Uniportal Endoscopic Lumbar Interbody Fusion.

ABSTRACT: The cause of radiculopathy is the compression of the nerve root which can be secondary to sliding of the vertebra and reduced disc height. In some patients, decompression alone does not resolve this problem. We describe the uniportal endoscopic transforaminal lumbar interbody fusion technique. Full-endocopic foraminotomy and discectomy are followed by cage implementation and percutaneous instrumentation. The goal of this surgical method is decompression of nerve roots, segment stabilization, disc height, and sagittal alignment restoration. Uniportal endoscopic facet sparing transforaminal transkambin lumbar interbody fusion is a good surgical option to treat degenerative disc disease, mechanical instability, and spondylolisthesis. This method shows favourable clinical outcomes in selected patients.

Project description:This meta-analysis aims to determine the clinical outcomes, complications, and fusion rates in endoscopic assisted intra-foraminal lumbar interbody fusion (iLIF) and minimally invasive transforaminal lumbar interbody fusion (MI-TLIF) for lumbar degenerative diseases. The MEDLINE, Embase, and Cochrane Library databases were searched. The inclusion criteria were: five or more consecutive patients who underwent iLIF or MI-TLIF for lumbar degenerative diseases; description of the surgical technique; clinical outcome measures, complications and imaging assessment; minimum follow-up of 12 months. Surgical time, blood loss, and length of hospital stay were extracted. Mean outcome improvements were pooled and compared with minimal clinically important differences (MCID). Pooled and direct meta-analysis were evaluated. We identified 42 eligible studies. The iLIF group had significantly lower mean intra-operative blood loss, unstandardized mean difference (UMD) 110.61 mL (95%CI 70.43; 150.80; p value < 0.0001), and significantly decreased length of hospital stay (UMD 2.36; 95%CI 1.77; 2.94; p value < 0.0001). Visual analogue scale (VAS) back, VAS leg and Oswestry disability index (ODI) baseline to last follow-up mean improvements were statistically significant (p value < 0.0001), and clinically important for both groups (MCID VAS back > 1.16; MCID VAS leg > 1.36; MCID > 12.40). There was no significant difference in complication nor fusion rates between both cohorts. Interbody fusion using either iLIF or MI-TLIF leads to significant and clinically important improvements in clinical outcomes for lumbar degenerative diseases. Both procedures provide high rates of fusion at 12 months or later, without significant difference in complication rates. iLIF is associated with significantly less intraoperative blood loss and length of hospital stay. Project description:ObjectiveThis study aims to perform a meta-analysis that integrates multiple literature sources to evaluate the clinical efficacy of oblique lumbar interbody fusion (OLIF) versus minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) for treating lumbar degenerative diseases (LDD).MethodsA systematic search was conducted across various databases, including CNKI, VIP, WANFANG DATA, SinoMed, PubMed, Cochrane Library, Embase, and Web of Science, for clinical comparative studies on OLIF and MIS-TLIF for treating LDD, covering the time frame from the inception of the databases to September 2024. Following PRISMA guidelines, studies were screened, assessed, and data were extracted rigorously. Indicators extracted included operative time, intraoperative blood loss, hospital stay, visual analog scale (VAS) scores, Oswestry Disability Index (ODI), disc height (DH), segmental lordotic angle (SLA), lumbar lordosis angle (LLA), postoperative JOA scores, patient satisfaction, complication rates, and fusion rates. Meta-analysis was performed using Review Manager 5.4 software.ResultsA total of 24 studies were included, comprising 11 randomized controlled trials and 13 retrospective cohort studies. The total population consisted of 1785 patients, with 898 in the OLIF group and 887 in the MIS-TLIF group. The meta-analysis indicated that, compared to the MIS-TLIF group, the OLIF group exhibited significantly lower intraoperative blood loss, shorter hospital stays, improved postoperative DH, shorter operative time, reduced postoperative VAS scores, lower postoperative ODI scores, and improved postoperative SLA and LLA. No significant differences were observed between the groups in postoperative JOA scores, fusion rates, complication rates, or patient satisfaction. The OLIF group exhibited advantages such as lower blood loss, shorter hospital stays, higher postoperative fusion rates, and improved recovery of disc and foraminal heights.ConclusionCompared to MIS-TLIF, OLIF is associated with a shorter operative time and less intraoperative blood loss, potentially leading to better relief of leg pain, restoration of DH, and prevention of subsidence. These findings offer valuable insights for clinical practice.
