Malignant solitary fibrous tumour of the kidney with an extensive thrombus: A case report and review of the literature.

ABSTRACT: Solitary fibrous tumours (SFTs) are neoplasms originating from mesenchymal cells, usually found in lung pleura. Extrapleuritic lesions are extremely rare, with about 60 cases of SFTs of the kidney available in the English literature. We report here the first case of a malignant SFT of the kidney presenting with an extensive vascular thrombus extending to the right atrium with associated pulmonary emboli. We describe management, pathological assessment, as well as radiological and clinical evolution. Our report provides a first therapeutic approach to a critical presentation of a rare pathology, which should help guide management of such disease in future cases.

Solitary Fibrous Tumor of Breast with Anaplastic Areas (Malignant Solitary Fibrous Tumor): A Case Report with Review of Literature.

Project description:Solitary fibrous tumor (SFT) is a rare, soft tissue neoplasm that rarely presents in breast tissue, with only 27 previously reported cases. To our knowledge, only one case of malignant SFT has been reported in the English literature. A 75-year-old Caucasian woman presented to our institution with a 3-month history of a palpable left breast mass. No other symptoms, including nipple discharge or skin changes, were noted. She underwent 3 previous biopsies for right breast masses, all of which were benign, with no evidence of spindle cell neoplasm, atypical hyperplasia, or malignancy. Microscopic examination of the mass demonstrated a classic area of SFT with areas of high-grade anaplastic component. In these areas, the tumor showed atypical epithelioid cells arranged in hypercellular sheets with diminished branching vasculature, nuclear pleomorphism, and increased mitotic count (up to 9/10 high-power fields). This case represents the second case of malignant SFT in the breast.

Case report: A rare case of malignant solitary fibrous tumor within the joint cavity with review of the literature.

Project description:Solitary fibrous tumors (SFTs) are classified as fibroblastic/myofibroblastic tumors that originate from CD34-positive dendritic cells and usually occur in the pleura. In this paper, we describe a case of SFT within the joint cavity of the left knee. A 60-year-old man was admitted to hospital due to swelling in the left knee for the past 8 months without relevant trauma history. X-ray, computed tomography (CT), magnetic resonance imaging (MRI), and positron emission tomography-computed tomography (PET-CT) presented a large, ill-circumscribed, hypervascular, and highly enhanced mass with eccentric calcification and peripheral, intra-lesional vessels. Subsequently, the patient underwent surgical resection. Postoperative pathology confirmed the neoplastic cells to be positive for CD34, Bcl-2, and SATA6, therefore was finally diagnosed as malignant SFT. The patient developed bone metastases within 1 year after surgery. SFT in the joint cavity is rare, and it is difficult to make a preoperative diagnosis.

Case Report: Molecular Characterization of Aggressive Malignant Retroperitoneal Solitary Fibrous Tumor: A Case Study.

Project description:Solitary fibrous tumors (SFT) are mesenchymal neoplasms with a favorable prognosis usually originating from the visceral pleura. Rarely, they may occur at various extrapleural sites and show malignant behavior coupled with dedifferentiation. NAB2-STAT6 fusion gene and STAT6 nuclear expression are biomarkers for diagnosis of SFT in addition to CD34, Bcl-2, and CD99. Furthermore, several reports have shown specific NAB2-STAT6 fusion variants and loss of STAT6 protein expression are associated with malignancy. We report a rare case of retroperitoneal SFT which rapidly progressed to death within 35 days after admission. Autopsy found a primary tumor containing both benign and malignant histologies, with multiple metastatic sites similar to the malignant, dedifferentiated tumor. STAT6 was detected in the primary differentiated tumor but not in the primary dedifferentiated tumor or lung/liver metastases. However, the NAB2-STAT6 fusion gene (NAB2ex6/STAT6ex16 variant) was detected in the primary tumor and lung/liver metastases. Intriguingly, fusion gene expression at the transcriptional level was downregulated in the dedifferentiated tumors compared to the differentiated tumor. We further performed target DNA sequencing and found gene mutations in TP53, FLT3, and AR in the dedifferentiated tumors, with TP53 mutations especially found among them. We demonstrate that downregulation of NAB2-STAT6 fusion gene at the transcriptional level is associated with malignant SFT for the first time. Moreover, the present study supports the idea that TP53 mutations promote malignancy in SFTs.

