Severity Prediction for COVID-19 Patients via Recurrent Neural Networks.

ABSTRACT: The novel coronavirus disease-2019 (COVID-19) pandemic has threatened the health of tens of millions of people worldwide and imposed heavy burden on global healthcare systems. In this paper, we propose a model to predict whether a patient infected with COVID-19 will develop severe outcomes based only on the patient's historical electronic health records (EHR) prior to hospital admission using recurrent neural networks. The model predicts risk score that represents the probability for a patient to progress into severe status (mechanical ventilation, tracheostomy, or death) after being infected with COVID-19. The model achieved 0.846 area under the receiver operating characteristic curve in predicting patients' outcomes averaged over 5-fold cross validation. While many of the existing models use features obtained after diagnosis of COVID-19, our proposed model only utilizes a patient's historical EHR to enable proactive risk management at the time of hospital admission.

Generated time-series prediction data of COVID-19's daily infections in Brazil by using recurrent neural networks.

Project description:In light of the COVID-19 pandemic that has struck the world since the end of 2019, many endeavors have been carried out to overcome this crisis. Taking into consideration the uncertainty as a feature of forecasting, this data article introduces long-term time-series predictions for the virus's daily infections in Brazil by training forecasting models on limited raw data (30 time-steps and 40 time-steps alternatives). The primary reuse potential of this forecasting data is to enable decision-makers to develop action plans against the pandemic, and to help researchers working in infection prevention and control to: (1) explore limited data usage in predicting infections. (2) develop a reinforcement learning model on top of this data-lake, which can perform an online game between the trained models to generate a new capable model for predicting future true data. The prediction data was generated by training 4200 recurrent neural networks (54 to 84 days validation periods) on raw data from Johns Hopkins University's online repository, to pave the way for generating reliable extended long-term predictions.

Genetic prediction of ICU hospitalization and mortality in COVID-19 patients using artificial neural networks.

Project description:There is an unmet need of models for early prediction of morbidity and mortality of Coronavirus disease-19 (COVID-19). We aimed to a) identify complement-related genetic variants associated with the clinical outcomes of ICU hospitalization and death, b) develop an artificial neural network (ANN) predicting these outcomes and c) validate whether complement-related variants are associated with an impaired complement phenotype. We prospectively recruited consecutive adult patients of Caucasian origin, hospitalized due to COVID-19. Through targeted next-generation sequencing, we identified variants in complement factor H/CFH, CFB, CFH-related, CFD, CD55, C3, C5, CFI, CD46, thrombomodulin/THBD, and A Disintegrin and Metalloproteinase with Thrombospondin motifs (ADAMTS13). Among 381 variants in 133 patients, we identified 5 critical variants associated with severe COVID-19: rs2547438 (C3), rs2250656 (C3), rs1042580 (THBD), rs800292 (CFH) and rs414628 (CFHR1). Using age, gender and presence or absence of each variant, we developed an ANN predicting morbidity and mortality in 89.47% of the examined population. Furthermore, THBD and C3a levels were significantly increased in severe COVID-19 patients and those harbouring relevant variants. Thus, we reveal for the first time an ANN accurately predicting ICU hospitalization and death in COVID-19 patients, based on genetic variants in complement genes, age and gender. Importantly, we confirm that genetic dysregulation is associated with impaired complement phenotype.

Characterisation of the blood RNA host response underpinning severity in COVID-19 patients

Project description:Infection with SARS-CoV-2 has highly variable clinical manifestations, ranging from asymptomatic infection through to life-threatening disease. Host whole blood transcriptomics can offer unique insights into the biological processes underpinning infection and disease, as well as severity. We performed whole blood RNA-Sequencing of individuals with varying degrees of COVID-19 severity. We used differential expression analysis and pathway enrichment analysis to explore how the blood transcriptome differs between individuals with mild, moderate, and severe COVID-19, performing pairwise comparisons between groups.

Multitasking via baseline control in recurrent neural networks.

Project description:Changes in behavioral state, such as arousal and movements, strongly affect neural activity in sensory areas, and can be modeled as long-range projections regulating the mean and variance of baseline input currents. What are the computational benefits of these baseline modulations? We investigate this question within a brain-inspired framework for reservoir computing, where we vary the quenched baseline inputs to a recurrent neural network with random couplings. We found that baseline modulations control the dynamical phase of the reservoir network, unlocking a vast repertoire of network phases. We uncovered a number of bistable phases exhibiting the simultaneous coexistence of fixed points and chaos, of two fixed points, and of weak and strong chaos. We identified several phenomena, including noise-driven enhancement of chaos and ergodicity breaking; neural hysteresis, whereby transitions across a phase boundary retain the memory of the preceding phase. In each bistable phase, the reservoir performs a different binary decision-making task. Fast switching between different tasks can be controlled by adjusting the baseline input mean and variance. Moreover, we found that the reservoir network achieves optimal memory performance at any first-order phase boundary. In summary, baseline control enables multitasking without any optimization of the network couplings, opening directions for brain-inspired artificial intelligence and providing an interpretation for the ubiquitously observed behavioral modulations of cortical activity.

Structured flexibility in recurrent neural networks via neuromodulation.

