Double trouble: A unique case of TRUS biopsy induced left upper moiety infection of a duplicated ectopic ureter.

ABSTRACT: Duplex collecting systems are seen in 0.7% of the population and its association with an ectopic ureter (specifically in the ejaculatory duct) is even less common. We presented a 57 year-old male with an elevated PSA and suspicious findings on a multiparametric MRI (mpMRI) who underwent transrectal ultrasound (TRUS) guided biopsies. The procedure led to an infection of the upper moiety of the duplicated left collecting system with ectopic ureter draining into the ejaculatory duct. CT-scan was used for the diagnosis and an urgent surgical treatment was performed. To our knowledge, no such case has ever been reported.

Project description:IntroductionIncidence of duplicated urinary system is 0.7-4% of population, mostly are females and often diagnosed in childhood. Various symptoms meet difficulties to be diagnosed.Case presentationA 20-years-old woman admitted to hospital with fever, fatigue, nausea, loss of appetite, colic epigastric pain and right flank pain since 5 days before admission, normal pattern of urination with dribbling and recurrent urinary tract infection. Abdominal ultrasonography showed complicated cystic tumor upper pole of right kidney. Abdominal computed tomography with contrast showed enlargement right kidney with duplicated collecting system and duplicated ureter obstruction (severe hydroureteronephrosis right upper pole moiety) right kidney and ectopic ureter insertion of upper pole moiety between urethra and anterior vagina, no insertion to bladder, seemed to be dead end.DiscussionVarious symptoms of duplicated collecting system are asymptomatic, flank pain, abdominal pain, urinary incontinence, and recurrent UTI, often accompanied by abnormality of upper pole or lower pole or both. Abnormality of upper renal moiety usually has ectopic ureter as in Weigert-Meyer principle. Insertion into infrasphincter usually manifests as urinary dribbling or urinary incontinence or normal urination with few volume leakage or spotting incontinence, whereas suprasphincter usually manifests as recurrent UTI without incontinence. There are imaging modalities for diagnosing these anomalies, including USG and abdominal CT with contrast.ConclusionUnderstanding embryology, symptoms, imaging modality, and complications are necessary to consider this diagnosis for early detection. Ultrasonography and abdominal CT with contrast can be used to diagnose the urinary tract anomalies, especially duplicated urinary systems with ectopic ureter insertion.

Is it necessary for all patients with suspicious lesions undergo systematic biopsy in the era of MRI-TRUS fusion targeted biopsy?

Project description:PurposeTargeted biopsy (TB) combined with systematic biopsy (SB) is an optimized mode of prostate biopsy but can often lead to oversampling and overdiagnosis accompanied by potential biopsy-related complications and patient discomfort. Here, we attempted to reasonably stratify the patient population based on multi-parameter indicators with the aim of avoiding unnecessary SB.MethodsIn total, 340 biopsy-naïve men with suspected lesions, prostate-specific antigen (PSA) < 20 ng/mL and prostate imaging-reporting and data system (PI-RADS) ≥ 3 enrolled for study underwent both TB and SB. The primary outcome was to determine independent predictors for a valid diagnosis, assuming that only TB was performed and SB omitted (defined as mono-TB), taking TB + SB as the reference standard. The secondary outcomes were exploration of the predictive factors of mono-TB and TB + SB in detection of prostate cancer (PCa) and clinically significant PCa (csPCa).ResultsThe mean PSA density (PSAD) of patient group was 0.27 ng/mL/mL. Multiparametric MRI PI-RADS scores were 3-5 in 146 (42.94%), 105 (30.88%), and 89 (26.18%) cases, respectively. PCa and csPCa were detected in 178/340 (52.35%) and 162/340 (47.65%) patients, respectively. Overall, 116/178 (65.17%) patients diagnosed with PCa displayed pathological consistencies between mono-TB and TB + SB modes. PSAD and PI-RADS were independent predictors of valid diagnosis using mono-TB.ConclusionsPSAD combined with PI-RADS showed utility in guiding optimization of the prostate biopsy mode. Higher PSAD and PI-RADS values were associated with greater confidence in implementing mono-TB and safely omitting SB, thus effectively balancing the benefits and risks.

Midline lesions of the prostate: role of MRI/TRUS fusion biopsy and implications in Gleason risk stratification.

