A preliminary study on the visual outcomes after LaserACE for presbyopia.

ABSTRACT:

Background

To investigate the visual outcomes over 1 year after laser anterior ciliary excision (LaserACE) procedure for presbyopic subjects.

Methods

A prospective, non-randomized, non-comparative study. Eight emmetropic eyes of four patients with presbyopia were included in this study. All eyes were treated with the LaserACE procedure using the VisioLite erbium-YAG laser (Ace Vision Group, USA). Subjects were assessed preoperatively and at 1 day, 1 week, 1 month, 3 months, 6 months, and 1 year postoperatively. Outcome measures included questionnaires, visual acuity, manifest refraction, pupil size, keratometry, reading prescription, and intraocular pressure.

Results

Binocular distance corrected intermediate visual acuity (DCIVA) had improved from 0.33±0.12 logMAR to 0.12±0.12 logMAR after 6 months (P<0.05) and 0.13±0.12 logMAR after 12 months postoperatively (P<0.01). Binocular distance corrected near visual acuity (DCNVA) improved from 0.41±0.10 logMAR to 0.24±0.12 logMAR after 6 months (P<0.05) and 0.26±0.09 log MAR after 12 months postoperatively (P<0.05). An average decrease of 0.91±0.28 D in the reading prescription at a 40 cm reading distance was observed and remained stable over 12 months (P<0.05). There were no significant differences in manifest refraction, uncorrected distance visual acuity (UDVA), corneal curvature, high order aberrations, pupil size compared with preoperative and 6 months as well as 12 months postoperative visits (P>0.05).

Conclusions

The LaserACE procedure is shown to be safe and efficient for presbyopia correction without influencing distance vision or visual quality. However, its mechanism and long-term effects need further validation.

Project description:IntroductionRefractive lens exchange (RLE) patients represent a unique population with a different baseline and set of expectations compared to their cataract counterparts. Visual outcomes and patient satisfaction following RLE with bilateral implantation of a trifocal intraocular lens (IOL) were assessed in a cohort of patients with presbyopia and without cataract.MethodsCharts of patients with presbyopia who underwent femtosecond laser-assisted RLE with bilateral implantation of the PanOptix trifocal IOL (toric and non-toric) at a single-surgeon, private practice center from September 2019 to August 2020 were retrospectively reviewed. Eyes with prior keratorefractive surgery, amblyopia, or underlying pathology were excluded. Primary endpoints included corrected and uncorrected visual acuity at distance (CDVA and UDVA), intermediate (DCIVA and UIVA), and near (DCNVA and UNVA) at 6 months. Secondary endpoints included residual refractive error, patient-reported satisfaction, spectacle independence, and visual disturbance profile as assessed by a validated questionnaire at 6 months.ResultsSeventy-eight eyes of 39 patients (mean age 56 ± 6 years; 79% female) were included. Most eyes were hyperopic (mean SE 2.35 ± 1.81). Postoperatively, 100% and 92% of eyes were within ± 1.00 D and ± 0.50 D of emmetropia, respectively. One hundred percent, 97%, and 97% of patients achieved UDVA, UNVA, and UIVA of logMAR 0.1 or better. Starbursts were the most frequent (67%) and bothersome (41%) visual disturbance cited, but nearly half (41%) of patients rated them as absent or "not bothersome at all." Overall, 77% of patients reported achieving complete spectacle independence with 87% and 90% of patients stating they were satisfied with their vision and would recommend the same procedure to others, respectively.ConclusionsRLE with bilateral implantation of the PanOptix IOL is a safe and effective procedure with good patient satisfaction. Because of their relatively clear native lenses and visually demanding needs compared to their cataract counterparts, surgeons should take extra precautions to counsel RLE patients on the limitations of trifocal technology.

Project description:Competition is a major determinant of plant community structure consisting of both species-specific and general interactions, either of which may influence competitive competency and plant abundance and size. In certain cases, competitive competency could arise from altered gene expression and plant function when an individual is confronted with new competitors. We explored competition at the molecular level by hybridizing transcripts from Centaurea maculosa (spotted knapweed), one of North America's most invasive exotic plant species, to an Arabidopsis microarray chip. Centaurea was grown in competition with Festuca idahoensis (Idaho fescue), a native grass species that generally has weak competitive effects against Centaurea; Gaillardia aristata (Indian blanketflower), a native herbaceous species that tends to be a much stronger competitor against Centaurea; or alone (control). The expression of some genes was found to be relatively uninfluenced by the type of plant neighbor, whereas other patterns of gene expression appeared to be more neighbor specific. To our knowledge, these results are the first to identify genes in an invasive plant that are induced or repressed by plant neighbors and provide a new avenue of insight into the molecular aspects of plant competitive ability. Keck Foundation Biotechnology Resource Laboratory, Yale University. The OAR27K gene chip consists of 70-mer oligos representing approximately 27,000 genes from Arabidopsis. Leaf and root samples were reverse-transcribed into cDNA and labeled with different fluorescent dyes (Cy3 and Cy5) using the Genisphere Array 900 kit (Cat No.W500180) (Genisphere, Hatfield PA). Labeled cDNAs were hybridized to the OAR27Kchip using the Advalytix ArrayBooster DNA Microarray Incubator (Advalytix, Boston MA). Approximately 10ug of each sample was used for hybridization.Experiment 2: Hybridization and labeling followed the procedure above, except the experimental control was Centaurea plants grown alone, and the test situations were Centaurea grown with Gaillardia or Festuca neighbors. In each experiment, RNA from Centaurea grown alone was used as the control (labeled with Cy3) and RNA from Centaurea grown with a neighbor was used as the test (labeled with Cy5). Two replicates were performed for each test situation.
