ABSTRACT: Background and purpose
The purpose of this study was to assess psychometric properties of the Perceptions of Brain Injury Survey (PBIS), an instrument designed to assess nurses' perceptions and preparation to care for patients with traumatic brain injury (TBI).Methods

Final instrument has 66 items and is composed of four subscales, which can be used independently. Results showed Cronbach's alpha for the overall scale was excellent (.93) and alphas for each composite subscale were acceptable to excellent (.73-.93). Findings also suggest good discriminant validity and evidence of external validity.Conclusions
The PBIS is a reliable and valid measure for assessing nurses' perceptions of caring for patients with TBI in practice or research.

Project description:ContextSelf-report questionnaires are an important method of evaluating lifespan health, exercise, and health-related quality of life (HRQL) outcomes among elite, competitive athletes. Few instruments, however, have undergone formal characterization of their psychometric properties within this population.ObjectiveTo evaluate the validity and reliability of a novel health and exercise questionnaire, the Trojan Lifetime Champions (TLC) Health Survey.DesignDescriptive laboratory study.SettingA large National Collegiate Athletic Association Division I university.Patients or other participantsA total of 63 university alumni (age range, 24 to 84 years), including former varsity collegiate athletes and a control group of nonathletes.Intervention(s)Participants completed the TLC Health Survey twice at a mean interval of 23 days with randomization to the paper or electronic version of the instrument.Main outcome measure(s)Content validity, feasibility of administration, test-retest reliability, parallel-form reliability between paper and electronic forms, and estimates of systematic and typical error versus differences of clinical interest were assessed across a broad range of health, exercise, and HRQL measures.ResultsCorrelation coefficients, including intraclass correlation coefficients (ICCs) for continuous variables and κ agreement statistics for ordinal variables, for test-retest reliability averaged 0.86, 0.90, 0.80, and 0.74 for HRQL, lifetime health, recent health, and exercise variables, respectively. Correlation coefficients, again ICCs and κ, for parallel-form reliability (ie, equivalence) between paper and electronic versions averaged 0.90, 0.85, 0.85, and 0.81 for HRQL, lifetime health, recent health, and exercise variables, respectively. Typical measurement error was less than the a priori thresholds of clinical interest, and we found minimal evidence of systematic test-retest error. We found strong evidence of content validity, convergent construct validity with the Short-Form 12 Version 2 HRQL instrument, and feasibility of administration in an elite, competitive athletic population.ConclusionsThese data suggest that the TLC Health Survey is a valid and reliable instrument for assessing lifetime and recent health, exercise, and HRQL, among elite competitive athletes. Generalizability of the instrument may be enhanced by additional, larger-scale studies in diverse populations.

Project description:Aims and objectivesThe purpose of this study was to determine nurses' perceptions about caring for patients with traumatic brain injury.BackgroundAnnually, it is estimated that over 10 million people sustain a traumatic brain injury around the world. Patients with traumatic brain injury and their families are often concerned with expectations about recovery and seek information from nurses. Nurses' perceptions of care might influence information provided to patients and families, particularly if inaccurate knowledge and perceptions are held. Thus, nurses must be knowledgeable about care of these patients.MethodsA cross-sectional survey, the Perceptions of Brain Injury Survey (PBIS), was completed electronically by 513 nurses between October and December 2014. Data were analysed with structural equation modelling, factor analysis, and pairwise comparisons.ResultsUsing latent class analysis, authors were able to divide nurses into three homogeneous sub-groups based on perceived knowledge: low, moderate and high. Findings showed that nurses who care for patients with traumatic brain injury the most have the highest perceived confidence but the lowest perceived knowledge. Nurses also had significant variations in training.ConclusionsAs there is limited literature on nurses' perceptions of caring for patients with traumatic brain injury, these findings have implications for training and educating nurses, including direction for development of nursing educational interventions.Relevance to clinical practiceAs the incidence of traumatic brain injury is growing, it is imperative that nurses be knowledgeable about care of patients with these injuries. The traumatic brain injury PBIS can be used to determine inaccurate perceptions about caring for patients with traumatic brain injury before educating and training nurses.

