Project description:BackgroundA large number of people participate in individual or unorganized sports on a recreational level. Furthermore, many participants drop out because of injury or lowered motivation. Potentially, physical activity-related apps could motivate people during sport participation and help them to follow and maintain a healthy active lifestyle. It remains unclear what the quality of running, cycling, and walking apps is and how it can be assessed. Quality of these apps was defined as having a positive influence on participation in recreational sports. This information will show which features need to be assessed when rating physical activity-related app quality.ObjectiveThe aim of this study was to identify expert perception on which features are important for the effectiveness of physical activity-related apps for participation in individual, recreational sports.MethodsData were gathered via an expert panel approach using the nominal group technique. Two expert panels were organized to identify and rank app features relevant for sport participation. Experts were researchers or professionals in the field of industrial design and information technology (technology expert panel) and in the field of behavior change, health, and human movement sciences who had affinity with physical activity-related apps (health science expert panel). Of the 24 experts who were approached, 11 (46%) agreed to participate. Each panel session consisted of three consultation rounds. The 10 most important features per expert were collected. We calculated the frequency of the top 10 features and the mean importance score per feature (0-100). The sessions were taped and transcribed verbatim; a thematic analysis was conducted on the qualitative data.ResultsIn the technology expert panel, applied feedback and feedforward (91.3) and fun (91.3) were found most important (scale 0-100). Together with flexibility and look and feel, these features were mentioned most often (all n=4 [number of experts]; importance scores=41.3 and 43.8, respectively). The experts in the health science expert panels a and b found instructional feedback (95.0), motivating or challenging (95.0), peer rating and use (92.0), motivating feedback (91.3), and monitoring or statistics (91.0) most important. Most often ranked features were monitoring or statistics, motivating feedback, works good technically, tailoring starting point, fun, usability anticipating or context awareness, and privacy (all n=3-4 [number of experts]; importance scores=16.7-95.0). The qualitative analysis resulted in four overarching themes: (1) combination behavior change, technical, and design features needed; (2) extended feedback and tailoring is advised; (3) theoretical or evidence base as standard; and (4) entry requirements related to app use.ConclusionsThe results show that a variety of features, including design, technical, and behavior change, are considered important for the effectiveness of physical activity-related apps by experts from different fields of expertise. These insights may assist in the development of an improved app rating scale.

Project description:ObjectiveThis study aims to advance understanding of globally valid versus country-specific quality dimensions and indicators, as perceived by relevant stakeholders. It specifically addresses patient-level indicators for cataract surgery.DesignA mixed-methods case study comparing Singapore and The Netherlands SETTING: Singapore (2017-2019) and The Netherlands (2014-2015).ParticipantsStakeholder representatives of cataract care in Singapore and The Netherlands.InterventionBased on the previously identified complete set of stakeholders in The Netherlands, we identified stakeholders of cataract care in Singapore. Stakeholder representatives then established a multi-stakeholder perspective on the quality of cataract care using a concept mapping approach. This yielded a multidimensional cluster map based on multivariate statistical analyses. Consensus-based quality dimensions were subsequently defined during a plenary session. Thereafter, Singaporean dimensions were matched with dimensions obtained in The Netherlands to identify commonalities and differences.Main outcome measureHealth-services quality dimensions of cataract care.Results19 Singaporean stakeholders representing patients, general practitioners, ophthalmologists, nurses, care providers, researchers and clinical auditors defined health-services quality of cataract care using the following eight dimensions: clinical outcome, patient outcomes, surgical process, surgical safety, patient experience, access, cost and standards of care. Compared with the Dutch results, 61% of the indicators were allocated to dimensions of comparable names and compositions. Considerable differences also existed in the composition of some dimensions and the importance attached to indicators.Conclusions and relevanceThis study on cataract care in Singapore and The Netherlands shows that cataract care quality measurement instruments can share a common international core. At the same time, it emphasises the importance of taking a country-specific multi-stakeholder approach to quality definition and measurement. Complementing an international core set with country-specific measures is required to ensure that the included dimensions and indicators adequately capture the country-specific quality views.

Project description:Asthma is a major cause of morbidity and mortality and is associated with significant economic burden worldwide. The objectives of this study were to map current resource use associated with the disease management and to estimate the annual direct and indirect costs per adult patient with asthma.A Delphi panel with seven leading pulmonologists was conducted. A semistructured questionnaire was developed to elicit data on resource use and treatment patterns. Unit costs from official, published sources were subsequently assigned to resource use to estimate direct medical costs. Indirect costs were estimated as number of work loss days. Cost base year was 2015, and the perspective adopted was that of the National Organization of Health Care Services Provision, as well as the societal.Patients with asthma are mainly managed by pulmonologists (71.4%) and secondarily by general practitioners and internists (28.6%). The annual cost of managing exacerbations was estimated at €273.1, while maintenance costs were estimated at €1,100.2 per year. Total costs of managing asthma per patient per year were estimated at €2,281.8, 64.4% of which represented direct medical costs. Of the direct costs, pharmaceutical treatment was the key driver, accounting for 63.9 and 41.2% of direct and total costs, respectively. Direct non-medical costs (patient travel and waiting time) were estimated at €152.3. Indirect costs accounted for 28.9% of total costs.Asthma is a chronic condition, the management of which constrains the already limited Greek health care resources. The increasing prevalence of the disease raises concerns as it could translate per patient costs into a significant burden for the Greek health care system. Thus, the prevention, self-management, and improved quality of care for asthma should find a place in the health policy agenda in Greece.

