Type II Medial Meniscus Root Repair With Peripheral Release for Addressing Meniscal Extrusion.

ABSTRACT: Medial meniscal root tears are often disabling injuries that can occur in isolation during low-velocity, deep knee flexion maneuvers in middle-aged patients. The most common meniscal root tear pattern is a radial tear near the root attachment (type II). Root tears are often associated with meniscal extrusion, identified on magnetic resonance imaging. Relocation of the meniscal root to its anatomic center is a reported current difficulty faced by surgeons during surgical repair. However, this can be achieved via sufficient peripheral release of the posteromedial capsular attachment of the medial meniscus. The purpose of this Technical Note is to describe the authors' current surgical technique for medial meniscus root repair with a peripheral release for addressing meniscal extrusion. Classifications: level I (knee); level II (meniscus).

Meniscal Root Repair With Mini-Open Medial Collateral Ligament Release.

Project description:Meniscal root repair is becoming more common with increasing knowledge regarding the function of the meniscal root and its role in long-term joint preservation, as well as improved surgical techniques and instrumentation. Despite this, adequate visualization of the posterior horn of the medial meniscus may be challenging, particularly in those with tight medial compartments. Instrument handling and attempt to visualize and repair the meniscal tear may lead to iatrogenic chondral injuries in these instances. We describe our preferred technique for improved visualization via mini-open release of the distal medial collateral ligament that allows for increased joint distension, increased access to the posterior horn of the medial meniscus, and the ability to repair the released medial collateral ligament.

Knotless Medial Meniscus Posterior Root Repair.

Project description:Medial meniscus posterior root tears are common injuries, especially in the Asian world. This injury must be repaired where indicated to restore knee biomechanics and prevent arthritis. Suturing the meniscus using suture tapes provides good hold of the tissue. The use of a 70° arthroscope and creation of a transseptal portal improve visualization of the posterior compartment. Creation of a high posteromedial portal achieves the correct trajectory for the suture anchor insertion. A knotless suture anchor can be used to fix the posterior root at its anatomic attachment site. This obliterates the need for transtibial drilling for a suture pull-out repair or for knot tying and suture shuttling as for a conventional suture anchor.

Association between postoperative meniscal extrusion and clinical outcomes of the pullout fixation technique for medial meniscus posterior root tear.

Project description:ObjectiveTo evaluate the correlation between meniscal extrusion in the coronal plane with clinical and radiological outcomes at a minimum of 1 year after transtibial pullout fixation for medial meniscus posterior root tear (MMPRT).MethodsData from 30 patients (five males, 25 females) with MMPRT who underwent arthroscopic root fixation by the transtibial pullout technique between 2011 and January 2015 were retrospectively analyzed. The mean patient age was 60.7 ± 8.3years. The median time between injury and surgery was 3.8 months (range, 0.5-15 months). Medial meniscus radial displacement was assessed on magnetic resonance imaging performed preoperatively and at 1 year after fixation. The patients were divided into two groups based on the presence on magnetic resonance imaging of decreased meniscal extrusion (group A) or increased meniscal extrusion (group B) at 1 year postoperatively compared with preoperatively. The two groups were compared regarding factors including age at surgery, sex, body mass index, time between injury and surgery, and the Lysholm score and Kellgren-Lawrence (K-L) grade preoperatively and at 1 year postoperatively.ResultsThere were seven knees in group A, and 23 in group B. Both groups had a significantly improved Lysholm score postoperatively compared with preoperatively (p < 0.001). Although the postoperative K-L grade was significantly worse than the preoperative K-L grade in group B (p < 0.001), the postoperative K-L grade did not worsen postoperatively in group A.ConclusionThe K-L grade did not worsen postoperatively in the group with decreased meniscal extrusion at 1 year after transtibial pullout fixation for MMPRT compared with preoperatively.

Arthroscopic Transtibial Medial Meniscus Posterior Root Repair Using the "Meniscal Track" to Locate the Anatomical Footprint.