Project description:The goal of theoretical neuroscience is to develop models that help us better understand biological intelligence. Such models range broadly in complexity and biological detail. For example, task-optimized recurrent neural networks (RNNs) have generated hypotheses about how the brain may perform various computations, but these models typically assume a fixed weight matrix representing the synaptic connectivity between neurons. From decades of neuroscience research, we know that synaptic weights are constantly changing, controlled in part by chemicals such as neuromodulators. In this work we explore the computational implications of synaptic gain scaling, a form of neuromodulation, using task-optimized low-rank RNNs. In our neuromodulated RNN (NM-RNN) model, a neuromodulatory subnetwork outputs a low-dimensional neuromodulatory signal that dynamically scales the low-rank recurrent weights of an output-generating RNN. In empirical experiments, we find that the structured flexibility in the NM-RNN allows it to both train and generalize with a higher degree of accuracy than low-rank RNNs on a set of canonical tasks. Additionally, via theoretical analyses we show how neuromodulatory gain scaling endows networks with gating mechanisms commonly found in artificial RNNs. We end by analyzing the low-rank dynamics of trained NM-RNNs, to show how task computations are distributed.

Drug-Disease Severity and Target-Disease Severity Interaction Networks in COVID-19 Patients.

Project description:Drug interactions with other drugs are a well-known phenomenon. Similarly, however, pre-existing drug therapy can alter the course of diseases for which it has not been prescribed. We performed network analysis on drugs and their respective targets to investigate whether there are drugs or targets with protective effects in COVID-19, making them candidates for repurposing. These networks of drug-disease interactions (DDSIs) and target-disease interactions (TDSIs) revealed a greater share of patients with diabetes and cardiac co-morbidities in the non-severe cohort treated with dipeptidyl peptidase-4 (DPP4) inhibitors. A possible protective effect of DPP4 inhibitors is also plausible on pathophysiological grounds, and our results support repositioning efforts of DPP4 inhibitors against SARS-CoV-2. At target level, we observed that the target location might have an influence on disease progression. This could potentially be attributed to disruption of functional membrane micro-domains (lipid rafts), which in turn could decrease viral entry and thus disease severity.

Prediction of mRNA subcellular localization using deep recurrent neural networks.

Project description:MotivationMessenger RNA subcellular localization mechanisms play a crucial role in post-transcriptional gene regulation. This trafficking is mediated by trans-acting RNA-binding proteins interacting with cis-regulatory elements called zipcodes. While new sequencing-based technologies allow the high-throughput identification of RNAs localized to specific subcellular compartments, the precise mechanisms at play, and their dependency on specific sequence elements, remain poorly understood.ResultsWe introduce RNATracker, a novel deep neural network built to predict, from their sequence alone, the distributions of mRNA transcripts over a predefined set of subcellular compartments. RNATracker integrates several state-of-the-art deep learning techniques (e.g. CNN, LSTM and attention layers) and can make use of both sequence and secondary structure information. We report on a variety of evaluations showing RNATracker's strong predictive power, which is significantly superior to a variety of baseline predictors. Early biochemical analysis of COVID-19 patients helps severity prediction.

Project description:COVID-19 pandemic has put the protocols and the capacity of our Hospitals to the test. The management of severe patients admitted to the Intensive Care Units has been a challenge for all health systems. To assist in this challenge, various models have been proposed to predict mortality and severity, however, there is no clear consensus for their use. In this work, we took advantage of data obtained from routine blood tests performed on all individuals on the first day of hospitalization. These data has been obtained by standardized cost-effective technique available in all the hospitals. We have analyzed the results of 1082 patients with COVID19 and using artificial intelligence we have generated a predictive model based on data from the first days of admission that predicts the risk of developing severe disease with an AUC = 0.78 and an F1-score = 0.69. Our results show the importance of immature granulocytes and their ratio with Lymphocytes in the disease and present an algorithm based on 5 parameters to identify a severe course. This work highlights the importance of studying routine analytical variables in the early stages of hospital admission and the benefits of applying AI to identify patients who may develop severe disease.

SnoRNAs and miRNAs expression profiles underlying COVID-19 disease severity

Project description:The causative organism, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), exhibits a wide spectrum of clinical manifestations in disease-ridden patients. Differences in the severity of COVID-19 ranges from asymptomatic infections and mild cases to the severe form, leading to acute respiratory distress syndrome (ARDS) and multiorgan failure with poor survival. MiRNAs can regulate various cellular processes, including proliferation, apoptosis, and differentiation, by binding to the 3′UTR of target mRNAs inducing their degradation, thus serving a fundamental role in post-transcriptional repression. Alterations of miRNA levels in the blood have been described in multiple inflammatory and infectious diseases, including SARS-related coronaviruses. We used microarrays to delineate the miRNAs and snoRNAs signature in the peripheral blood of severe COVID-19 cases (n=9), as compared to mild (n=10) and asymptomatic (n=10) patients, and identified differentially expressed transcripts in severe versus asymptomatic, and others in severe versus mild COVID-19 cases. A cohort of 29 male age-matched patients were selected. All patients were previously diagnosed with COVID-19 using TaqPath COVID-19 Combo Kit (Thermo Fisher Scientific, Waltham, Massachusetts), or Cobas SARS-CoV-2 Test (Roche Diagnostics, Rotkreuz, Switzerland), with a CT value < 30. Additional criterion for selection was age between 35 and 75 years. Participants were grouped into severe, mild and asymptomatic. Classifying severe cases was based on requirement of high-flow oxygen support and ICU admission (n=9). Whereas mild patients were identified based on symptoms and positive radiographic findings with pulmonary involvement (n=10). Patients with no clinical presentation were labelled as asymptomatic cases (n=10).
