A protocol of hepatic volume measurement using magnetic resonance imaging in individuals from the Eastern Brazilian Amazon population.

ABSTRACT: Determination of hepatic volume is an important preoperative procedure and is done through imaging exams or standard liver volume (SLV) formulas developed based on the biotype of each population. In the absence of a specific SLV formula for the Brazilian Eastern Amazon population, the measurement of liver volume is made with reference values from other populations. The aim of study was to compare the hepatic volume in healthy residents from the Brazilian Eastern Amazon population obtained with magnetic resonance imaging (MRI) and recommended SLV formulas validated to other populations. This was a Observational, cross-sectional study. Anthropometric data of 42 healthy individuals aged 18-60 years of both sexes was collected to measure the liver volume through SLV formulas calculations and MRI. MRI shows similarity with the Western European SLV liver volume values and significant differences with the Japan SLV formula, mainly for women, with a moderate-to-weak correlation with the MRI measurements. There was a strong correlation between weight and body surface area in male patients analysed with measurements of the liver volume by the MRI and SLV formulas. The SLV formula based on the Western European population could be used in the absence of a specific formula for individuals living in the Amazon region. The results suggest that liver measurement formulas should take into consideration the sex of individuals, as well as the development of a specific SLV formula for the Eastern Amazon population and the conduction of similar studies in other Brazilian regions.

Project description:ImportanceCharacterizing longitudinal patterns of regional brain volume changes in a population with normal cognition at the individual level could improve understanding of the brain aging process and may aid in the prevention of age-related neurodegenerative diseases.ObjectiveTo investigate age-related trajectories of the volumes and volume change rates of brain structures in participants without dementia.Design, setting, and participantsThis cohort study was conducted from November 1, 2006, to April 30, 2021, at a single academic health-checkup center among 653 individuals who participated in a health screening program with more than 10 years of serial visits.ExposureSerial magnetic resonance imaging, Mini-Mental State Examination, health checkup.Main outcomes and measuresVolumes and volume change rates across brain tissue types and regions.ResultsThe study sample included 653 healthy control individuals (mean [SD] age at baseline, 55.1 [9.3] years; median age, 55 years [IQR, 47-62 years]; 447 men [69%]), who were followed up annually for up to 15 years (mean [SD], 11.5 [1.8] years; mean [SD] number of scans, 12.1 [1.9]; total visits, 7915). Each brain structure showed characteristic age-dependent volume and atrophy change rates. In particular, the cortical gray matter showed a consistent pattern of volume loss in each brain lobe with aging. The white matter showed an age-related decrease in volume and an accelerated atrophy rate (regression coefficient, -0.016 [95% CI, -0.012 to -0.011]; P < .001). An accelerated age-related volume increase in the cerebrospinal fluid-filled spaces, particularly in the inferior lateral ventricle and the Sylvian fissure, was also observed (ventricle regression coefficient, 0.042 [95% CI, 0.037-0.047]; P < .001; sulcus regression coefficient, 0.021 [95% CI, 0.018-0.023]; P < .001). The temporal lobe atrophy rate accelerated from approximately 70 years of age, preceded by acceleration of atrophy in the hippocampus and amygdala.Conclusions and relevanceIn this cohort study of adults without dementia, age-dependent brain structure volumes and volume change rates in various brain structures were characterized using serial magnetic resonance imaging scans. These findings clarified the normal distributions in the aging brain, which are essential for understanding the process of age-related neurodegenerative diseases.

Project description:ObjectivesTo evaluate the relationship between biopsy-assessed hepatic steatosis, magnetic resonance imaging (MRI)-assessed proton density fat fraction (PDFF), and magnetic resonance elastography (MRE)-assessed liver stiffness measurement (LSM), in patients with or at risk for nonalcoholic fatty liver disease (NAFLD).MethodsA retrospective study was performed, encompassing 256 patients who had a liver biopsy and MRI/MRE examination performed within 1 year. Clinical and laboratory data were retrieved from the electronic medical record. Hepatic steatosis and fibrosis were assessed by histopathological grading/staging. First, we analyzed the diagnostic performance of PDFF for distinguishing hepatic steatosis with the receiver operating characteristic analyses. Second, variables influencing LSM were screened with univariant analyses, then identified with multivariable linear regression. Finally, the potential relationship between PDFF and LSM was assessed with linear regression after adjustment for other influencing factors, in patients with diagnosed steatosis (PDFF ≥ 5%).ResultsThe diagnostic accuracy of PDFF in distinguishing steatosis grades (S0-3) was above 0.82. No significant difference in LSM was found between patients with S1, S2, and S3 steatosis and between all steatosis grades after patients were grouped according to fibrosis stage. No statistically significant relationship was found between the LSM and PDFF (estimate = - 0.02, p = 0.065) after adjustment for fibrosis stage and age in patients with diagnosed steatosis (PDFF ≥ 5%).ConclusionsIn patients with NAFLD, the severity of hepatic steatosis has no significant influence on the liver stiffness measurement with magnetic resonance elastography.Key points• The MRI-based proton density fat fraction provides a quantitative assessment of hepatic steatosis with high accuracy. • No significant effect of hepatic steatosis on MRE-based liver stiffness measurement was found in patients with S1, S2, and S3 steatosis and between all steatosis grades after patients were grouped according to fibrosis stage. • After adjusting for fibrosis stage and age, there was no statistically significant relationship between liver stiffness and proton density fat fraction in patients with hepatic steatosis (p = 0.065).