Solitary fibrous tumor involving urinary bladder: a case report and literature review.

Project description:Solitary fibrous tumor (SFT) is a rare fibroblast stroma tumor involving the mediastinum and pleura. We herein describe an SFT of bladder which is extremely rare and review 29 similar cases in the last decades. We present a case of a 52-year-old male patient who suffered from urinary urgency and frequency for 12 months. Non-contrast computed tomography (CT) showed a slightly high density calcified mass with 70 mm × 61 mm in diameter. Contrast-enhanced CT demonstrated the mass was slightly enhanced. Cystoscopy revealed a huge mass with flat surface. Histopathological review of the biopsy specimens could not confirm the diagnosis. Partial cystectomy was then performed and the diagnosis of SFT was confirmed by immunohistochemistry. The patient is doing well at 12 months follow-up without recurrence and metastasis. In conclusion, the diagnosis of SFT involving bladder should combine clinical presentation and imaging features. Complete surgical resection is the primary method and long-term follow-up is necessary.

Case report: A case of giant malignant solitary fibrous tumor of the pleura with Doege-Potter's syndrome and review of the literature.

Project description:The solitary fibrous tumor of the pleura (SFTP) is a rare intrathoracic neoplasm that commonly originates from the subpleural mesenchymal cells of the visceral pleura and accounts for less than 5% of all pleural tumors. We reported a case of a 54-year-old man with a two-week history of hypoglycemia, a six-month history of productive cough and fatigue, and chronic right chest pain. Radiological techniques revealed a giant intra-thoracic mass with hypervascularization, and pathological staining was carried out to make a definitive diagnosis of SFTP. Interventional embolization was conducted to block the main feeding vessels before the surgery, and an anterolateral thoracotomy combined with a transverse sternotomy was performed to achieve a complete resection, which demonstrates significant potential for further application in patients with unilateral giant SFTP. The postoperative course was uneventful, with no signs of hypoglycemia observed during the follow-up. Additionally, we reviewed and prospected the research progress on SFTP. The aim of this study is to enhance clinicians' understanding of SFTP through our case and to provide a detailed review of the current research.

Retroperitoneal malignant solitary fibrous tumor with second recurrence and lymphatic metastases: A case report.

Project description:Malignant solitary fibrous tumor (SFT) in the retroperitoneum is rare. The present study reported on the case of a 67-year-old man who had retroperitoneal SFT for ~13 years, which resulted in two recurrences and lymphatic metastases. After the second recurrence, the patient presented with hematochezia and multiple retroperitoneal masses were found through computed tomography (CT). Histopathological examination showed that the tumor was mainly comprised of short spindle cells, arranged into sparse and dense areas. Mitotic figures were observed, generally 6-8 mitoses/10 high power fields, along with local necrosis. The tumor invaded the circumferential liver, intestines, lymphatic vessels and lymph nodes. Combined with the immunohistochemical results, it was diagnosed as a malignant SFT, which regrew just 2 months after the latest surgery. Retroperitoneal SFTs with repeated relapses, infiltrative growth and lymphatic metastasis suggest the need for careful and long-term follow-up.

Giant Cell-Rich Solitary Fibrous Tumor in the Nasopharynx: Case Report and Literature Review.

Project description:Solitary fibrous tumors (SFTs) can occur in several locations outside the pleura, but rarely in the sinonasal tract, and particularly not in the nasopharynx. Herein, we describe an unusual case of giant cell-rich SFT (GCRSFT) occurring in the nasopharynx. A 64-year-old man experienced dizziness and headache for more than 10 years with no obvious cause. Computed tomography (CT) scan showed a 3.9 cm × 2 cm tumor on the posterior lateral wall of the left nasopharynx, and angiography revealed a hypervascular tumor fed by branches of the left carotid artery. Hence, preoperative embolization was performed, and then the tumor was endoscopically resected. The symptoms were relieved after the resection, and postoperative head CT and video laryngoscopy showed that the tumor was completely resected. We next characterized the specific pathological characteristics of the resected tumor. Histologically, the tumor was characterized by varying cellular proliferation of cytologically bland spindle cells within a collagenous stroma, with prominent interspersed branching vessels. Mitotic activity was low (2/50HPF), and there was no evidence of pleomorphism or tumor necrosis. Moreover, multinucleated giant cells with deep nuclear staining and distributed in pseudovascular spaces were found within the tumor. We ruled out the possibility that our case was giant cell fibroblastoma (GCF) by immunohistochemical analysis, showing that the tumor cells were positive for CD34, CD99, STAT6, and BCL-2, and that the Ki-67 labeling index was 3%, indicating that our case was SFT and not GCF. The patient's condition is generally good after a 14-month follow-up. This report serves to broaden the morphologic spectrum of GCRSFT and will help clinicians and pathologists better understand this entity to prevent misdiagnosis.

