The Mediating Effect of Unhealthy Behaviors and Body Mass Index in the Relation Between High Physical Workload and Self-Rated Poor Health in Male Construction Workers.

ABSTRACT:

Objectives

To examine the mediating role of unhealthy behaviors and body mass index (BMI) in the relation between high physical workload and self-rated health in male construction workers.

Methods

Longitudinal data over 2010 to 2018 were used of 30,224 male construction workers in The Netherlands. Smoking, lack of physical activity, and alcohol consumption were self-reported. BMI was based on measured body weight and height. Multilevel modeling path analyses were used to determine the mediating role of unhealthy behaviors in the relation between physical workload and poor health.

Results

A direct effect of high physical workload on poor health adjusted for unhealthy behaviors and BMI was found (odds ratio [OR] 1.49, 95% confidence interval [CI] 1.38 to 1.68). Indirect effects of the unhealthy behaviors and BMI in the relation between high physical workload and poor health were small (OR varying from 0.96 to 1.04).

Conclusion

The pathway of high physical workload and poor health through unhealthy behaviors and BMI was not supported.

Project description:BackgroundNight shift work and poor sleep quality are gradually becoming more prevalent in modern society. Nevertheless, there have been limited studies assessing the association between night shift work, sleep behaviors, and risk of epilepsy. The aim of our study was to ascertain whether a positive association exists between night shift work, sleep quality, sleep behaviors, and risk of epilepsy.MethodsOur study included a total of over 270,000 individuals with or without epilepsy from the UK Biobank, followed up over a period of 13.5 years. Information on current night shift work and major sleep behaviors was also obtained. We used Cox proportional hazard models to assess the association between night shift work, sleep quality, sleep behaviors, and the risk of epilepsy after adjusting for multiple variables.ResultsNight shift work was positively associated with a higher risk of epilepsy (P for trend = 0.059). There was a gradual increase in epilepsy risk from 'never/rarely' to 'usual/permanent' night shifts, with 'usual/permanent' night shifts work presenting the highest risk [hazard ratio (HR) 1.29, 95% confidence interval (CI) 1.01-1.65). Additionally, there was a significant association between sleep quality and risk of epilepsy (P < 0.001). Among the five major sleep behaviors, sleep duration (< 7 or > 8 h/day), frequent insomnia, and daytime sleepiness were significantly associated with a higher risk of epilepsy (HR 1.19, 95% CI 1.11-1.28; HR 1.19, 95% CI 1.09-1.30; HR 1.46, 95% CI 1.24-1.72, respectively). Furthermore, sleep duration exhibited a 'U-shaped' association with epilepsy risk. Nevertheless, no significant association was found between sleep chronotype and snoring and the risk of incident epilepsy (HR 1.04, 95% CI 0.96-1.12; HR 0.96, 95% CI 0.89-1.04).Conclusions'Usual/permanent' night shifts and poor sleep quality were positively associated with a greater risk of incident epilepsy. Major sleep behaviors, including unhealthy sleep duration (< 7 or > 8 h/day), frequent insomnia, and daytime sleepiness, also tended to increase the risk of epilepsy.

Project description:Lifestyle behaviors significantly impact health, yet remain poorly defined in Chinese rural-to-urban migrants.In a cross-sectional study of health-related behaviors of 5484 rural-to-urban migrants who had worked in Shanghai for at least six months, we assessed the contribution of demographics and physical and mental health to lifestyle behaviors in male and female participants by multiple stepwise cumulative odds logistic regression.Respondents were 51.3% male. 9.9% exhibited abnormal blood pressure; 27.0% were overweight or obese; 11.2% reported abnormal mental health; 36.9% reported healthy lifestyle. Multiple stepwise cumulative odds logistic regression indicated that men working in manufacturing reported less unhealthy lifestyle than those in hospitality (cumulative odds ratio (COR) = 1.806, 95%CI 1.275-2.559) or recreation/leisure (COR = 3.248, 95%CI 2.379-4.435); and women working in manufacturing and construction reported less unhealthy lifestyle than those in all other sectors. Unhealthy lifestyle was associated with small workplaces for men (COR = 1.422, 95%CI 1.154-1.752), working more than 8 or 11 hours per day for women and men, respectively, and earning over 3500 RMB in women (COR = 1.618, 95%CI 1.137-2.303). Single women and women who had previously resided in three or more cities were more likely to report unhealthy lifestyle (COR = 2.023, 95%CI 1.664-2.461, and COR = 1.311, 95%CI 1.072-1.602, respectively). Abnormal mental status was also correlated with unhealthy lifestyle in men (COR = 3.105, 95%CI 2.454-3.930) and women (COR = 2.566, 95%CI 2.024-3.252).There were different risk factors of unhealthy lifestyle score in male and female rural-to-urban migrants, especially in number of cities experienced, salary, marital status, work place scale. Several demographic groups: employment sectors (e.g. hospitality and recreation/leisure), working conditions (e.g. long hours) and abnormal mental status were associated with unhealthy lifestyle behaviors in Chinese rural-to-urban migrants, and health interventions should be targeted to these groups.
