Project description:Sarcoidosis is a systemic, chronic, inflammatory disease characterized by noncaseating granuloma formations. The fact that the etiopathogenesis of the disease has not been elucidated yet brings it many theories and assumptions. Being a systemic disease and ability to involve many organs and systems, it attracts the attention of physicians from different branches. In addition to lung involvement, skin, eye, heart, and locomotor system involvement is an important clinical finding. Sarcoidosis may present with very different clinical presentations, and therefore, it is one of the important "imitators" in the medical literature. I like sarcoidosis as a "rainbow," it is a disease that contains the characteristics of many diseases. Different clinical, radiological, and laboratory prognostic factors (lupus pernio, chronic uveitis, late-onset disease, chronic hypercalcemia, nephrocalcinosis, Afro-American race, progressive pulmonary sarcoidosis, radiologic Stage 4, bone involvement, neurosarcoidosis, cardiac involvement, and chronic respiratory failure) have been defined in this "rainbow." Early identification of these factors plays an important role in the determination of treatment strategies, morbidity, and mortality of the disease. In this article, clinical, genetic, laboratory, and radiological factors that determine the prognosis of sarcoidosis are discussed in light of the latest data in the literature.

Project description:The failure of international efforts to manage commercial whaling was exemplified by revelations of large-scale illegal whale catches by the USSR over a 30 year period following World War II. Falsifications of catch data have also been reported for Japanese coastal whaling, but to date there has been no investigation of the reliability of catch statistics for Japanese pelagic (factory fleet) whaling operations. Here, we use data of known reliability from Soviet whaling industry reports to show that body lengths reported to the International Whaling Commission (IWC) by Japanese factory fleets for female sperm whales caught in the North Pacific are not credible. In 1968/1969, Japanese whaling fleets in the North Pacific killed 1568 females, of which 1525 (97.3%) were reported as being at or above the IWC's minimum length of 11.6 m (legal-sized females, LSFs). By contrast, Soviet fleets operating during this period killed 12 578 females; only 824 (6.6%) were LSFs. Adjusting for effort, catches of LSFs were up to 9.1 times higher for Japan compared with the USSR, and even higher for very large females. Dramatic differences in body length statistics were evident when both nations operated in the same area. Significantly, the frequency of LSFs and very large females in the Japanese catch markedly declined after the IWC's International Observer Scheme in 1972 made illegal whaling more difficult. We conclude that the Japanese length data reflect systematic falsification of catch statistics submitted to the IWC, with serious implications for the reliability of data used in current population assessments. The apparent ease with which catch data were falsified in the past underscores the necessity of transparent and independent inspection procedures in any future commercial whaling.

Project description:Both costs and benefits must be considered when implementing marine protected areas (MPAs), particularly those associated with fishing effort displaced by potential closures. The Southern Ocean offers a case study in understanding such tradeoffs, where MPAs are actively being discussed to achieve a range of protection and sustainable use objectives. Here, we evaluated the possible impacts of two MPA scenarios on the Antarctic krill (Euphausia superba) fishery and krill-dependent predators in the Scotia Sea, explicitly addressing the displacement of fishing from closed areas. For both scenarios, we employed a minimally realistic, spatially explicit ecosystem model and considered three alternative redistributions of displaced fishing. We projected both MPAs to provide positive outcomes for many krill-dependent predators, especially when closed areas included at least 50-75% of their foraging distributions. Further, differences between the scenarios suggest ways to improve seal and penguin protection in the Scotia Sea. MPA scenarios also projected increases in total fishery yields, but alongside risks of fishing in areas where relatively low krill densities could cause the fishery to suspend operations. The three alternatives for redistributing displaced fishing had little effect on benefits to predators, but did matter for the fishery, with greater differences in overall catch and risk of fishing in areas of low krill density when displaced fishing was redistributed evenly among the open areas. Collectively, results suggest a well-designed MPA in the Scotia Sea may protect krill-dependent predators, even with displaced fishing, and preclude further spatial management of the krill fishery outside the MPA. More broadly, outcomes denote the importance of delineating fishing and predator habitat, spatial scales, and the critical trade-offs inherent in MPA development.

Project description:The majority of the 30-100 million people infected with Strongyloides stercoralis, a soil transmitted intestinal nematode, have subclinical (or asymptomatic) infections. These infections are commonly chronic and longstanding because of the autoinfective process associated with its unique life cycle. A change in immune status can increase parasite numbers, leading to hyperinfection syndrome, dissemination, and death if unrecognized. Corticosteroid use and HTLV-1 infection are most commonly associated with the hyperinfection syndrome. Strongyloides adult parasites reside in the small intestine and induce immune responses both local and systemic that remain poorly characterized. Definitive diagnosis of S. stercoralis infection is based on stool examinations for larvae, but newer diagnostics - including new immunoassays and molecular tests - will assume primacy in the next few years. Although good treatment options exist for infection and control of this infection might be possible, S. stercoralis remains largely neglected.

