automated cardiac risk assessment tools, which are essential for efficient, large-scale assessment in opportunistic settings.Key pointsDeep learning methods for automatic assessment of epicardial adipose tissue (EAT) have great potential.
