This article relates to data concerning the number of passengers using buses operating in Abashiri city, Hokkaido, Japan, from 2013 to 2018. Data on the number of passengers for three lines-the <i>shopping</i> line, the <i>Tokyo NODAI</i> line, and the <i>Hagoromo-danchi</i> line-were collected by counting the passengers getting on and off the buses at each stop. Project description:Many airlines instituted social distancing practices to keep passengers safe during the pandemic. The practices include keeping the middle seats empty, reducing the number of passengers taking an apron bus from the terminal to the airplane, and prescribing that passengers maintain 1 m social distance of separation from other passengers in the aisle while advancing to their seats. However, not all passengers comply with a prescribed 1 m aisle social distance. Through agent-based simulations of passenger boarding when apron buses are used, we examine boarding policies adapted for the pandemic when the level of passenger compliance varies. To compare policies, we consider the duration of time that passengers are too close to other passengers while walking or standing in the aisle. We consider other health metrics from previous research and the time to complete boarding of the airplane. We find that the WilMA–Spread and Reverse-pyramid–Spread boarding methods provide favorable outcomes. Airlines should use WilMA–Spread if their primary concern is the risk to passengers while walking down the aisle and Reverse-pyramid–Spread if they want faster times to complete boarding of the airplane and reduced risk to aisle seat passengers from later boarding passengers. The level of the passengers’ non-compliance with the prescribed aisle social distance can impact a health metric by up to 6.75%—depending on the boarding method and metric. However, non-compliance reduces the time to complete boarding of the airplane by up to 38.8% even though it increases the average time an individual passenger spends boarding.

Project description:The COVID-19 epidemic has affected public transport in various cities worldwide, including Guwahati city in India, where a 59.09% decline in the ridership of city bus services occurred in autumn 2021. The present work aims to investigate the impact of COVID-19 on travelers' perception and identify the parameters associated with the user's dissatisfaction for improving bus service in Guwahati city. In addition, the study also estimated the revenue loss of bus services due to this pandemic. This was carried out based on survey data from travelers in pre-covid (January 2019) situation, unlocked with 50% seating capacity (November 2020), and unlocked with 100% seat capacity plus some limited amount of standee (January 2021) situation. Results showed an increase in the number of users' priorities in post covid compared to pre covid conditions. Factors such as comfort, fairness, reliability & convenience, and safety are the priority areas of users' pre covid. Whereas bus service attribute such as vehicle condition & hygiene emerged as an additional priority of users post covid. Unfortunately, the bus service in the city performed poorly in terms of hygiene, leading to the shifting of the user to other modes of transport. The poor condition of the vehicles is another major factor that made the users to discard the bus service. This led to huge financial losses of the public transit authorities. The cost analysis result showed that the city bus operators faced a financial loss due to the COVID19 outbreak. The average wage of the drivers reduced from 780 Rs./day to 339 Rs./day in the first unlock phase and 476 Rs./day in the second unlock phase. The present study discusses the introduction of the new route in the city, regular cleaning, regular servicing of the buses, and other recommendations to improve the bus service in Guwahati city.

Project description:BackgroundThe UK National Screening Committee currently recommends against antenatal screening for Hepatitis C virus (HCV) infection in England due to lack of HCV prevalence data and treatment licensed for use in pregnancy. We aimed to produce regional and national estimates of the number and proportion of livebirths to HCV seropositive women in England in 2013 and 2018.MethodsA logistic regression model fitted in the Bayesian framework estimated probabilities of HCV seropositivity among 24,599 mothers delivering in the North Thames area of England in 2012 adjusted by maternal age and region of birth. These probabilities were applied to the underlying population structures of women delivering livebirths in England in 2013 and 2018 to estimate the number of livebirths to HCV seropositive women in these years nationally and by region. The Bayesian approach allowed the uncertainty associated with all estimates to be properly quantified.ResultsNationally, the estimated number of livebirths to women seropositive for HCV for England was 464 (95% credible interval [CI] 300-692) in 2013 and 481 (95%CI 310-716) in 2018, or 70.0 (95%CI 45.0-104.1) per 100,000 and 76.9 (95%CI 49.5-114.4) per 100,000 in these years respectively. Regions with the highest estimated number of livebirths to HCV seropositive women in 2013 and 2018 included London with 118.5 and 124.4 and the South East with 67.0 and 74.0 per 100,000 livebirths.ConclusionFew previous studies have investigated HCV among pregnant women in England. These findings complement and supplement existing research by providing national and regional estimates for the number of livebirths to HCV seropositive women in England. Bayesian modelling allows future national and regional estimates to be produced and the associated uncertainty to be properly quantified.

