The effect of 17% EDTA and QMiX ultrasonic activation on smear layer removal and sealer penetration: ex vivo study.

ABSTRACT: This study aimed to compare the effect of conventional irrigation (CI) and passive ultrasonic irrigation (PUI) with 17% EDTA and QMiX on the maximum depth and percentage of sealer penetration into the dentinal tubules by confocal laser scanning microscopy (CLSM) and to describe the cleaning of root canal walls by scanning electron microscopy (SEM). Eighty single-rooted human mandibular premolars were instrumented and randomly assigned to four groups (n?=?20): EDTA?+?CI, QMiX + CI, EDTA?+?PUI, and QMiX + PUI. Ten samples from each group were examined by SEM (2,000×) and the remaining 40 roots were filled with a single gutta-percha cone and AH Plus sealer mixed with 0.1% rhodamine B for analysis by CLSM (10×). Images were assessed at distances of 2?mm (apical), 5?mm (middle), and 8?mm (coronal) from the apex with the Leica Application Suite V4.10 software. The EDTA?+?PUI and QMiX + PUI protocols presented higher rates of debris/smear layer removal in the apical and middle thirds. The PUI was superior to CI in the maximum depth of sealer penetration at the middle third. The QMiX + PUI group had a higher percentage of sealer penetration at the apical third. The PUI and QMiX protocol improved debris/smear layer removal and tubular dentin sealer penetration.

Project description:IntroductionTo evaluate the bacterial biofilm, smear layer and debris removal efficacy of a hydro-dynamic cavitation system with physiological saline using a new ex vivo model.MethodsSeventy-five dentin discs were prepared from fifty-four extracted teeth. Seventy-five artificial root sockets were prepared. Sixty dentin discs were used to grow 3-week-old bacterial biofilms, while smear layer and debris were produced on fifteen dentin discs. These dentin discs were adhered to the middle third of the artificial root canals. The sixty ex vivo models with biofilm-covered dentin discs were divided into six groups: control, needle with physiological saline, ultrasonic with physiological saline, Odne™ Clean with physiological saline, needle with 3% NaOCl, and ultrasonic with 3% NaOCl. Biofilm removal efficacy was evaluated using confocal laser scanning microscopy. The fifteen ex vivo models with smear layer and debris-covered dentin discs were divided into three groups: control, Odne™ Clean with physiological saline, and 5% NaOCl followed by 17% EDTA. Smear layer and debris removal efficacy was evaluated using scanning electron microscopy. Statistical analysis was performed using one-way analysis of variance for comparisons involving more than two groups. Post-hoc pairwise comparisons were conducted using the Tukey test.ResultsOdneTMClean with physiological saline (98%) performed significantly better than needle irrigation (47%) or ultrasonic activation (54%) with physiological saline (P < 0.05). OdneTMClean with physiological saline removed biofilms as effectively as needle irrigation (97%) or ultrasonic activation (98%) with 3% NaOCl (P > 0.05). Additionally, 5% NaOCl followed by 17% EDTA (score: 1.33) removed the smear layer significantly better than OdneTMClean with physiological saline (score: 4.47) (P < 0.05). However, OdneTMClean with physiological saline (score: 1.27) removed debris as effectively as 5% NaOCl followed by 17% EDTA (score: 1.13) (P > 0.05).ConclusionsOdneTMClean with physiological saline can effectively remove bacterial biofilm and debris from the dentin surface but cannot effectively remove the smear layer. Utilizing OdneTMClean during the final irrigation may enhance root canal cleaning efficacy.

Project description:(1) Background: The ex vivo porcine ear model is often used for the determination of the dermal penetration efficacy of chemical compounds. This study investigated the influence of the post-slaughter storage time of porcine ears on the dermal penetration efficacy of chemical compounds. (2) Methods: Six different formulations (curcumin and different fluorescent dyes in different vehicles and/or nanocarriers) were tested on ears that were (i) freshly obtained, (ii) stored for 24 or 48 h at 4 °C after slaughter before use and (iii) freshly frozen and defrosted 12 h before use. (3) Results: Results showed that porcine ears undergo post-mortem changes. The changes can be linked to rigor mortis and all other well-described phenomena that occur with carcasses after slaughter. The post-mortem changes modify the skin properties of the ears and affect the penetration efficacy. The onset of rigor mortis causes a decrease in the water-holding capacity of the ears, which leads to reduced penetration of chemical compounds. The water-holding capacity increases once the rigor is released and results in an increased penetration efficacy for chemical compounds. Despite different absolute penetration values, no differences in the ranking of penetration efficacies between the different formulations were observed between the differently aged ears. (4) Conclusions: All different types of ears can be regarded to be suitable for dermal penetration testing of chemical compounds. The transepidermal water loss (TEWL) and/or skin hydration of the ears were not correlated with the ex vivo penetration efficacy because both an impaired skin barrier and rigor mortis cause elevated skin hydration and TEWL values but an opposite penetration efficacy. Other additional values (for example, pH and/or autofluorescence of the skin) should, therefore, be used to select suitable and non-suitable skin areas for ex vivo penetration testing. Finally, data from this study confirmed that smartFilms and nanostructured lipid carriers (NLC) represent superior formulation strategies for efficient dermal and transdermal delivery of curcumin.
