Anterior Capsule Reconstruction of the Native Hip: A Technique Guide.

ABSTRACT: This is a technique guide for open anterior capsule reconstruction in unstable native hips due to Ehlers-Danlos syndrome. Ehlers-Danlos syndrome is a connective tissue disease that predisposes an individual to hypermobility of their soft tissues and recurrent subluxations and dislocations of their joints. This may lead to constant instability, guarding of the hip, and subsequent arthritic changes and poor quality of life. We demonstrate anterior capsule reconstruction of the hip using an Achilles tendon allograft performed through a direct anterior approach to the hip (Heuter's approach). This technique guide outlines in detail the surgical steps required to accomplish this reconstructive procedure. Furthermore, a narrated surgical video is provided that clearly depicts the anticipated result of the increased constrained provided to the hip from the anterior capsule reconstruction using an Achilles tendon allograft.

Project description:Iatrogenic hip instability is increasingly recognized as a cause of persistent pain and disability after hip arthroscopy. Many authors currently advocate capsular repair to reduce postoperative instability. However, anatomic deficiencies in the anterosuperior capsule can prevent a functional capsular repair, particularly in the revision setting. Capsular reconstruction has been shown to restore biomechanical stabilization in cadaveric models and improve short-term patient outcomes in patients with primary hip arthroscopy failure. Arthroscopic hip capsular reconstruction is technically challenging, largely owing to complex suture management and difficulties with graft placement and sizing. This article describes the capsular reconstruction technique, detailing the technical aspects of anterosuperior capsular defect identification; capsular preparation; suture management; and dermal allograft sizing, preparation, and positioning.

Anterior Capsule Reconstruction Technique With an Acellular Dermal Allograft.

Project description:An irreparable subscapularis may have a debilitating influence on glenohumeral joint biomechanics. Traditional treatment approaches are focused on tendon transfers, among which the most popular are pectoralis major and latissimus dorsi transfers. However, these techniques present significant retear rates, possible nerve injuries, and altered biomechanics. Other techniques like tibialis anterior or iliotibial autograft grafting also have many reported failures. We describe an all-arthroscopic anterior capsule reconstruction technique with an acellular dermal graft.

Anterior Cruciate Ligament Reconstruction With Double-Bundle Internal Bracing: A Technique Guide.

Project description:There has been a recent increase in interest for double-bundle (db) anterior cruciate ligament reconstructions (ACLRs). Although this surgical technique has shown to improve rotational and translational stability, the literature has been inconsistent, finding its graft failure rates to be superior to that of the single-bundle (sb) ACLR. So far, no studies have reported the sb ACLR to be superior to db ACLR. It is possible that the db ACLR provides the most benefit in greatest-risk patients such as young athletes, female athletes, patients with generalized ligamentous laxity, and those undergoing revision ACLRs. The senior author's db ACLR technique incorporates a FiberTape internal brace, which is applied to both bundles. Despite inconsistencies in the literature, we suspect that autologous db ACLR with internal bracing could reduce graft failure rates and provide earlier return to preinjury activity level in high-risk patients compared with sb ACLR.

Anterior Capsule Reconstruction for Irreparable Subscapularis Tears.

Project description:A subscapularis complete tear presents its own challenges in management. The glenohumeral biomechanics and force couple are reliant on a competent and functioning subscapularis muscle. An irreparable subscapularis makes those same challenges even more difficult to address. Traditionally, this problem has been addressed with tendon transfers, including pectoralis major or latissimus dorsi. These techniques can alter the ideal biomechanics of the shoulder and have high rates or failure. Iliotibial autograft or tibialis anterior have also been wrought with high failure rates. Recently, the superior capsular reconstruction has been described for irreparable tears of the supraspinatus and infraspinatus. Theoretically, this procedure can act as a check rein against subluxation, and may serve to reconnect the force couples of the rotator cuff. Anterior escape may represent a similar challenge when the irreparable rotator cuff tendon is the subscapularis. To address this, we describe an open anterior capsule reconstruction technique with an acellular dermal graft. We theorize that this procedure may serve in a similar capacity to its superior capsular counterpart.

Capsular Reconstruction of the Hip Using Modified Kite Technique: A Technical Guide for Efficient Graft Management and Fixation.

Project description:Preserving capsular integrity has become an important principle of hip preservation surgery given the increasingly recognized deleterious effects of instability in cases of capsular insufficiency. When capsular tissue is deficient, capsular reconstruction may be indicated to restore function of the iliofemoral ligament and improve hip biomechanics. To date, few studies have presented technical guidance on performing arthroscopic capsular reconstruction of the hip. In this Technical Note, we introduce a modified kite technique for arthroscopic entry, control, and fixation of a capsular reconstruction graft. Similar to flying a kite with multiple fly lines, and to the previously described kite technique for hip labral reconstruction, the principles of this method are founded on the belief that control sutures within a pulley system facilitate safe and efficient graft management during capsular reconstruction procedures.