Solitary fibrous tumor of the pineal gland: a case report and review of the literature.

Project description:Solitary fibrous tumor (SFT) is a type of fibroblastic neoplasm that can occur in various parts of the body, with SFT of the pineal gland being exceedingly rare. We report the case of a 58-year-old male presenting with recurrent hiccups, acid reflux, and headache. Magnetic resonance imaging revealed an occupying lesion in the pineal region, suggestive of a neoplastic process. Intraoperatively, the lesion was located in the pineal region, exhibiting a grayish-red color, and was largely resected. Pathological examination confirmed the diagnosis of solitary fibrous tumor (CNS WHO Grade 1). Postoperatively, the patient was supplemented with radiotherapy, and long-term follow-up showed no signs of recurrence or metastasis.

A rare case of sinonasal solitary fibrous tumour in a patient with concurrent papillary thyroid carcinoma: A case report.

Project description:Introduction and importanceA solitary fibrous tumour (SFT) is a rare neoplasm that commonly arises in the pleura and can occur in other extrathoracic sites. Extrapleural SFT, particularly in the sinonasal cavity, is extremely rare. There are no definite diagnostic criteria for sinonasal SFT as it is rare. Histologic analysis with immunohistochemistry plays an important role in diagnosing SFT.Case presentationWe report herein a case of SFT of the sinonasal cavity, which later spread to the oral cavity in a 67-year-old male with underlying papillary thyroid carcinoma (PTC) stage IV. He complained of recurrent epistaxis from a mass in his left nasal cavity for two weeks. The mass grew bigger, and spread to the oral cavity, causing dysphagia and upper airway obstruction. Tracheostomy was done under local anaesthesia and a biopsy of the mass was taken to rule out metastasis from the PTC. However, histopathological examination revealed a mesenchymal tumour of fibroblastic type, consistent with an SFT. He was planned for surgical resection of the tumour. However, he refused the operation and was lost to follow-up.Clinical discussionWe describe the clinical presentation of this rare tumour of the sinonasal and oral cavity, including upper airway obstruction, and the importance of immunohistochemical markers such as CD34 and BCL-2 in diagnosing SFT. Complete resection of the tumour is the definitive treatment for SFT.ConclusionSFT of the sinonasal and oral cavity is extremely rare. Upper airway obstruction may occur due to the location of the tumour in the airway region. Immunohistochemistry is crucial to distinguish this tumour from other mesenchymal tumours.

Extremely rare case of intravascular solitary fibrous tumour in the inferior vena cava with review of the literature.

Project description:BACKGROUND:Solitary fibrous tumour (SFT) is a mesenchymal tumour of fibroblastic type, and it develops in almost any part of the human body. However, according to previous studies, the occurrence of intravascular SFTs is extremely rare. CASE PRESENTATION:We reported a case of intravascular SFT in a 67-year-old woman who has been experiencing swelling and pain in the right leg for 2?months. Computed tomography venography scan revealed a well-defined mass obstructing the inferior vena cava (IVC). Surgical resection was performed, and histopathologic and immunohistochemical results were consistent with SFT. Further, next-generation sequencing (NGS) analysis was performed, and results revealed two tumour-related gene mutations (deletion of PMS2 and variation of ESR1 [L536P]). The patient did not receive any adjuvant therapy, and no signs of tumour progression were observed during the 6-month follow-up. CONCLUSION:To the best of our knowledge, this study first presented about SFT arising from the IVC and carried out an NGS analysis to validate the molecular mechanism of such condition.