Project description:BACKGROUND:Hand, foot, and mouth disease (HFMD) has become one of the most important infectious diseases recent years. Qingdao City has suffered from serious HFMD epidemic. This study aimed to describe epidemiological characteristics and investigate spatial-temporal distribution at town level in Qingdao City. METHOD:The surveillance data of HFMD during 2013-2018 were collected from the National Notifiable Disease Surveillance System. The global Moran's I statistic was used to detect the spatial autocorrelation of HFMD cases by ArcGis 10.0 software. Purely spatial and spatial-temporal analysis was used to detect epidemic clusters by SatScanTM v9.6 software. RESULTS:The annual average incidence of HFMD cases in Qingdao City from 2013 to 2018 was 123.16 per 100000, while the incidence rate of children≤5years old was 2879.80 per 100000. The majority (88.97%) of HFMD cases were aged within 0-5 years old and the males were 60.20%. Other enterovirus (EV), enteriovirus 71(EV71), and Coxsackievirus A16 (CA16) accounted for 48.75%, 30.91% and 20.34%. The seasonal peak was between May and October. HFMD had positive spatial autocorrelation at town level with global Moran's I from 0.19 to 0.31(P<0.001). Spatial-temporal cluster analysis detected six most likely clusters and three secondary clusters from 2013 to 2018. The most likely cluster was located in urban and urban-rural fringe areas. CONCLUSIONS:Urban and urban-rural fringe areas were the major locations of the clusters with other EV as the dominant pathogen between May and October. The findings suggested that the prevention and control of HFMD in Qingdao City should be focus on these high-risk periods and locations which had important public health significance for the allocation of public health resources.

Project description:IntroductionOpioid use disorder has grown rapidly over the years and is a public health crisis in the U.S. Although opioid use disorder is widely studied, relatively little is known about it among older adults. The goal of this study is to gain a better understanding of opioid use disorder among older Medicare beneficiaries over time and across several sociodemographic dimensions.MethodsData from the 2013-2018 Centers for Medicare & Medicaid Services Master Beneficiary Summary Files were analyzed in 2020 to examine the trends in opioid use disorder prevalence among Fee-for-Service Medicare beneficiaries aged ≥65 years. Utilizing the overarching opioid use disorder flag, trends in opioid use disorder prevalence were examined for the following sociodemographic dimensions: age, sex, race/ethnicity, and dual eligibility status (i.e., enrolled in both Medicare and Medicaid owing to low income). Chi-square tests were used to compare opioid use disorder prevalence across groups.ResultsSince 2013, estimated rates of opioid use disorder among older adults have increased by >3-fold overall in the U.S. Estimated opioid use disorder is more prevalent among the young-old (i.e., ages 65-69 years) beneficiaries than among other older adults, and dually eligible beneficiaries have consistently shared a heavier burden of opioid use disorder than Medicare-only beneficiaries. Regarding race/ethnicity, Blacks and American Indians/Alaskan Natives are more vulnerable to opioid use disorder than other groups.ConclusionsThe descriptive trends between 2013 and 2018 indicate that estimated opioid use disorder prevalence has increased greatly over the study period in all sociodemographic subgroups of older adults, highlighting an urgent challenge for public health professionals and gerontologists.

Project description:ObjectiveAccurate naloxone distribution data are critical for planning and prevention purposes, yet sources of naloxone dispensing data vary by location, and completeness of local datasets is unknown. We sought to compare available datasets in Massachusetts, Rhode Island, and New York City (NYC) to a commercially available pharmacy national claims dataset (Symphony Health Solutions).Data sources and study settingWe utilized retail pharmacy naloxone dispensing data from NYC (2018-2019), Rhode Island (2013-2019), and Massachusetts (2014-2018), and pharmaceutical claims data from Symphony Health Solutions (2013-2019).Study designWe conducted a descriptive, retrospective, and secondary analysis comparing naloxone dispensing events (NDEs) captured via Symphony to NDEs captured by local datasets from the three jurisdictions between 2013 and 2019, when data were available from both sources, using descriptive statistics, regressions, and heat maps.Data collection/extraction methodsWe defined an NDE as a dispensing event documented by the pharmacy and assumed that each dispensing event represented one naloxone kit (i.e., two doses). We extracted NDEs from local datasets and the Symphony claims dataset. The unit of analysis was the ZIP Code annual quarter.Principal findingsNDEs captured by Symphony exceeded those in local datasets for each time period and location, except in RI following legislation requiring NDEs to be reported to the PDMP. In regression analysis, absolute differences in NDEs between datasets increased substantially over time, except in RI before the PDMP. Heat maps of NDEs by ZIP code quarter showed important variations reflecting where pharmacies may not be reporting NDEs to Symphony or local datasets.ConclusionsPolicymakers must be able to monitor the quantity and location of NDEs in order to combat the opioid crisis. In regions where NDEs are not required to be reported to PDMPs, proprietary pharmaceutical claims datasets may be useful alternatives, with a need for local expertise to assess dataset-specific variability.