Project description:BACKGROUND: Literature on scoliosis screening is vast, however because of the observational nature of available data and methodological flaws, data interpretation is often complex, leading to incomplete and sometimes, somewhat misleading conclusions. The need to propose a set of methods for critical appraisal of the literature about scoliosis screening, a comprehensive summary and rating of the available evidence appeared essential. METHODS: To address these gaps, the study aims were: i) To propose a framework for the assessment of published studies on scoliosis screening effectiveness; ii) To suggest specific questions to be answered on screening effectiveness instead of trying to reach a global position for or against the programs; iii) To contextualize the knowledge through expert panel consultation and meaningful recommendations. The general methodological approach proceeds through the following steps: Elaboration of the conceptual framework; Formulation of the review questions; Identification of the criteria for the review; Selection of the studies; Critical assessment of the studies; Results synthesis; Formulation and grading of recommendations in response to the questions. This plan follows at best GRADE Group (Grades of Recommendation, Assessment, Development and Evaluation) requirements for systematic reviews, assessing quality of evidence and grading the strength of recommendations. CONCLUSIONS: In this article, the methods developed in support of this work are presented since they may be of some interest for similar reviews in scoliosis and orthopaedic fields.

Project description:Research on knowledge exchange in multi-stakeholder platforms (MSPs) within smallholder farmer settings has primarily focused on adaptive designs and increased innovation rates. This paper addresses a knowledge gap by providing insights into the limited understanding of how knowledge sharing in MSPs can be inclusive of smallholder farmers' knowledge. It shares experiences with a farmer-centred approach to learning and knowledge-sharing with smallholder farmers in Ghana and places this iteratively developed approach within a theoretical perspective. We relate the farmer-centred approach to five design principles inspired by human-centred design rules and inclusive development dimensions. Findings from observations, interviews, and focus group discussions illustrate how the farmer-centred approach creates multifunctional spaces for peer-to-peer learning, interaction with institutional actors, and network building (design principle 1). Prior farmer-centred research ensured that themes addressed were relevant to participant farmers and brought their innovations 'from below' to the table (design principles 2 and 3). A majority representation of farmers, facilitation by local researchers, and organising platform meetings at the district level created a safe space for farmers to share their knowledge and feel included and empowered (design principle 4). Several local institutional actors took ownership of the platform principles, which created opportunities for their manifestation in future collaborations with farmers (design principle 5). However, challenges remain in engaging crucial actors from higher scale levels and reducing donor dependency. The design principles presented in this paper contribute to the genuine inclusion of farmers' knowledge and innovations. We recommend government agencies at the national and district levels incorporate these design principles into their work plans and budgets and prioritise farmers' knowledge and innovation capacity for inclusive learning and knowledge co-creation.Supplementary informationThe online version contains supplementary material available at 10.1007/s13132-023-01661-7.

Project description:BackgroundPosterior circulation infarction (POCI) is common. Imaging techniques such as non-contrast-CT (NCCT) and diffusion-weighted-magnetic-resonance-imaging commonly fail to detect hyperacute POCI. Studies suggest expert inspection of Computed Tomography Perfusion (CTP) improves diagnosis of POCI. In many settings, there is limited access to specialist expertise. Deep-learning has been successfully applied to automate imaging interpretation. This study aimed to develop and validate a deep-learning approach for the classification of POCI using CTP.MethodsData were analysed from 3541-patients from the International-stroke-perfusion-registry (INSPIRE). All patients with baseline multimodal-CT and follow-up imaging performed at 24-48 h were identified. A cohort of 541-patients was constructed on a 1:3 POCI-to -reference-ratio for model analysis. A 3D-Dense-Convolutional-Network (DenseNet) was trained to classify patients into POCI or non-POCI using CTP-deconvolved-maps. Six-stroke-experts also independently classified patients based upon stepwise access to multimodal CT (mCT) data. DenseNet results were compared against expert clinician results. Model and clinician performance was evaluated using area-under-the-receiver-operating-curve, sensitivity, specificity, accuracy and precision. Clinician agreement was measured with the Fleiss-Kappa-statistic.ResultsBest mean clinician diagnostic accuracy, sensitivity and agreement was demonstrated after review of all mCT data (AUC: 0.81, Sensitivity: 0.65, Fleiss-Kappa-statistic: 0.73). There was a spectrum of individual clinician results with an AUC-range of 0.73-0.86. Best DenseNet performance was recorded with an input combination of NCCT and delay-time maps. The DenseNet model was superior to the best mean clinician performance (AUC: 0.87) and was due to enhanced sensitivity (DenseNET: 0.77, Clinician: 0.65). The degree to which the DenseNet model outperformed each clinician ranged and was clinician specific (AUC improvement 0.01-0.14).ConclusionComprehensive review of CTP improves diagnostic performance and agreement amongst clinicians. A DenseNet model was superior to best mean clinician performance. The degree of improvement varied by specific clinician. Development of a clinician-DenseNet approach may improve inter-clinician agreement and diagnostic accuracy. This approach may alleviate limited specialist services in resource constrained settings.

