Gene-Diet Interactions in Colorectal Cancer: Survey Design, Instruments, Participants and Descriptive Data of a Case-Control Study in the Basque Country.

ABSTRACT: Epidemiologic studies have revealed inconsistent evidence of gene-diet interaction in relation to colorectal cancer (CRC). The aim of this study was to analyze them in a sample of cases and controls from the population-based bowel cancer screening program of the Osakidetza/Basque Health Service. This study analyzed dietetic, genetic, demographic, socioeconomic factors and lifestyles. In the present manuscript, the survey design, sampling, instruments, measurements and related quality management were presented. Moreover, we analyze differences between cases and controls in some data, especially those related to diet. The participants were 308 cases and 308 age- and sex-matched subjects as controls. Cases were more likely than controls to have overweight/obesity (67.5% vs. 58.1%, p < 0.05), a lower intake of vitamin B2 (0.86 ± 0.23 vs. 0.92 ± 0.23 mg/1000 kcal, p < 0.01) and calcium:phosphorus ratio (0.62 ± 0.12 vs. 0.65 ± 0.13, p < 0.01). A higher proportion of cases than controls did not meet the Nutritional Objectives for saturated fatty acids (85.7% vs. 67.5%, p < 0.001) or cholesterol (35.4% vs. 25.0%, p < 0.01). In conclusion, the present study provides valuable data for analyzing the complexity of gene-diet interaction in relation to CRC. The results presented here suggest that overweight/obesity and a high intake of certain dietary components, especially saturated fatty acids and cholesterol, are more frequent in cases than in controls.

Project description:BackgroundMosquito-borne diseases cause millions of deaths each year and are increasingly spreading from tropical and subtropical regions into temperate zones, posing significant public health risks. In the Basque Country region of Spain, changing climatic conditions have driven the spread of invasive mosquitoes, increasing the potential for local transmission of diseases such as dengue, Zika, and chikungunya. The establishment of mosquito species in new areas, coupled with rising mosquito populations and viremic imported cases, presents challenges for public health systems in non-endemic regions.MethodsThis study uses models that capture the complexities of the mosquito life cycle, driven by interactions with weather variables, including temperature, precipitation, and humidity. Leveraging machine learning techniques, we aimed to forecast Aedes invasive mosquito abundance in the provinces of the Basque Country, using egg count as a proxy and weather features as key independent variables. A Spearman correlation was used to assess relationships between climate variables and mosquito egg counts, as well as their lagged time series versions. Forecasting models, including random forest (RF) and seasonal autoregressive integrated moving average (SARIMAX), were evaluated using root mean squared error (RMSE) and mean absolute error (MAE) metrics.ResultsStatistical analysis revealed significant impacts of temperature, precipitation, and humidity on mosquito egg abundance. The random forest (RF) model demonstrated the highest forecasting accuracy, followed by the SARIMAX model. Incorporating lagged climate variables and ovitrap egg counts into the models improved predictions, enabling more accurate forecasts of Aedes invasive mosquito abundance.ConclusionsThe findings emphasize the importance of integrating climate-driven forecasting tools to predict the abundance of mosquitoes where data are available. Furthermore, this study highlights the critical need for ongoing entomological surveillance to enhance mosquito spread forecasting and contribute to the development and assessment of effective vector control strategies in regions of mosquito expansion.

Project description:BackgroundMultimorbidity is clearly a major challenge for healthcare systems. However, currently, its magnitude and impact on healthcare expenditures is still not well known. The objective of this paper is to present an overview of the prevalence of multimorbidity by deprivation level in the elderly population of the Basque Country.MethodsWe conducted a cross-sectional analysis that included all the inhabitants of the Basque Country aged 65 years and over (N = 452,698). This was based on data from primary care electronic medical records, hospital admissions, and outpatient care databases, for a 4-year period. The health problems of the patients were identified from their diagnoses and prescriptions. Multimorbidity was defined as the presence of two or more chronic diseases out of a list of 47 of the most important and common chronic conditions consistent with the literature. In addition, we explored socio-economic and demographic variables such as age, sex, and deprivation level.ResultsMultimorbidity was found in 66.13% of the population aged 65 and over and increases with age until 80 years. The prevalence of multimorbidity was higher in deprived (69.94%) than better-off (60.22%) areas. This pattern of differences between the most and least disadvantaged areas was observed in all age groups and more marked in female (70.96-59.78%) than in male (68.54-60.86%) populations. In almost all diseases studied (43 out of 47), 90% of patients had been diagnosed with at least one other illness. It was also frequent the coexistence of mental and physical health problems in the same person and the presence of multiple physical diseases is higher in patients with mental disease than in the rest of population (74.97% vs. 58.14%).ConclusionMultimorbidity is very common among people over 65 years old in the Basque Country, particularly in unfavourable socioeconomic environments. Given the ageing population, multimorbidity and its consequences should be taken into account in healthcare policy, organization of care and medical research. Administrative health databases are readily available sources of a range of information that can be useful for such purposes.
