Depression Is Associated With the Absence of Sex Differences in the 2D:4D Ratio of the Right Hand.

ABSTRACT: The 2D:4D digit ratio reflects prenatal testosterone relative to estradiol exposure of a developing embryo. Higher levels of prenatal testosterone have been related to lower 2D:4D ratios. In addition, higher 2D:4D ratios have been associated with female gender, neuroticism, and depression severity. Therefore, the present study investigated whether 2D:4D ratios differ between inpatients with major depression and matched healthy controls and whether 2D:4D ratios correlate with depression severity. We examined 139 inpatients diagnosed with major depression according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) criteria and 137 healthy controls regarding 2D:4D ratios of both hands and BDI-II scores. While we observed significant sex differences in the 2D:4D ratio of the right hand in the healthy control group (women on average showed a significantly higher 2D:4D ratio), no such differences were found in the group of depressed patients. The 2D:4D digit ratios did not correlate with depression severity even when examined for group and sex separately. We conclude that major depression is associated with an absence of sex differences in the 2D:4D ratio.

Project description:Abstract Background and Aims There are sex differences in blood cell count and hemoglobin (HGB) in adulthood due to differences in the levels of circulating sex hormones. The second‐to‐fourth digit ratio (2D:4D) is the putative marker of prenatal hormone exposure. The 2D:4D or the right‐left difference (Dr‐l) are sexually dimorphic and are correlates of sex hormones in adulthood. The study sought to determine whether sex differences in adult blood cell count and HGB can be partly explained by the 2D:4D or Dr‐l. Methods The study was cross‐sectional between June and December 2021 at the University for Development Studies. The study involved 207 healthy participants (females = 113) aged from 18 to 32 years. The right‐hand (2D:4DR), and the left‐hand (2D:4DL) digit ratio and their difference (Dr‐l) were measured using Computer‐assisted analysis. Blood cell count, HGB, testosterone, and estradiol were measured from venous blood samples using an automated HGB analyzer and ELIZA technique. Results The platelet count was inversely related to the 2D:4DR in the total sample with the 2D:4DR accounting for about 0.2% (adjR2 = 0.002) of the variability in platelet count. However, there was a sex difference as indicated by the significant interaction between sex and the 2D:4DR on platelet count (p = 0.03). The relationship between platelet count and the 2D:4DR was negative in females but positive in males. Also, there was a positive relationship between HGB concentration and the Dr‐l in the total study sample, where the Dr‐l accounted for about 0.6% (adjR2 = 0.006) of the variability in HGB concentration. Sex interacted significantly with the Dr‐l on HGB concentration (p = 0.01) such that the relationship between HGB and the Dr‐l was positive in females but negative in males. Conclusion Prenatal hormone exposure, as indexed by the 2D:4D ratio, may partly account for the observed sex differences in platelet count and HGB levels in adulthood. Key points There was a significant interaction between sex and the 2D:4DR on platelet count. The relationship between platelet count and the 2D:4DR was negative in females but positive in males. There was a significant interaction between sex and right‐left difference (Dr‐l) on hemoglobin concentration. The relationship between hemoglobin and the Dr‐l was positive in females but negative in males. Prenatal hormone exposure may partly account for the observed sex differences in platelet count and hemoglobin levels in adulthood.

Project description:BackgroundDespite the global increase in the prevalence of autism spectrum disorders (ASD), relevant research studies are lacking in Brunei Darussalam. Various studies have shown a significant association between a lowered 2D:4D ratio (ratio of second digit/index finger to the fourth digit/ring finger) and ASD, making it one of the potential phenotypic biomarkers for early detection of autism, which is important for early intervention and management.ObjectiveThe objective of this study is to explore the association between 2D:4D ratio and ASD in Brunei Darussalam, as a potential tool to complement early ASD diagnosis.MethodsWe conducted a case-control study comprising 28 ASD and 62 typically developing (TD) children in the case and control group, respectively (age range: 3-11 years old; median age: 6 years old). Median 2D:4D ratios were measured, compared and analysed between the two groups. Logistic regression models were used to explore potential associations between the median 2D:4D ratio and ASD in respective gender, for both left and right hands, independently.ResultsOur study shows that the median 2D:4D ratio of left hand in ASD males is significantly lower than those in TD males, after adjusting for ethnicity and age [Odds Ratio (OR) = 0.57 (95% Confidence Interval (CI): 0.31-0.96); p = 0.044]. For females, there is no association of ASD with the median left hand 2D:4D ratio [OR = 3.09 (95% CI: 0.98-19.86); p = 0.144] or the median right hand 2D:4D ratio [OR = 1.23 (95% CI: 0.42-3.88); p = 0.702]. Our study also shows a significant positive correlation and/or a reduced asymmetry between the average 2D:4D ratio of left hands and right hands in ASD males (Pearson's correlation (r) = 0.48; 95% CI: 0.076-0.75, p = 0.023).ConclusionsThere is significant association between a lowered median 2D:4D ratio of the left hand (in males only) and ASD diagnosis. Once validated in a larger sample size, a lowered median 2D:4D ratio on the left hand may be a potential tool to complement ASD diagnosis for males in our study population. There is no association between the median 2D:4D ratio (left or right hands) and ASD in females, which could be due to the small female sample size and/or the possibility of different aetiology for ASD in females. Reduced asymmetry between the average 2D:4D ratio of left and right hands observed in ASD males only (not in ASD females) also suggests the importance of considering gender-specific biomarkers for ASD diagnosis.
