The clinical outcome of the 'Surgilig' technique for the reconstruction of acromioclavicular dislocations: A systematic review.

ABSTRACT: Purpose:To investigate whether the 'Surgilig' technique is safe and effective for the treatment of patients suffering from acromioclavicular joint (ACJ) dislocations graded as Rockwood's type III or higher. Methods:A systematic review was conducted according to Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines. Results:The failure rate of the "Surgilig" implant was very low (3.5%), while patients' satisfaction was high (88.3%). However, the quality of most studies was low. Conclusions:There is low evidence to show that the reconstruction of ACJ dislocations with the 'Surgilig' technique could be a safe and effective treatment.

Project description:Acromioclavicular joint dislocations are a common injury particularly among contact sports players. There has been an increasing trend toward arthroscopic management of these injuries. To date, these reconstructions have primarily addressed superoinferior instability by reconstructing the coracoclavicular ligaments. We describe an all-arthroscopic technique for reconstruction of the coracoclavicular ligaments using Arthrex ABS TightRopes (Arthrex, Naples, FL), with additional stabilization of the superior acromioclavicular joint capsule using an anchor-based suture bridge to address anteroposterior instability.

Double figure of eight technique using fiber tape for acute high-grade acromioclavicular joint dislocations - Reconstruction technique.

Project description:Acromioclavicular (AC) joint dislocations are not uncommon injuries. High-grade dislocations can be managed by arthroscopic as well as open techniques, but the most important thing is to address both vertical and horizontal stability. The purpose of this study is to evaluate the functional outcome of a cost effective double figure of eight technique of stabilizing both coracoclavicular and acromioclavicular ligaments in acute high-grade disruption (Type IIIB -VI) using fibre tape. This study was performed at our tertiary care centre from 2017 to 2020 after appropriate IRB approval and patient consent. Out of 134 cases, 23 high-grade AC joint disruptions treated with our technique were included in the study. Demographic data analysis, pre-op and post-op Constant score and UCLA scoring were done to assess the functional outcome; Coracoclavicular distance (CCD) and CCD ratio were used to assess the radiological outcome. Significant improvement in Constant and UCLA shoulder scores were observed from pre-op to final follow up. With respect to the radiological outcome, all cases had a significant improvement in CCD and CCD ratio from baseline to final follow-up. To conclude, our double figure of eight technique is more anatomical, addresses both vertical and horizontal stability, easily reproducible, cost-effective and safe technique.

Surgical Technique for Arthroscopy-Assisted Anatomical Reconstruction of Acromioclavicular and Coracoclavicular Ligaments Using Autologous Hamstring Graft in Chronic Acromioclavicular Joint Dislocations.

Project description:Injuries to the acromioclavicular (AC) joint are becoming common with contact sports and bike accidents. It is well known that in AC dislocations, the first structure to fail is the AC capsule followed by the trapezoid and conoid ligaments. The function of these ligaments must be restored to restore the anatomy and physiology of the AC joint to get the best results. Until now, no technique has emerged as the gold standard for restoration of the AC joint anatomy and function. In our technique, the stress is on recreating the anatomy to make it more individualized based on individual variations. This Technical Note describes a procedure to reconstruct the coracoclavicular ligaments and AC joint by an arthroscopy-assisted technique. Arthroscopy helps to diagnose additional intra-articular pathologies that can be treated simultaneously, and better preparation of the undersurface of the coracoid helps in bone-to-graft healing. Our approach is more individualized as clavicle tunneling is done according to the size of the coracoid base instead of a fixed distance. Vertical stability is provided by coracoclavicular ligament reconstruction, horizontal stability is provided by AC ligament reconstruction, and the articular disc is recreated by soft-tissue graft interposition, thus restoring the complete anatomy.

Arthroscopically Assisted Reconstruction of Acute Acromioclavicular Joint Dislocations: Anatomic AC Ligament Reconstruction With Protective Internal Bracing-The "AC-RecoBridge" Technique.

Project description:An arthroscopically assisted technique for the treatment of acute acromioclavicular joint dislocations is presented. This pathology-based procedure aims to achieve anatomic healing of both the acromioclavicular ligament complex (ACLC) and the coracoclavicular ligaments. First, the acromioclavicular joint is reduced anatomically under macroscopic and radiologic control and temporarily transfixed with a K-wire. A single-channel technique using 2 suture tapes provides secure coracoclavicular stabilization. The key step of the procedure consists of the anatomic repair of the ACLC ("AC-Reco"). Basically, we have observed 4 patterns of injury: clavicular-sided, acromial-sided, oblique, and midportion tears. Direct and/or transosseous ACLC repair is performed accordingly. Then, an X-configured acromioclavicular suture tape cerclage ("AC-Bridge") is applied under arthroscopic assistance to limit horizontal clavicular translation to a physiological extent. The AC-Bridge follows the principle of internal bracing and protects healing of the ACLC repair. The AC-Bridge is tightened on top of the repair, creating an additional suture-bridge effect and promoting anatomic ACLC healing. We refer to this combined technique of anatomic ACLC repair and protective internal bracing as the "AC-RecoBridge." A detailed stepwise description of the surgical technique, including indications, technical pearls and pitfalls, and potential complications, is given.

