The effects of gastrodin injection on hypertension: A systematic review and meta-analysis.

ABSTRACT:

Background

Hypertension is a common chronic disease and poses a huge burden to health care systems. Recent studies have shown that gastrodin injection (GI) has a potential supplementary therapeutic effect on hypertension.

Objectives

To systematically assess the efficacy and safety of GI in treatment of hypertension.

Methods

Systematic search was conducted on 7 databases (PubMed, Cochrane Library, Embase, Wanfang database, China biomedical literature service system, VIP Chinese Sci-tech journal database and China national knowledge internet). The retrieval time was from the establishment of database to February 15, 2020. Two researchers independently selected literature, extracted data and evaluated the risk of bias in the study. The methodological quality was evaluated with Cochrane handbook. The meta-analysis was performed with Stata 14.0 software.

Results

Thirteen studies were included in this study involving 1525 patients. Compared with using conventional therapy alone, GI combined with conventional therapy can decrease systolic blood pressure (weighted mean difference [WMD] -6.67, 95% confidence interval [CI]: -10.30, -3.04. number of estimates [k] = 9, I = 89.3%), diastolic blood pressure (WMD -4.52, 95% CI: -7.79, -1.26. k = 9, I = 92.3%), and improve the clinical efficacy (relative risk [RR] 1.18, 95% CI: 1.10, 1.26. k = 6, I = 12.6%).

Conclusions

The current evidence showed that GI combined with conventional therapy can improve systolic blood pressure, diastolic blood pressure and clinical efficacy. GI can become a supplementary treatment for hypertension.

Project description:Background: Breviscapine (Dengzhanhua) injection has been wildly used in clinical treatment for cerebral infarction, cardiovascular disease, diabetic nephropathy, renal impairment of essential hypertension and stroke in China. Breviscapine injection and antihypertensive drugs combination therapy is supposed to be beneficial for hypertension-induced renal damage patients. Objectives: To evaluate the beneficial and adverse effects of breviscapine injection on hypertension in hypertension-induced renal damage patients, an extensive meta-analysis was performed. Methods: We searched PubMed, the Cochrane Library, Embase, CNKI, Sino Med, VIP, and Wanfang Data for relevant literature. The timeframe of retrieval was set from the founding date of each database to September 28, 2018. Results: Fourteen papers were included in this study. The quality of all the studies included was determined to be low. All studies were conducted with Chinese populations. Meta-analysis showed that, compared with single-use antihypertensive drugs, using breviscapine injection in combination with antihypertensive drugs to treat hypertension in hypertension-induced renal damage patients can reduce 24-h urinary total protein (24 h UTP) [WMD = -0.04, 95% CI (-0.05, -0.02), P ≤ 0.001], but does not lower systolic blood pressure (SBP) [WMD = -1.02, 95% CI (-2.88, 0.84), P = 0.281] or diastolic blood pressure (DBP) [WMD = -0.21, 95% CI (-1.71, 1.29), P = 0.786] more effectively. There was also no statistically significant difference in adverse events between experimental groups and control groups. Conclusion: Breviscapine injection, in combination with antihypertensive drugs, appears to be more effective in improving the 24 h UTP, but overall have no effect on improving the blood pressure in hypertension-induced renal damage patients. Moderate dose of breviscapine injection (10 ml) may have effects on reducing blood pressure in hypertension-induced renal damage patients but high doses of breviscapine injection (20 ml) may increase blood pressure by subgroup analysis. However, the evidence of methodological quality and sample sizes is weak, and thus, further standardized research is required.

Project description:Transforaminal epidural injection of steroids is used to treat lumbar radicular pain. However, there are only a few well-designed randomized, controlled studies on the effectiveness of steroid injection.Hence, this study aims to assess the effectiveness of steroid injection to treat lumbar radicular pain using a meta-analysis of transforaminal epidural injection therapy for low back and lumbar radicular pain. The comparison was based on the mean difference in the Visual Analogue Score (VAS) and Oswestry Disability Index (ODI) from baseline to the specified followed up.The available literature of lumbar transforaminal epidural injections in managing low back and radicular pain was reviewed. Data sources included relevant literature of the English language identified through searches of PubMed and EMBASE from 1966 to 2009, and manual searches of the bibliographies of known primary and review articles. Finally, the search included the Current Controlled Trials Register and the Cochrane Database of Controlled Trials.The initial search identified 126 papers. After screening, five randomised controlled trials (RCTs) were studied for analysis and only three of these had followed-up patients systematically with pain and disability outcome scores to 3 months and of these, only one had follow up to 12 months. A total of 187 patients ('treatment group' receiving local anaesthetic/steroid injection) were compared with 181 patients ('control' group, receiving local anaesthetic only or saline injection). Improvement in pain (standardised mean difference in VAS 0.2 in favour of 'treatment'; 95%CI: -0.41 to 0.00, p = 0.05, I squared 0%) but not disability (standardised mean difference in ODI 0; 95%CI: -0.21 to 0.20, p = 0.99, I squared 0%) was observed between 'treatment' and 'control' groups; these differences were not significant. Additionally, the one study following patients to 12 months did not find any significant difference in VAS and ODI between treatment and control groups.The current meta-analysis shows that transforaminal epidural steroid injections, when appropriately performed, should result in an improvement in pain, but not disability. The three RCTs that followed patients to 3 months (and the single study to 12 months) have found no benefit by the addition of steroids. The limitations of this study include the paucity of the available literature.