Project description:Recent studies have reported that various brain regions, mainly sensory, unimodal regions, display phase synchronization with the stomach's slow (0.05 Hz) myoelectrical rhythm. These gastric-brain interactions have broad implications, from feeding behavior to functional gastrointestinal disorders. However, in contrast to other interoceptive signals (e.g., heart rate) and their relation to the brain, little is known about the reliability of these gastric-brain interactions, their robustness to artifacts such as motion, and whether they can be generalized to new samples. Here we examined these aspects in 43 subjects that had undergone multiple runs of concurrent electrogastrography (EGG), brain fMRI, and pulse oximetry. We also repeated all analyses in an open dataset of a highly sampled individual. We found a set of brain regions that were coupled with the EGG signal after controlling for non-grey matter (GM) signals, head motion, and cardiac artifacts. These regions exhibited significant overlap with previous work. However, we also showed that prior to confound regression, the spatial extent of the gastric network was largely overestimated. Finally, we found substantial test-retest reliability in both the brain and the gastric signals when estimated alone, but not for measures of gastric-brain synchrony. Together, these results provide methodological scaffolding for future research into brain-stomach interactions and for a better understanding of the role of the gastric network.

Project description:BackgroundThe Child Perceptions Questionnaires (CPQ8-10 and CPQ11-14) are indicators of child oral health-related quality of life. The aim of this study was to assess the validity and reliability of the self-applied CPQ8-10 and CPQ11-14 in Brazilian children, after translations and cultural adaptations in the Brazilian Portuguese language.MethodsSchoolchildren were recruited from general populations for pre-testing (n = 80), validity (n = 210), and test-retest reliability (n = 50) studies. They were also examined for dental caries, gingivitis, fluorosis, and malocclusion.ResultsChildren with greater dental caries experience in primary dentition had higher impacts on CPQ domains. Girls had higher scores for CPQ8-10 domains than boys. Mean CPQ11-14 scores were highest for 11-year-old children and lowest for 14-year-old children. Construct validity was supported by significant associations between the CPQ8-10 and CPQ11-14 scores and the global rating of oral health (r = 0.38, r = 0.43) and overall well-being (r = 0.39, r = 0.60), respectively. The Cronbach's alpha was 0.95 for both questionnaires. The test-retest reliabilities of the overall CPQ8-10 and CPQ11-14 scores were both excellent (ICC = 0.96, ICC = 0.92).ConclusionThe Brazilian Portuguese version of CPQ8-10 and CPQ11-14 was valuable and reliable for use in the Brazilian child population, although discriminant validity was sporadic due to the fact that impacts are mediated by others factors, such personal, social, and environmental variables.

Project description:Currently health care pathways (the combination and order of services that a patient receives to manage their injury) following a mild traumatic brain injury vary considerably. Some clinicians lack confidence in injury recognition, management and knowing when to refer. A clinical expert group developed the Brain Injury Screening Tool (BIST) to provide guidance on health care pathways based on clinical indicators of poor recovery. The tool aims to facilitate access to specialist services (if required) to improve longer term prognosis. The tool was developed using a three-step process including: 1) domain mapping; 2) item development and 3) item testing and review. An online retrospective survey of 114 adults (>16 years) who had experienced a mild brain injury in the past 10 years was used to determine the initial psychometric properties of the 15-item symptom scale of the BIST. Participants were randomised to complete the BIST and one of two existing symptom scales; the Rivermead Post-concussion Symptom Questionnaire (RPQ) or the Sports Concussion Assessment Test (SCAT-5) symptom scale to determine concurrent validity. Participant responses to the BIST symptom scale items were used to determine scale reliability using Cronbach's alpha. A principal components analysis explored the underlying factor structure. Spearman's correlation coefficients determined concurrent validity with the RPQ and SCAT-5 symptom scales. The 15 items were found to require a reading age of 6-8 years old using readability statistics. High concurrent validity was shown against the RPQ (r = 0.91) and SCAT-5 (r = 0.90). The BIST total symptom scale (α = 0.94) and the three factors identified demonstrated excellent internal consistency: physical/emotional (α = 0.90), cognitive (α = 0.92) and vestibular-ocular (α = 0.80). This study provides evidence to support the utility, internal consistency, factor structure and concurrent validity of the BIST. Further research is warranted to determine the utility of the BIST scoring criteria and responsiveness to change in patients.

Project description:AimThis study aimed to translate the TeamSTEPPS® Teamwork Perceptions Questionnaire (T-TPQ) into Japanese and assess its validity and reliability.DesignTranslation of the T-TPQ and a cross-sectional survey.MethodsFollowing a forward and back translation of the questionnaire, content validity was assessed by an expert panel using item-level content validity index. Construct validity was assessed by a confirmatory factor analysis. Further, intraclass correlation coefficient was estimated by test-retest methods.ResultsA total of 587 healthcare professionals responded to the translated T-TPQ. The item-level content validity index ranged between 0.8 and 1.0, indicating an acceptable content validity. The multiple fit indices showed an acceptable fitting model. Fifty-one healthcare professionals participated in the test-retest method. Intraclass correlation coefficients for all dimensions ranged from 0.838 to 0.957, indicating acceptable test-retest reliability. Our findings suggest that the Japanese version of the T-TPQ has acceptable validity and reliability.