Arthroscopic Acromioclavicular Joint Treatment With Coracoclavicular Fixation and Allograft Coracoclavicular Ligament Reconstruction for Acute Acromioclavicular Dislocations.

Project description:Treatment of severe acromioclavicular joint injuries remains controversial and has evolved over the past 4-plus decades. Although several variations on reconstruction exist, an ideal technique will likely use a combination of coracoclavicular ligament reconstruction with suture backup stabilization, minimal drill holes to reduce the risk of fracture, arthroscopic-assisted guidance for anatomic graft and suture placement in and around the coracoid, and fluoroscopic-aided reduction to ensure an anatomic acromioclavicular joint. The objective of this Technical Note is to describe an arthroscopic-assisted coracoclavicular ligament reconstruction with allograft using fluoroscopically guided and cerclage-controlled anatomic reduction of the acromioclavicular joint.

Treatment of acromioclavicular dislocations with a concomitant coracoid fracture: a systematic review of 37 patients.

Project description:BackgroundAcromioclavicular (AC) dislocations with a concomitant fracture of the coracoid process (CP) are rare and there is ambiguity on treatment options. This systematic review was performed to address the clinically relevant question: what are the shoulder functions, union rates, and expected time until return to daily life in patients with a dislocation of the AC joint with a concomitant CP fracture after (1) nonsurgical treatment, (2) sole fixation of the AC joint, and (3) fixation of both the AC joint and the coracoid process?MethodsStudies were identified by conducting an online. Thirty records met the inclusion criteria and were suitable for data extraction.ResultsA total of 37 shoulders from 37 patients were included. Surgical treatment was provided to 22 out of 37 patients, and 15 patients had nonsurgical treatment. Out of the surgically treated patients, 12 patients were treated with a fixation of both the AC joint and the CP, 9 patients with a sole fixation of the AC joint, and 1 patient with a sole fixation of the CP.ConclusionExisting literature does not indicate that one treatment option is superior, and more data are needed to guide evidence-based decisions on this rare injury.

Clinical outcomes of arthroscopic and navigation-assisted two tunnel technique for coracoclavicular ligament augmentation of acute acromioclavicular joint dislocations.

Project description:BackgroundThe purpose of this study was to present a navigated image-free augmentation technique for the acromioclavicular joint (ACJ) and coracoclavicular (CC) ligaments and to report the clinical and radiological outcomes.MethodsFrom 2013 to 2018, 35 eligible patients were treated with our navigated image-free ACJ- and CC-augmentation technique. The average follow-up was 3 years. Follow-up evaluations included the Constant-Murley Score, subjective shoulder value, Taft score, and the acromioclavicular joint instability (ACJI) score. The patients' quality of life was assessed using the EuroQol-5D (EQ-5D) questionnaire. In addition, in accordance with the instability criteria, radiographs were evaluated before surgery, after surgery, and during follow-up.ResultsOverall, 25 patients (71%) suffered an acute type V disruption, 5 (14%) had a type IV disruption, and 5 (14%) had an acute Rockwood type IIIb injury. The mean Constant-Murley Score was 90 (range: 56-100; p = 0.53) on the injured side, and the mean subjective shoulder value was 92% (range: 80-100%). The mean Taft and ACJI scores were 10 (range: 4-12) and 86 (range: 34-100), respectively and the mean EQ-5D was 86 (range: 2-100). The mean CC difference of the injured side was 4 mm (range: 1.9-9.1 mm) at follow-up, which was not significantly different than that of the healthy side (p = 0.06). No fractures in the area of the clavicle or the coracoid were reported.ConclusionsThe arthroscopic- and navigation-assisted treatment of high-grade ACJ injuries in an anatomical double-tunnel configuration yields similar clinical and radiological outcomes as the conventional technique using an aiming device. Precise positioning of the navigation system prevents multiple drillings, which avoids fractures.

Anatomic Acromioclavicular Joint Reconstruction With Semitendinosus Allograft: Surgical Technique.

Project description:Acromioclavicular joint separations are common shoulder injuries in the active patient population. Nonoperative management is recommended for Rockwood type I and II injuries, whereas surgical reconstruction is recommended for type IV and VI separations. The management for type III and V injuries is more controversial and is determined on a case-by-case basis. A multitude of surgical reconstruction techniques exist, and there is little evidence to support one technique over another. The anatomic technique aims at reconstructing the coracoclavicular ligaments and bringing the clavicle back into its anatomic position. When the anatomic technique is augmented with a graft, biomechanical studies have shown superior reconstruction strength and stability compared with standard nonanatomic techniques. Additionally, anatomic reconstruction allows for better cosmesis and functional outcome measures at midterm follow-up compared with nonanatomic techniques. In this Technical Note, we describe our preferred technique for anatomic repair of acromioclavicular joint separation using a semitendinosus allograft.

Project description:Acromioclavicular joint dislocation is a common pathology, affecting mostly young male patients. High-grade injuries require surgical treatment, but evidence is lacking regarding a gold standard technique. Chronic cases frequently are treated with graft reconstruction, but complications and availability remain as a limitation for autograft and allograft use, respectively. The objective of this Technical Note is to describe the treatment of chronic acromioclavicular dislocations by a reconstruction made with a local tendon graft, the pectoralis minor, in addition to transferring the coracoacromial ligament as a horizontal stabilizer.
