Panic and generalized anxiety during the COVID-19 pandemic among Bangladeshi people: An online pilot survey early in the outbreak.

ABSTRACT:

Background

Precisely how the COVID-19 pandemic has impacted mental health worldwide is currently poorly understood. The study aimed to assess panic and anxiety among individuals in the general Bangladesh population early in the COVID-19 outbreak.

Methodology

A cross-sectional online survey was conducted from March 29 to April 06, 2020, involving 1311 community-dwelling individuals aged between 13 and 63 years and residing in Bangladesh. After providing informed consent, participants completed an online survey assessing socio-demographic variables and using the Panic Disorder Severity Scale and Generalized Anxiety Disorder (GAD-7) to assess panic and anxiety symptomatology, respectively. Binary logistic regression analyses were conducted.

Results

Estimates of panic and generalized anxiety were 79.6%, and 37.3%, respectively. Factors statistically predicting panic were being older (more than 30 years), having higher education (above bachelor), being married, and living with a joint family. Factors statistically predicting generalized anxiety were being female, being older (more than 30 years), having higher education (above bachelor), being married, being a non-governmental employee.

Limitations

As this study employs the cross-sectional and self-reported measures, causal inferences cannot be indicated. Sampling biases may have influenced estimates of panic and generalized anxiety.

Conclusion

Sizable proportions of respondents reported panic and generalized anxiety in the setting of COVID-19. The findings suggest the need for additional surveillance of panic and generalized anxiety through longitudinal assessments. Evidence-based intervention programs and supportive services to address panic and generalized anxiety appear important for Bangladeshi individuals during this stage (and likely later stages) of the COVID-19 pandemic.

Project description:Background: Generalized anxiety disorder (GAD) and panic disorder (PD) are the two severe subtypes of anxiety disorders (ADs), which are similar in clinical manifestation, pathogenesis, and treatment. Earlier studies have taken a whole-brain perspective on GAD and PD in the assumption that intrinsic fluctuations are static throughout the entire scan. However, it has recently been suggested that the dynamic alternations in functional connectivity (FC) may reflect the changes in macroscopic neural activity patterns underlying the critical aspects of cognition and behavior, and thus may act as biomarkers of disease. Methods: In this study, the resting-state functional MRI (fMRI) data were collected from 26 patients with GAD, 22 patients with PD, and 26 healthy controls (HCs). We investigated dynamic functional connectivity (DFC) by using the group spatial independent component analysis, a sliding window approach, and the k-means clustering methods. For group comparisons, the temporal properties of DFC states were analyzed statistically. Results: The dynamic analysis demonstrated two discrete connectivity "States" across the entire group, namely, a more segregated State I and a strongly integrated State II. Compared with HCs, patients with both GAD and PD spent more time in the weakly within-network State I, while performing fewer transitions and dwelling shorter in the integrated State II. Additionally, the analysis of DFC strength showed that connections associated with ADs were identified including the regions that belonged to default mode (DM), executive control (EC), and salience (SA) networks, especially the connections between SA and DM networks. However, no significant difference was found between the GAD and PD groups in temporal features and connection strength. Conclusions: More common but less specific alterations were detected in the GAD and PD groups, which implied that they might have similar state-dependent neurophysiological mechanisms and, in addition, could hopefully help us better understand their abnormal affective and cognitive performances in the clinic.

Project description:BackgroundDuring infectious disease outbreaks, healthcare workers are at high risk of infection, infecting others, and psychological distress. This study aimed to determine the prevalence of stress and anxiety in physicians during the COVID-19 outbreak in the Iraqi Kurdistan Region and assess their associated factors.MethodsThis cross-sectional study was carried out in Erbil, Iraqi Kurdistan Region, from March 28 to April 15, 2020. An online self-administered survey questionnaire was used to collect data from physicians working in specialized COVID-19 centers and other healthcare facilities. The level of stress was measured based on the 10-items Perceived Stress Scale. The level of anxiety was measured based on the 7-item Generalized Anxiety Disorder scale.ResultsA total of 370 participants responded to the perceived stress component of the survey, of whom 57 (15.4%) had low perceived stress, 249 (67.3%) had moderate stress, and 64 (17.3%) had high stress. Being female was significantly associated with having moderate/high stress (adjusted odds ratio (AOR) = 2.40 (95% CI 1.31-4.39)). A total of 201 participants responded to the generalized anxiety disorder component of the survey, of whom 19 (9.5%) had no anxiety, 57 (28.4%) had mild anxiety, 79 (39.3%) had moderate anxiety, and 46 (22.9%) had severe anxiety. Working in COVID-19 centers (AOR = 2.23 (95% CI 1.02-4.86)) and being general practitioners (AOR = 4.16 (95% CI 1.14-15.14)) were significantly associated with having moderate/severe anxiety.ConclusionA considerable proportion of physicians experience stress and anxiety during the COVID-19 outbreak in Iraqi Kurdistan region. Generalists and those in special COVID-19 units report the greatest anxiety. There a need to establish mechanisms to reduce the risks of stress and anxiety among physicians. Mental health coping interventions through counseling should be based on COVID-19 protocol guidelines. Interventions should also emphasize physicians' ability to work safely and efficiently in providing care to the patients.