Project description:The medial meniscus posterior root tears are a common problem in early elder patients. From a biomechanical study, the anatomical repair demonstrated a restored contact area and contact pressure than the nonanatomical repair. Nonanatomical repair of the medial meniscus posterior root resulted in decreased tibiofemoral contact area and increased contact pressure. Various surgical repair techniques were reported in the literature. However, there was no reported precise arthroscopic landmark to define the anatomical footprint of the posterior root attachment of the medial meniscus. We propose the "meniscal track", an arthroscopic landmark to guide the location of the anatomical footprint of the medial meniscus posterior root attachment.

Peripheral Stabilization Suture to Address Meniscal Extrusion in a Revision Meniscal Root Repair: Surgical Technique and Rehabilitation Protocol.

Project description:Meniscal root tears are an increasingly recognized condition. These tears can cause the meniscus to become extruded outside the joint, which can diminish the biomechanical functionality of the meniscus. Anatomic repair of the meniscal root has previously been described, but this surgical procedure may not adequately address severe extrusion of the meniscal tissue. Additionally, when a primary anatomic repair fails, meniscal extrusion can increase, which can possibly accelerate joint degeneration if untreated. Therefore, the purpose of this Technical Note is to describe our surgical technique for revision medial meniscal root repair with a peripheral stabilization suture to address medial meniscal root tears with severe meniscal extrusion.

Project description:Repair of medial meniscus posterior root tear is important in preventing rapid progression of knee osteoarthritis. There are many repair techniques for medial meniscus posterior root tears, and good clinical results have been reported. Conversely, in terms of improving extrusion and healing meniscal injuries, sufficient results have not been obtained. Reconstruction of the medial meniscus posterior root can restore meniscal hoop action and normal knee biomechanics. Moreover, pull-out repair technique provides strong traction. We describe arthroscopic medial meniscus posterior root reconstruction and pull-out repair technique combined technique for medial meniscus root tear.

Medial Meniscus Posterior Root Repair Using a Transtibial Technique.

Project description:The meniscal roots are critical in maintaining the normal shock absorbing function of the meniscus. If a meniscal root tear is left untreated, meniscal extrusion can occur rendering the meniscus nonfunctional resulting in degenerative arthritis. Two main repair techniques are described: (1) suture anchors (direct fixation) and (2) sutures pulled through a tibial tunnel (indirect fixation). Meniscal root repair using a suture anchor technique is technically challenging requiring a posterior portal and a curved suture passing device that can be difficult to manipulate within the knee. We present a technique for posterior medial meniscus root repair using 3 sutures (1 leader, 2 cinch), standard arthroscopy portals, and transtibial fixation. Overall, this technique simplifies a challenging procedure and allows for familiarity and efficiency.

Delta-Grip Stitch for Medial Meniscus Posterior Root Repair.

Project description:Medial meniscus posterior root tear (MMPRT) is a common medial meniscus injury among elderly patients and often necessitates repair. Although the clinical outcomes of MMPRT repair have demonstrated improvements, subsequent second-look arthroscopy shows poor healing postrepair. Consequently, numerous repair techniques have been reported. Herein, we introduce a simple locking-suture method, the "delta-grip" stitch, for MMPRT repair. This technique exhibited sufficient pullout strength compared with conventional suturing methods, offering promising prospects for enhancing the outcomes of MMPRT repair.

Medial Meniscus Repair Using "AJStitch": A Cost-Effective All-Inside All-Suture Meniscal Repair System.

Project description:All-inside techniques are based on devices that use PEEK (polyether ether ketone) or biocomposite anchors placed at extracapsular locations such as anchorage points over which the sutures are tied. However, because of complications like irritability and intra-articular migration of these hard anchors, suture-based all-inside meniscal repair systems are now gaining popularity. Although these devices have advantages over conventional all-inside devices, they are costly, thus limiting their widespread use. The AJStitch meniscus repair system uses a specially designed spear to insert all-suture anchors, which can be made using a No. 5 Netbond and 2-0 Ultranet. This Technical Note describes the use of this system. It is an all-inside, all-suture meniscus repair system that provides a locally made, cost-effective option for posterior horn medial meniscus repair.
