Esophageal submucosal gland duct adenoma: a case report and literature review.

Project description:IntroductionEsophageal submucosal gland duct adenoma (ESGDA) is a rare, benign tumor with non-specific clinical features and imaging findings, often leading to misdiagnosis.Case reportIn this report we describe the clinicopathological features of a new-onset case of ESGDA and review 19 previously-reported ESGDA cases in the literature.ResultsThe median age of the 20 patients was 70 years, and the male-to-female ratio was 5:2. Lesions located in the lower esophagus accounted for 81% of the cases. Chest and abdominal discomfort were the main clinical symptoms, and endoscopic findings mostly included polypoid masses that were impossible to correctly diagnose prior to excision and biopsy. On microscopic examination, the submucosal tumor was found to be composed of glandular cavities of various sizes, bland cytology with infrequent mitotic figures, and the interstitium was infiltrated by a large number of lymphocytes. Immunohistochemical analysis revealed the expression of adenomyoepithelial cytokeratins (CK) CK7, CK5/6, and p63. Local tumor resection was performed, the longest follow-up period was 132 months of the 20 patients, the prognosis was favorable, and no recurrence or metastasis.DiscussionESGDA is more common in the lower esophagus in elderly patients, and its clinical symptoms are atypical. Taking into account its characteristic microscopic morphology and immunohistochemical markers, the possibility of this rare disease should be considered to avoid misdiagnosis or missed diagnosis. Complete en bloc resection with Endoscopicp may be the best strategy for both the diagnosis and treatment of this entity.

Risk factor-based optimal endoscopic surveillance intervals after endoscopic submucosal dissection for gastric adenoma.

Project description:To date, there exists no established endoscopic surveillance interval strategy after endoscopic submucosal dissection (ESD) for gastric adenoma. In this study, we suggest a risk factor-based statistical model for optimal surveillance intervals for gastric adenoma after ESD with curative resection. A cox proportional hazard model was applied to identify risk factors for recurrence after ESD. Patients (n = 698) were categorized into groups based on the identified risk factors. The cumulative density of recurrence over time was computed using a cubic splined baseline hazard function, and the customized surveillance interval was modeled for each risk group. The overall cumulative incidence of recurrence was 7.3% (n = 51). Risk factors associated with recurrence were male (hazard ratio [HR], 2.60, P = 0.030), protruded scar (HR, 3.18, P < 0.001), and age ≥ 59 years (HR, 1.05, P < 0.001). The surveillance interval for each group was developed by using the recurrence limit for the generated risk groups. According to the developed schedule, high-risk patients would have a maximum of seven surveillance visits for 5 years, whereas low-risk patients would have biennial surveillance for cancer screening. We proposed a simple and promising strategy for determining a better endoscopic surveillance interval by parameterizing diverse and group-specific recurrence risk factors into a well-known survival model.

Successful esophageal endoscopic submucosal dissection with intraoperative release of stenosis due to previous endoscopic submucosal dissection scarring.

Project description:Endoscopic submucosal dissection (ESD) is the standard endoscopic treatment for early esophageal cancer. Esophageal stricture often occurs at the site of ESD for large lesions. When treating a metachronous lesion appearing at the severe stricture, it may be difficult to negotiate a conventional endoscope through the stricture. Using a thin endoscope may be a useful strategy for such lesions, though ESD using a thin endoscope is challenging because of poor maneuverability. Herein, we report a case of successful ESD for early esophageal cancer at the severe stricture, using a conventional endoscope. A 72-year-old man with a previous history of ESD for esophageal cancer and a post-ESD esophageal stricture was referred to our hospital for metachronous early esophageal cancer. The lesion, 10 mm in diameter, was located at the stricture with a slight distal extension. Conventional endoscopes could not be negotiated through stricture. Therefore, submucosal dissection was performed from the oral to the anal aspect of the lesion, as far as possible. After completion of submucosal dissection of the oral aspect of the lesion and part of the lesion located on the stricture, the severe stricture was released, allowing the passage of conventional endoscope, and ESD of the entire lesion was completed en bloc. Histopathological examination showed squamous cell carcinoma, pT1a-LPM. Stricture due to scarring may occur during the regeneration process of the defective mucosa, muscularis mucosa, and submucosal layer. Therefore, incision and dissection of the contracted mucosa, mucularis mucosa, and submucosal layer would release the stenosis.
