Bilateral Femoral Neuropathy: A Rare Complication of Drug Overdose due to Prolonged Posturing in Lithotomy Position.

ABSTRACT: Background:Bilateral femoral neuropathy is an uncommon complication of various surgical and nonsurgical procedures, such as pelvic/abdominal surgery or vaginal delivery. Case Report. We report a case of a 41-year-old male who was found unresponsive against the wall in a "lithotomy-type" position with both knees flexed at approximately 90 degrees and both hips flexed and externally rotated at approximately 90 and 60 degrees, respectively, 24-48 hours after a drug overdose (combination of dihydrocodeine, paracetamol, diazepam, and amitriptyline). During his recovery, he complained of severe bilateral proximal lower limb weakness and bilateral distal lower limb pain and allodynia. His symptoms were initially attributed to critical illness myopathy/neuropathy (CIMN). However, thorough clinical and neurophysiological evaluation revealed that his symptoms were due to severe bilateral femoral neuropathies. Conclusions:To our knowledge, this is the first reported case of bilateral femoral nerve palsy due to prolonged posturing in a "lithotomy-type" position in the context of a drug overdose.

Project description:BackgroundThe lithotomy position (LP) may pose limitations and discomfort for elderly patients and those with a history of lower limb surgery, potentially leading to an increased risk of complications. And the LP is the conventional position during flexible ureteroscopic lithotomy for the treatment of ureteral calculi. However, it has some disadvantages, such as peripheral nerve injury and deep venous thrombosis in the lower limbs, etc. Therefore, we performed a new approach, which is named as modified dorsal recumbent position (MDRP). Currently, there is a lack of systematic analysis and standardization regarding the surgical positioning for flexible ureteroscopic lithotripsy. The objective of the study was to assess whether there were any disparities in the overall duration of the procedure when comparing the MDRP with the LP. The investigation of the optimal position for flexible ureteroscopic lithotomy is essential for enhancing patient safety and comfort.MethodsThis is a prospective, multicenter, randomized clinical trial. A total of 144 patients with renal or ureteral calculi from April 2021 to June 2022 were enrolled. Eligible patients were randomized to the MDRP group (n=72) or LP group (n=72). The patient's demographics, the placement of the surgical position (time of position placement, time of disinfection and towel laying, time of position return, degree of medical fatigue) and the operation safety (time of operation, time of ureteroscope from bladder neck to ureteral orifice, heart rate, blood pressure) of two groups were compared and analyzed.ResultsBetween the two groups, the body positioning time (93.8±31.6 vs. 134.8±40.1 s, P=0.02), operation time (71.8±36.7 vs. 77.7±48.6 min, P=0.04), the time from the bladder neck to the ureteral orifice of the flexible ureteroscope spent by the doctors (3.4±4.7 vs. 10.3±14.7 s, P<0.001) and incidence rate of patient's lower limb soreness (19.4% vs. 49.7%, P=0.01) in the MDRP group were significantly shorter than those in the LP group. However, there was no significant difference in the stone removal rate (87.6% vs. 85.4%, P=0.09) or postoperative hospitalization days (4.3±1.4 vs. 4.1±1.6 d, P=0.08) between the two groups.ConclusionsThis trial showed that the MDRP could not only effectively shorten the operation time, shorten the time from the bladder neck to the ureteral orifice of the ureteroscopic lithotripsy, but also place the patient's body in a functional position, stabilize the blood pressure during the operation, improve the comfort of the patient.Trial registrationChinese Clinical Trial Registry (No. ChiCTR2100053416).

Project description:BackgroundAfter first-time lateral patellar dislocation, 44% to 70% of patients sustain redislocations. Increased femoral anteversion (FA) is considered to result in increased lateralizing forces on the patella, which might predispose one to patellar instability. When recurrent patellar dislocations (RPDs) are bilateral, it is unclear if the FA is even more increased.HypothesisPatients with bilateral RPD would have greater FA than patients with unilateral RPD.Study designCase-control study; Level of evidence, 3.MethodsA total of 52 skeletally mature patients with RPD and a clinical suspicion of rotational malalignment underwent rotational computed tomography or magnetic resonance imaging. The uni- or bilaterality of the RPD was determined. A control group comprising 54 adult patients with trauma underwent computed tomography of both lower extremities as part of a multitrauma protocol. The FA values of both lower extremities were evaluated separately.ResultsIn total, 20 of 52 (38.5%) patients in the study group had a history of unilateral RPD and 32 of 52 (61.5%) patients had a bilateral RPD diagnosis. The mean FA of the asymptomatic limb in unilaterally symptomatic patients was 18.0° (SD, 11.2°; range, 0.5°-40.0°; median, 16.5°). In the symptomatic limb, the mean FA was 19.2° (SD, 9.1°; range, 2.0°-33.0°; median, 19.0°) (mean difference, 1.2°; 95% CI -1.3° to 3.8°). In bilaterally symptomatic patients, the mean FA on the right side was 23.2°, and 22.5° on the left. The mean differences between the symptomatic limbs in the unilateral RPD group and the right or left limbs in the bilateral RPD group were 4.0° (P = .263) and 3.3° (P = .326), respectively. In the control group without RPD, the mean FA was 12.5° (SD, 8.5°; range, 0.8°-33.0°; median, 10.9°). The mean difference between right limbs of the patients with bilateral RPD and right limbs of controls was 10.8° (P = .001).ConclusionPatients with bilateral RPD have bilaterally greater FA than patients without a history of RPD. Patients with unilateral RPD have greater FA on both sides compared with the control group without a history of lateral patellar dislocation. No statistically significant difference of FA can be seen between patients with bilateral or unilateral RPD.