Project description:Macrolide usage in Japan exceeds that in Europe and the United States. Investigating the actual conditions in which macrolides are used is important for identifying further interventions for appropriate antimicrobial use; however, this situation has not been evaluated in Japan. Therefore, we aimed to clarify the number of macrolide prescriptions and their changes before and after implementation of the Antimicrobial Resistance (AMR) Action Plan. In addition, we also investigated the names of diseases for which macrolides have been prescribed and the number of days of prescription. A retrospective observational study was conducted using JMDC claims data from January 2013 to December 2018. The proportion of all oral antimicrobials and macrolides used during this period and the diseases for which macrolides were used in the 3 years before and after the AMR Action Plan were determined separately for acute (< 14 prescription days) and chronic (> 14 prescription days) diseases. The number of prescriptions for macrolides constituted approximately 30% of those for all oral antimicrobials; of these, clarithromycin accounted for approximately 60%. Most prescriptions for acute diseases were for common cold, whereas allergic and dermatological diseases were included among chronic diseases. The names of these illnesses did not change before and after the AMR Action Plan. Overall, these results indicate that appropriate macrolide use involves a review of their use for common cold along with appropriate evaluation of their long-term use for skin and allergic diseases. They also indicate the need for further fact-finding studies and ongoing AMR measures.

Project description:Inland crustal earthquakes usually occur in the brittle upper crust (0-20 km depths), but the 6 September 2018 Eastern Iburi earthquake (M 6.7) took place in southern Hokkaido with a focal depth of ~37 km, causing 41 fatalities and serious damage to the local infrastructure. The reason why this event was so deep and its causal mechanism are still unclear. In this work we study the three-dimensional P and S wave seismic attenuation (1/Q) structure in the source zone of the 2018 Iburi earthquake. Our results show that this event occurred at the boundary between the Sorachi-Yezo belt (low Q) and the dipping Northeastern (NE) Japan arc (high Q) that is descending beneath the Kuril arc. The collision between the NE Japan and Kuril arcs as well as fluids from dehydration of the subducting Pacific plate caused this big event and its unusual focal depth. Similar attenuation structures are revealed in source zones of the 1970 Hidaka earthquake (M 6.7) and the 1982 Urakawa-oki earthquake (M 7.1), suggesting that they were caused by similar processes. We think that large earthquakes will take place again on the active thrust faults in southern Hokkaido in the coming decades. Hence, we should pay much attention to the seismic risk and prepare for reduction of earthquake hazards there.

Project description:DNA copy number variations (CNVs) have been shown to be associated with cancer development and progression. The detection of these CNVs has the potential to impact the basic knowledge and treatment of many types of cancers, and can play a role in the discovery and development of molecular-based personalized cancer therapies. One of the most common types of high-resolution chromosomal microarrays is array-based comparative genomic hybridization (aCGH) methods that assay DNA CNVs across the whole genomic landscape in a single experiment. In this article we propose methods to use aCGH profiles to predict disease states. We employ a Bayesian classification model and treat disease states as outcome, and aCGH profiles as covariates in order to identify significant regions of the genome associated with disease subclasses. We propose a principled two-stage method where we first make inferences on the underlying copy number states associated with the aCGH emissions based on hidden Markov model (HMM) formulations to account for serial dependencies in neighboring probes. Subsequently, we infer associations with disease outcomes, conditional on the copy number states, using Bayesian linear variable selection procedures. The selected probes and their effects are parameters that are useful for predicting the disease categories of any additional individuals on the basis of their aCGH profiles. Using simulated datasets, we investigate the method's accuracy in detecting disease category. Our methodology is motivated by and applied to a breast cancer dataset consisting of aCGH profiles assayed on patients from multiple disease subtypes.

Project description:BackgroundCatastrophic health expenditure (CHE) is an important indicator of measuring health inequality. Previous studies mainly focused on specific vulnerable populations rather than a wider range of vulnerable areas through panel data. Rural China is often associated with an underdeveloped economy and insufficient health resources. This study aims to update the information on the extent of and trends in the incidence and inequality of CHE among the households of rural China through longitudinal survey data.MethodsData were obtained from three waves of the China Health and Retirement Longitudinal Study (CHARLS): 2013, 2015, and 2018. In total, 2,575 households were included in the analysis. CHE was defined as household health expenditures exceeding 40% of non-food expenditures. Inequality in CHE was measured using the concentration curve and concentration index. The contribution to CHE inequality was decomposed using the concentration index decomposition method.ResultsThe incidence of CHE was 0.2341 (95% CI: 0.22, 0.25) in 2013, 0.2136 (95% CI: 0.20, 0.23) in 2015, and 0.2897 (95% CI: 0.27, 0.31) in 2018 in rural China. The concentration curve lay above the equality line, and the concentration index was negative: -0.1528 (95% CI: -0.1941, -0.1115) in 2013, -0.1010 (95% CI: -0.1442, -0. 0577) in 2015, and -0.0819 (95% CI: -0.1170, -0.0467) in 2018. Economic status, age, and chronic diseases were the main contributors to inequality in CHE.ConclusionsThe incidence of CHE in rural China displayed an upward trend from 2013 to 2018, although it was not continuous. Furthermore, a strong pro-low-economic inequality in CHE existed in rural China. Mainly economic status, age, and chronic diseases contributed to this pro-low-economic inequality. Health policies to allocate resources and services are needed to satisfy the needs of rural households and provide more accessible and affordable health services. More concern needs to be directed toward households with chronic diseases and older persons to reduce the incidence of CHE and promote health equality.