Project description:PurposeMRI-TRUS fusion biopsy (FBx) has proven efficacy in targeting suspicious areas that are traditionally missed by systematic 12-core biopsy (SBx). Midline prostate lesions are undersampled during SBx, as traditional approaches aim laterally during TRUS biopsy. The aim of our study was to determine the utility of FBx for targeting midline lesions identified on multiparametric MRI (mpMRI).MethodsA review was performed of a prospectively maintained database of patients undergoing mpMRI followed by FBx and SBx from 2007 to 2015. Midline location was defined as any lesion crossing the midline on axial imaging and involving both prostatic lobes. Index lesion was defined as lesion with highest Gleason score on biopsy. Patient demographic, imaging, and histopathologic data were collected. Multivariate logistic regression was conducted to determine independent predictors of having clinically significant (CS) cancer (Gleason ≥ 7) in midline lesions.ResultsOut of 1266 patients, we identified 202 suspicious midline lesions in 190 patients [median (IQR) age 63 (10) years; PSA 7.6 (6.6)]. Eighty-eight (46.3 %) patients had cancer detection on FBx of midline lesion. A midline target represented the index lesion of the prostate in 63/190 (33.2 %) patients. Risk category upgrading based on FBx of a midline lesion compared to SBx occurred in 45/190 patients (23.7 %). On multivariate analysis, higher PSA (p = .001), lower MRI-derived prostate volume (p < .001), and moderate-high or high suspicion on mpMRI (p = .014) predicted CS cancer in midline lesions.ConclusionsMRI-TRUS FBx facilitates sampling of midline lesions. Integration of mpMRI and FBx for targeting of midline lesions improves detection of CS prostate cancer.

Can we improve the detection rate of prostate cancer using standard 12-core TRUS-guided prostate biopsy? Focused on the location of prostate biopsy.

Project description:BackgroundWe assessed the effect of biopsy location on the prostate cancer detection and clinically significant prostate cancer.MethodsA total of 2774 patients with 12-core prostate transrectal ultrasound-guided prostate biopsy were included for per core analysis. Multivariate Cox regression analysis was performed to evaluate the effect of the location of biopsy on the prostate cancer and clinically significant prostate cancer detection.ResultsProstate cancer was found in 775 patients (27.9%) and 576 prostate cancer patients (20.8%) were found to be clinically significant. The core length (P = .043), tumor length (P < .001), and % tumor length (P < .001) were significantly different according to the biopsy location. The detection rates for prostate cancer and clinically significant prostate cancer differed significantly according to the location of biopsy. Multivariate analysis revealed that the apical core was significantly related with increased detection of prostate cancer and clinically significant prostate cancer. The lateral core, in addition to apical core, was found to be significantly related with increased detection rates of prostate cancer and clinically significant prostate cancer in men with prostate-specific antigen <10 ng/mL.ConclusionsMore in-depth discussions on the location of standard 12-core prostate biopsy are considered necessary. Apical core and lateral core biopsies may be helpful, especially in patients with prostate-specific antigen ˂10 ng/mL if additional biopsies are planned following findings of no target lesions on imaging studies.

Introduction of surgical site surveillance post transrectal ultrasound (TRUS) guided prostate biopsy and the impact on infection rates

Project description:Summary Background Transrectal ultrasound (TRUS)-guided prostate biopsy is associated with infection rates between 0.3 % and 3.2%. Infectious complications include urinary tract infection, prostatitis, bacteraemia and sepsis. Surgical site surveillance in this patient cohort is becoming increasingly important given global increases in antimicrobial resistance. Methods Surgical site surveillance for patients undergoing TRUS biopsies was introduced in our hospital in 2017. All patients had a risk assessment form completed to assess for carriage or risk of carriage of multi-drug resistant organisms. An intense analysis was completed on any patient who developed an infection post-TRUS biopsy. Data was fed back on a quarterly basis to a multi-disciplinary working group. Members of this group include a Consultant Microbiologist, Infection Prevention and Control Nurse, Consultant Urologist, Antimicrobial Pharmacists and Clinical Nurse Ward Managers. Results 784 TRUS-guided biopsy of the prostate procedures were performed between January 1 st 2017 and the end of the third quarter, 2021. The rate of infection post-TRUS was 2.7% in 2017, 3.4% in 2018 and 3.2% in 2019. This improved to 0% in 2020 and 0.8% in the first three quarters of 2021. Conclusions Several interventions were introduced resulting in a sustained reduction in infection rates in this cohort. These include changing the choice of surgical antibiotic prophylaxis, improvement in the timing of antibiotic prophylaxis and scheduling of other urology procedures. The introduction of surgical site surveillance and multi-disciplinary input has demonstrated a reduction in infection rates post TRUS biopsy.