Superior Capsule Reconstruction Technique Using an Acellular Dermal Allograft.

Project description:Chronic, massive, irreparable rotator cuff tears remain one of the most challenging pathologies in shoulder surgery to treat. Because of this, many treatment options exist for the management of chronic retracted rotator cuff tears. Superior capsule reconstruction is one option that provides the potential to restore and rebalance the force couples necessary for dynamic shoulder function. We describe an all-arthroscopic technique using an acellular dermal allograft for superior capsule reconstruction indicated for patients with a deficient superior rotator cuff. This technique provides an option for patients with an irreparable rotator cuff tear without compromising future treatment options. Although this is a relatively new and unproven method for treating chronic irreparable rotator cuff tears, our short-term results are promising. Nevertheless, larger studies with long-term follow-up are required to fully evaluate the success of this technique.

Surgical technique: a simple technique for closing the capsule of the hip in posterolateral approach total hip arthroplasty.

Project description:BackgroundRepair of the posterior soft tissue via a posterolateral approach following total hip arthroplasty has been reported to mitigate the risk of dislocation. The joint capsule serves as the primary protective barrier, and while previous techniques for closure have been predominantly complex, we present a novel, simplified technique for joint capsule closure.MethodsA total of 215 patients who underwent primary THA via the posterolateral approach at our hospital from May 2018 to December 2022 were retrospectively enrolled in this study: 111 patients had their joint capsules sutured using traditional methods, while 114 patients had their joint capsules sutured using the new technology.ResultsCompared to the traditional suture group, the novel joint capsular suture group exhibited shorter operation times (p = 0.004) and reduced postoperative drainage volumes (p < 0.001). There were no significant differences in intraoperative blood loss, acetabular anteversion angle and abduction angle between the two groups. Although the Harris scores at 2 weeks, 3 months, and 6 months after surgery were marginally greater in the novel joint capsular suture group than in the traditional suture group, these differences did not reach statistical significance. Regarding adverse reactions observed during follow-up, hematoma occurred in seven patients (6.6%), and dislocation occurred in four patients (3.6%) in the traditional suture group; conversely, within the new joint capsule suture group, there were only two patients (1.8%) of hematoma with no instances of dislocation recorded thus far.ConclusionsThe preliminary results show that this new capsular closure technique allows the joint capsule to close more tightly and is an effective technique for reducing the rate of early postoperative dislocation.

A Modified Femtosecond Laser Technique for Anterior Capsule Contraction Syndrome.

Project description:Anterior capsule contraction syndrome (ACCS) is a rare, late complication of cataract surgery, associated with impairment of visual function. In this paper, we describe a new surgical technique to treat ACCS by femtosecond laser procedure. The femtosecond laser was used to perform an anterior capsulotomy with a customized size, in order to avoid IOL damage. After ophthalmic viscosurgical device injection in the anterior chamber, the anterior capsule flap was separated from the IOL surface by gentle hydrodissection. This manoeuvre enabled an easy and safe removal of the fibrotic material by vitreal microscissors. Our technique allowed a complete removal of the fibrotic material and opening of the capsule, with immediate complete visual acuity recovery without IOL damage. In conclusion, femtosecond laser appears to be safe and effective for treatment of ACCS with long-lasting efficacy.

Arthroscopic capsule reconstruction in the hip using iliotibial band allograft.

Project description:The hip capsule has been identified as an important static stabilizer of the hip joint. Despite the intrinsic bony stability of the hip socket, the capsule plays a key role in hip stability, particularly at the extremes of motion, and the iliofemoral ligament is the most important stabilizer in extension and external rotation. Patients who do not undergo capsular closure or plication may continue to complain of hip pain and dysfunction postoperatively, likely because of microinstability or muscle invagination into the capsular defect, and high-resolution magnetic resonance imaging or magnetic resonance arthrography will identify the capsular defect. Seen primarily in the revision setting, capsular defects can cause recurrent stress at the chondrolabral junction. An attempt at secondary closure can be challenging because of capsular limb adherence to the surrounding soft tissues. Therefore reconstruction may be the only possible surgical solution for this problem. We describe our new surgical technique for arthroscopic hip capsular reconstruction using iliotibial band allograft.

Technique for Double Row Superior Capsule Reconstruction with Dermal Allograft.

Project description:Superior capsular reconstruction (SCR) is increasingly being used as a procedure for addressing irreparable rotator cuff tears. The procedure was initially described for failed rotator cuff repairs where the retears are severely retracted and when grade 3-4 fatty infiltration and atrophy exist. The SCR procedure can also be considered for irreparable rotator cuff tears in patients that are either too young or too high demand to be appropriate candidates for arthroplasty. Early short and medium term follow up studies support SCR with favorable outcomes compared with other salvage procedures.
