A theory of change for community interventions to prevent domestic violence against women and girls in Mumbai, India.

ABSTRACT: Background: We describe the development of a theory of change for community mobilisation activities to prevent violence against women and girls. These activities are part of a broader program in urban India that works toward primary, secondary, and tertiary prevention of violence and includes crisis response and counselling and medical, police, and legal assistance. Methods: The theory of change was developed in five phases, via expert workshops, use of primary data, recurrent team meetings, adjustment at further meetings and workshops, and a review of published theories. Results: The theory summarises inputs for primary and secondary prevention, consequent changes (positive and negative), and outcomes. It is fully adapted to the program context, was designed through an extended consultative process, emphasises secondary prevention as a pathway to primary prevention, and integrates community activism with referral and counselling interventions. Conclusions: The theory specifies testable causal pathways to impact and will be evaluated in a controlled trial.

Project description:Despite ambitions in development and global health policy to transform communities into supportive environments for women facing risks of violence, our understanding of how to best engage communities remains incomplete. In particular, there is little evidence on the types of strategies that communities employ to address violence against women (VAW). We aimed to describe and analyse the processes involved in community responses to incidents of VAW in a non-governmental organisation (NGO) violence prevention programme in Mumbai, India. We conducted a grounded theory study involving 30 focus group discussions and 36 semi-structured interviews with 113 community members and 9 NGO staff, as well as over 170 h of field observation. Informed by comparative case study methods, we compared community actions across six informal settlement neighbourhoods. We found considerable variation in the type of action taken across neighbourhoods. This variation was not arbitrary, but reflected systematic cost-benefit considerations in heterogeneous environments, accounting for factors such as trust in neighbours, violent (armed or gang-related) crime, and corruption in state institutions. We found that institutional action was only favoured in neighbourhoods with strong state capacity and high social capital, whilst remedial action-resolving violence through 'private' talks in the family and community-and radical action involving extra-judicial violence-became favourable under conditions of weak social capital or low state capacity. Institutional action was, however, sometimes as violent as radical action, as police were reported, sometimes even relied upon, by residents to use force against perpetrators of VAW. We argue that these contextual features oblige policymakers and prevention researchers to grapple with contentious issues such as the legitimate use of force by the state. We caution against maximising community action without a clear vision of the type of action hoped for.

Project description:ObjectivesDomestic violence against women harms individuals, families, communities and society. Perpetrated by intimate partners or other family members, its overlapping forms include physical, sexual and emotional violence, control and neglect. We aimed to describe the prevalence of these forms of violence and their perpetrators in informal settlements in Mumbai.DesignCross-sectional survey.SettingTwo large urban informal settlement areas.Participants5122 women aged 18-49 years.Primary and secondary outcome measuresPrevalence and perpetrators in the last year of physical, sexual and emotional domestic violence, coercive control and neglect. For each of these forms of violence, responses to questions about individual acts and composite estimates.ResultsIn the last year, 644 (13%) women had experienced physical domestic violence, 188 (4%) sexual violence and 963 (19%) emotional violence. Of ever-married women, 13% had experienced physical or sexual intimate partner violence in the last year. Most physical (87%) and sexual violence (99%) was done by partners, but emotional violence equally involved marital family members. All three forms of violence were more common if women were younger, in the lowest socioeconomic asset quintile or reported disability. 1816 women (35%) had experienced at least one instance of coercive control and 33% said that they were afraid of people in their home. 10% reported domestic neglect of their food, sleep, health or children's health.ConclusionsDomestic violence against women remains common in urban informal settlements. Physical and sexual violence were perpetrated mainly by intimate partners, but emotional violence was attributed equally to partners and marital family. More than one-third of women described controlling behaviours perpetrated by both intimate partners and marital family members. We emphasise the need to include the spectrum of perpetrators and forms of domestic violence-particularly emotional violence and coercive control-in data gathering.Trial registration numberISRCTN84502355; Pre-results.

Project description:Intimate partner violence (IPV) in India remains an entrenched and prevalent public health issue. Despite ample evidence of the widespread problem of IPV in India and associated mental and physical morbidities, far less is known about intervention models to reduce IPV in India. The aims of this meta-analysis and systematic review are to assess the effectiveness of community-based interventions to reduce IPV in India and to provide a narrative synthesis of these intervention approaches. A total of 9 databases were searched to identify peer-reviewed, English-language articles published between January 2000 and September 2022. The search identified 10 studies that met study inclusion criteria, including 3 randomized control trials, 4 quasi-experimental, 2 pre/post, and 1 time-series evaluation. Eight studies were included in the meta-analysis. There was notable variation in the interventions and approaches employed to reduce IPV and varying measurement of IPV outcomes. The results of the meta-analysis show that participating in community-based IPV interventions produced a significant reduction in IPV among women. When considering different types of IPV, study participants were less likely to report physical and psychological IPV victimization. In addition, participants were also less likely to report approving of IPV after participating in community-based IPV interventions. Community-based interventions and research addressing IPV are still evolving in India. Missing descriptions of theoretical frameworks, sampling, intervention design, and inadequately reported effectiveness of intervention (both quantitative and qualitative reporting) need to be addressed. Moreover, long-term evaluations of the pilot interventions are needed to provide a clear picture of the long-term effectiveness, sustainability, and replicability of the community-based IPV interventions. The findings have implications for researchers, practitioners (community health workers, clinicians, and social workers), and policymakers keen on IPV reduction in India and globally.

Project description:ObjectiveThe field of violence against women and girls (VAWG) prevention research has rapidly advanced, with ample evidence now demonstrating that VAWG is preventable. The aim of this systematic review of reviews is to update the evidence that underpins the RESPECT women framework with the newest available evidence from 2013 onward.MethodsAcademic and non-academic databases were searched using terms related to VAWG prevention (January 2013-April 2022). Evaluations had to have a target population of women or girls aged 10 and older. Data were extracted from included reviews, and AMSTAR-2 was used to assess the risk of bias in systematic reviews. The primary outcome of interest was change in any form of VAWG, including physical, sexual and emotional intimate partner violence and sexual violence and harassment from non-partners, including child and adolescent sexual abuse.ResultsFrom the thousands of articles screened, 178 were included in this study. Six (3%) reviews focused on interventions that aim to strengthen relationship skills, 14 (8%) focused on empowerment of women and girls, 79 (44%) on services for survivors, 5 (3%) on poverty reduction, 16 (9%) on creating safe environments, 36 (20%) on preventing child and adolescent abuse and 22 (12%) on transforming gender attitudes, beliefs and norms. Little new evidence has emerged to meaningfully change the nature and strength of evidence for interventions related to relationship skills strengthening and poverty reduction. However, there is new evidence to reflect the effectiveness of select types of interventions across settings in the other five strategies.ConclusionDespite progress in VAWG prevention research, significant gaps in the evidence base persist. Further research is needed to explore intervention areas and marginalised populations in various contexts. Several programmatic approaches exhibiting efficacy in low-income and middle-income countries remain unexplored and unevaluated in high-income countries, and vice versa, warranting further adaptation and evaluation.