Project description:BackgroundA review of the literature reveals a lack of validated instruments that particularly measure quality in the hospital discharge process. This study aims to develop and validate a survey instrument feasible for measuring quality (≥65 years) related to the discharge process based on elderly patients' experiences.MethodsConstruction of the Discharge Care Patient Experience Survey (DICARES) was based on 16 items identified by literature reviews. Intraclass correlation for test-retest was applied to assess consistency of the survey. Explorative factors analysis was applied to identify and validate the factor structures of the DICARES. Cronbach's α was used to assess internal reliability. To evaluate the external validity of the final DICARES questionnaire the patients' scores were correlated with scores obtained from the three other questionnaires; the Nordic Patient Experiences Questionnaire, the 12-Item Short-Form Health Survey and Subjective Health Complaints. The DICARES association with readmissions was examined.ResultsA total of 270 patients responded (64.4%). The mean age of participants was 77.1 years and 57.8% were men. The exploratory factor analysis resulted in a 10-item instrument consisting of three factors explaining 63.5% of the total variance. The Cronbach's α were satisfactory (≥70). Overall intraclass correlation was 0.76. A moderate Spearman correlation (rho = 0.54, p <0.01) was found between the total mean DICARES score and total mean score of the Nordic Patient Experiences Questionnaire. The total mean DICARES score was inversely associated with the quality indicator based on readmissions (OR 0.62, CI 95: 0.41-0.95, p = 0.028).ConclusionWe have developed a 10-item questionnaire consisting of three factors which may be a feasible instrument for measuring quality of the discharge process in elderly patients. Further testing in a wider population should be carried out before implementation in health care settings.

Project description:Dogs have distinct, consistent personalities, but the structure of dog personality is still unclear. Dog personality and unwanted behavior are often studied with behavioral questionnaires. Even though many questionnaires are reliable and valid measures of behavior, all new questionnaire tools should be extensively validated. Here, we examined the structure of personality and six unwanted behavior questionnaire sections: noise sensitivity, fearfulness, aggression, fear of surfaces and heights, separation anxiety and impulsivity/inattention with factor analyses. Personality consisted of seven factors: Insecurity, Training focus, Energy, Aggressiveness/dominance, Human sociability, Dog sociability and Perseverance. Most unwanted behavior sections included only one factor, but the impulsivity/inattention section divided into two factors (Hyperactivity/impulsivity and Inattention) and the aggression section into four factors (Barking, Stranger directed aggression, Owner directed aggression and Dog directed aggression). We also examined the internal consistency, test-retest reliability, inter-rater reliability and convergent validity of the 17 personality and unwanted behavior traits and discovered excellent reliability and validity. Finally, we investigated the discriminant validity of the personality traits, which was good. Our findings indicate that this personality and unwanted behavior questionnaire is a reliable and valid tool that can be used to study personality and behavior extensively.

Project description:ObjectiveThe aim of this study was to determine the health utility states of the most commonly used traumatic brain injury (TBI) clinical trial endpoint, the Extended Glasgow Outcome Scale (GOSE).Summary background dataHealth utilities represent the strength of one's preferences under conditions of uncertainty. There are insufficient data to indicate how an individual would value levels of disability after a TBI.MethodsThis was a cross-sectional web-based online convenience sampling adaptive survey. Using a standard gamble approach, participants evaluated their preferences for GOSE health states 1 year after a hypothetical TBI. The categorical GOSE was studied from vegetative state (GOSE2) to upper good recovery (GOSE8). Median (25th percentile, 75th percentile) health utility values for different GOSE states after TBI, ranging from -1 (worse than death) to 1 (full health), with 0 as reference (death).ResultsOf 3508 eligible participants, 3235 (92.22%) completed the survey. Participants rated lower GOSE states as having lower utility, with some states rated as worse than death, though the relationship was nonlinear and intervals were unequal between health states. Over 75% of participants rated a vegetative state (GOSE2, absence of awareness and bedridden) and about 50% rated lower severe disability (GOSE3, housebound needing all-day assistance) as conditions worse than death.ConclusionsIn the largest investigation of public perceptions about post-TBI disability, we demonstrate unequally rated health states, with some states perceived as worse than death. Although limited by selection bias, these results may guide future comparative-effectiveness research and shared medical decision-making after neurologic injury.