Project description:The provision of upper limb prosthetic devices through the National Health Services (NHS) within the United Kingdom is driven by national policies. NHS England have recently published a new policy to provide multi-grip myoelectric hands. The policy highlighted that there was limited evidence to support its deployment and it will be reviewed should new information arise. The clear identification of the evidence gap provides an opportunity for the academic research community to conduct studies that will inform future iterations of this and other upper limb prosthetic related policies. This paper presents a summary of findings and recommendations based on two multi-stakeholder workshops held in June 2022 and July 2022, which explored the design requirements for policy-driven research studies. The workshops involved people from a broad range of stakeholder groups: policy, academia, NHS clinical and management, industry, and a person with upper limb absence. The workshop discussions focused on the research questions that NHS England identified in the policy evidence review: (1) Clinical Effectiveness; (2) Cost Effectiveness; (3) Safety; and (4) Patient Subgroups. The recommendations based on stakeholder discussions included the need to gather qualitative and quantitative research evidence, use goal-based outcome measures, and conduct longitudinal studies. Future research studies also need to address the complexities of conducting national and international policy-driven research, such as clinical resource capacity and participant involvement.

Project description:Multi-stakeholder platforms have become mainstream in projects, programmes and policy interventions aiming to improve innovation and livelihoods systems, i.e. research for development interventions in low- and middle-income contexts. However, the evidence for multi-stakeholder platforms' contribution to the performance of research for development interventions and their added value is not compelling. This paper focuses on stakeholder participation as one of the channels for multi-stakeholder platforms' contribution to the performance of research for development interventions, i.e. stakeholder participation. It uses a quantitative approach and utilizes descriptive statistics and ARIMA models. It shows that, in three Ugandan multi-stakeholder platform cases studied, participation increased both in nominal and in unique terms. Moreover, participation was rather cyclical and fluctuated during the implementation of the research for development interventions. The study also shows that, in addition to locational and intervention factors such as type of the area along a rural-urban gradient targeted by the intervention and human resources provided for multi-stakeholder platform implementation, temporal elements such as phases of research for development intervention objectives and the innovation development process play significant roles in influencing participation. The study concludes that contribution of multi-stakeholder platforms to the performance of research for development projects, programs, policies and other initiatives is constrained by locational and temporal context and conditional on the participation requirements of the objectives pursued by research for development intervention.

Project description:(1) The project "Tatort Streetlight" implements an insect-friendly road light design in a four year before-after, control-impact (BACI) approach involving citizen scientists. It will broaden the stakeholder interests from solely anthropogenic perspectives to include the welfare of insects and ecosystems. Motivated by the detrimental impacts of road lighting systems on insects, the project aims to find solutions to reduce the insect attraction and habitat fragmentation resulting from roadway illumination. (2) The citizen science approach invites stakeholders to take part and join forces for the development of a sustainable and environmentally friendly road lighting solution. Here, we describe the project strategy, stakeholder participation and motivation, and how the effects of the alternative road luminaire and lighting design can be evaluated. (3) The study compares the changes in (a) insect behavior, (b) night sky brightness, and (c) stakeholder participation and awareness. For this purpose, different experimental areas and stakeholders in four communities in Germany are identified. (4) The project transfers knowledge of adverse effects of improperly managed road illumination and interacts with various stakeholders to develop a new road lighting system that will consider the well-being of street users, local residents, and insects.

Project description:Nanotechnology can be described as an emerging technology and, as has been the case with other emerging technologies such as genetic modification, different socio-psychological factors will potentially influence societal responses to its development and application. These factors will play an important role in how nanotechnology is developed and commercialised. This article aims to identify expert opinion on factors influencing societal response to applications of nanotechnology. Structured interviews with experts on nanotechnology from North West Europe were conducted using repertory grid methodology in conjunction with generalized Procrustes analysis to examine the psychological constructs underlying societal uptake of 15 key applications of nanotechnology drawn from different areas (e.g. medicine, agriculture and environment, chemical, food, military, sports, and cosmetics). Based on expert judgement, the main factors influencing societal response to different applications of nanotechnology will be the extent to which applications are perceived to be beneficial, useful, and necessary, and how 'real' and physically close to the end-user these applications are perceived to be by the public.