Project description:While the incidence of nephrotic syndrome (NS) is decreasing in Korea, the morbidity of difficult-to-treat NS is significant. Efforts to minimize treatment toxicity showed that prolonged treatment after an initial treatment for 2-3 months with glucocorticosteroids was not effective in reducing frequent relapses. For steroid-dependent NS, rituximab, a monoclonal antibody against the CD20 antigen on B cells, was proven to be as effective, and short-term daily low-dose steroids during upper respiratory infections reduced relapses. Steroid resistance or congenital NS are indications for genetic study and renal biopsy, since the list of genes involved in NS is lengthening.

Project description:BackgroundAbout 600 million people are estimated to be infected with Strongyloides stercoralis, the species that causes most of the human strongyloidiasis cases. S. stercoralis can also infect non-human primates (NHPs), dogs and cats, rendering these animals putative sources for zoonotic human S. stercoralis infection. S. fuelleborni is normally found in old world NHPs but occasionally also infects humans, mainly in Africa. Dogs in southeast Asia carry at least two types of Strongyloides, only one of which appears to be shared with humans ("dog only" and "human and dog" types). For S. stercoralis with molecular taxonomic information, there is a strong sampling bias towards southeast and east Asia and Australia.Methodology/principle findingsIn order to extend the geographic range of sampling, we collected human and dog derived Strongyloides spp. and hookworms from two locations in Bangladesh and subjected them to molecular taxonomic and genomic analysis based on nuclear and mitochondrial sequences. All hookworms found were Necator americanus. Contrary to earlier studies in Asia, we noticed a rather high incidence of S. fuelleborni in humans. Also in this study, we found the two types of S. stercoralis and no indication for genetic isolation from the southeast Asian populations. However, we found one genomically "dog only" type S. stercoralis in a human sample and we found two worms in a dog sample that had a nuclear genome of the "dog only" but a mitochondrial genome of the "human and dog" type.Conclusions/significanceS. fuelleborni may play a more prominent role as a human parasite in certain places in Asia than previously thought. The introgression of a mitochondria haplotype into the "dog only" population suggests that rare interbreeding between the two S. stercoralis types does occur and that exchange of genetic properties, for example a drug resistance, between the two types is conceivable.

Project description:Childhood pneumonia is still a significant clinical and public health problem. India contributes the highest number of deaths due to pneumonia, accounts for about 20% of global mortality among under five children. Various etiologic agents including bacteria, viruses and atypical organism are responsible for childhood pneumonia. Recent studies suggest that viruses are one of the major causes of childhood pneumonia. Among viruses, respiratory syncytial virus has got great attention and several recent studies are reporting it as an important organism for pneumonia. Lack of exclusive breast feeding during first six months, improper timing of start and content of complimentary feeding, anemia, undernutrition, indoor pollution due to tobacco smoking and use of coal and wood for cooking food and lack of vaccinations are important risk factors. X-ray chest is not routinely performed to diagnose pneumonia while use of lung ultrasound is increasing to detect consolidation, pleural effusion, pneumothorax and pulmonary edema (interstitial syndrome). Role of C-reactive protein (CRP) and procalcitonin is similar, to differentiate between viral and bacterial pneumonia, however duration of antibiotics is better guided by procalcitonin. Newer biomarkers like IL-6, presepsin and triggering receptor expressed on myeloid cells 1 are needed to be evaluated for their use in children. Hypoxia is significantly associated with childhood pneumonia. Therefore, use of pulse oximetry should be encouraged for early detection and prompt treatment of hypoxia to prevent adverse outcomes. Among the available tools for risk of mortality assessment in children due to pneumonia, PREPARE score is the best but external validation will be needed.

Project description:The accumulated evidence from two decades of randomized controlled trials has not yet resolved the question of how best to monitor colorectal cancer (CRC) survivors for early detection of recurrent and metachronous disease or even whether doing so has its intended effect. A new wave of trial data in the coming years and an evolving knowledge of relevant biomarkers may bring us closer to understanding what surveillance strategies are most effective for a given subset of patients. To best apply these insights, a number of important research questions need to be addressed, and new decision making tools must be developed. In this review, we summarize available randomized controlled trial evidence comparing alternative surveillance testing strategies, describe ongoing trials in the area, and compare professional society recommendations for surveillance. In addition, we discuss innovations relevant to CRC surveillance and outline a research agenda which will inform a more risk-stratified and personalized approach to follow-up.
