Measuring Spinal Mobility Using an Inertial Measurement Unit System: A Validation Study in Axial Spondyloarthritis.

ABSTRACT: Portable inertial measurement units (IMUs) are beginning to be used in human motion analysis. These devices can be useful for the evaluation of spinal mobility in individuals with axial spondyloarthritis (axSpA). The objectives of this study were to assess (a) concurrent criterion validity in individuals with axSpA by comparing spinal mobility measured by an IMU sensor-based system vs. optical motion capture as the reference standard; (b) discriminant validity comparing mobility with healthy volunteers; (c) construct validity by comparing mobility results with relevant outcome measures. A total of 70 participants with axSpA and 20 healthy controls were included. Individuals with axSpA completed function and activity questionnaires, and their mobility was measured using conventional metrology for axSpA, an optical motion capture system, and an IMU sensor-based system. The UCOASMI, a metrology index based on measures obtained by motion capture, and the IUCOASMI, the same index using IMU measures, were also calculated. Descriptive and inferential analyses were conducted to show the relationships between outcome measures. There was excellent agreement (ICC > 0.90) between both systems and a significant correlation between the IUCOASMI and conventional metrology (r = 0.91), activity (r = 0.40), function (r = 0.62), quality of life (r = 0.55) and structural change (r = 0.76). This study demonstrates the validity of an IMU system to evaluate spinal mobility in axSpA. These systems are more feasible than optical motion capture systems, and they could be useful in clinical practice.

Project description:IntroductionThe use of portable gait measurement systems in research is appealing to collect real-world data at low-cost, low participant burden, and without requirement for dedicated lab space. Most commercially available inertial measurement units (IMU's) designed for running only capture temporospatial data, the ability to capture biomechanics data such as shock and motion metrics with the RunScribe IMU makes it the closest to a lab alternative. The RunScribe system has been validated in running, however, is yet to be validated for walking.MethodQualisys motion capture, AMTI force plates, and Delsys Trigno accelerometers were used as gold standard lab measures for comparison against the RunScribe IMU. Twenty participants completed 10 footsteps per foot (20 total) measured by both systems simultaneously. Variables for validation included: Vertical Ground reaction force (GRF), instantaneous GRF rate, pronation excursion, pronation velocity, total shock, impact force, braking force. Interclass correlation (ICC) was used to determine agreement between the measurement systems, mean differences were used to evaluate group level accuracy.ResultsICC results showed moderate agreement between measurement systems when both limbs were averaged. The greatest agreement was seen for GRF rate, pronation excursion, and pronation velocity (ICC = 0.627, 0.616, 0.539), low agreement was seen for GRF, total shock, impact shock, braking shock (ICC = 0.269, 0.351, 0.244, 0.180). However mean differences show the greatest level of accuracy for GRF, GRF rate, and impact shock.DiscussionResults show mixed agreement between the RunScribe and gold standard lab measures, and varied agreement across left and right limbs. Kinematic variables showed the greatest agreement, however GRF had the lowest relative mean difference for group results. The results show acceptable levels of agreement for most variables, however further work must be done to assess the repeatability and sensitivity of the RunScribe to be applied within areas such as footwear testing and gait retraining protocols.

Project description:BackgroundAxial spondyloarthritis (axSpA) can lead to spinal mobility restrictions associated with restricted lower limb ranges of motion, thoracic kyphosis, spinopelvic ankylosis, or decrease in muscle strength. It is well known that these factors can have consequences on spatiotemporal gait parameters during walking. However, no study has assessed spatiotemporal gait parameters in patients with axSpA. Divergent results have been obtained in the studies assessing spatiotemporal gait parameters in ankylosing spondylitis, a subgroup of axSpA, which could be partly explained by self-reported pain intensity scores at time of assessment. Inertial measurement units (IMUs) are increasingly popular and may facilitate gait assessment in clinical practice.ObjectiveThis study compared spatiotemporal gait parameters assessed with foot-worn IMUs in patients with axSpA and matched healthy individuals without and with pain intensity score as a covariate.MethodsA total of 30 patients with axSpA and 30 age- and sex-matched healthy controls performed a 10-m walk test at comfortable speed. Various spatiotemporal gait parameters were computed from foot-worn inertial sensors including gait speed in ms-1 (mean walking velocity), cadence in steps/minute (number of steps in a minute), stride length in m (distance between 2 consecutive footprints of the same foot on the ground), swing time in percentage (portion of the cycle during which the foot is in the air), stance time in percentage (portion of the cycle during which part of the foot touches the ground), and double support time in percentage (portion of the cycle where both feet touch the ground).ResultsAge, height, and weight were not significantly different between groups. Self-reported pain intensity was significantly higher in patients with axSpA than healthy controls (P<.001). Independent sample t tests indicated that patients with axSpA presented lower gait speed (P<.001) and cadence (P=.004), shorter stride length (P<.001) and swing time (P<.001), and longer double support time (P<.001) and stance time (P<.001) than healthy controls. When using pain intensity as a covariate, spatiotemporal gait parameters were still significant with patients with axSpA exhibiting lower gait speed (P<.001), shorter stride length (P=.001) and swing time (P<.001), and longer double support time (P<.001) and stance time (P<.001) than matched healthy controls. Interestingly, there were no longer statistically significant between-group differences observed for the cadence (P=.17).ConclusionsGait was significantly altered in patients with axSpA with reduced speed, cadence, stride length, and swing time and increased double support and stance time. Taken together, these changes in spatiotemporal gait parameters could be interpreted as the adoption of a so-called cautious gait pattern in patients with axSpA. Among factors that may influence gait in patients with axSpA, patient self-reported pain intensity could play a role.
