Computed Tomography Measurements of Sarcopenia Predict Length of Stay in Older Burn Patients.

ABSTRACT: Sarcopenia and frailty are associated with aging. In older burn patients, frailty has been associated with mortality and discharge disposition, but sarcopenia has not been examined. This study aims to investigate the relationship between frailty and computed tomography (CT)-derived sarcopenia with length of stay and mortality in older burn patients. Burn patients ≥60 years old admitted between 2008 and 2017 who had chest or abdomen CT scans within 1 week of admission were evaluated. Frailty was assessed using the Canadian Study of Health and Aging Clinical Frailty Scale (CFS). Sarcopenia was assessed on CT exams by measuring skeletal muscle index (SMI) of paraspinal muscles at T12 and all skeletal muscles at L3. The relationship between frailty scores and SMI with length of stay (LOS) and mortality was determined using logistic regression. Eighty-three patients (59 men; mean age 70.2 ± 8.5 years) had chest (n = 50) or abdomen (n = 60) CT scans. Mean TBSA = 14.3 ± 14.0%, LOS = 25.8 ± 21.3 days, CFS = 4.36 ± 0.99. Sixteen patients (19.3%) died while in the hospital. CT-derived measurement of SMI at T12 was significantly associated with LOS (P < .05), but not with mortality (P = .561). CT-derived metrics at L3 were not significantly associated with outcomes. CFS was not associated with LOS (P = .836) or mortality (P = .554). In older burn patients, low SMI of the paraspinal muscles at T12 was associated with longer LOS.

Project description:PurposeTo examine the use of multiple fast-helical free breathing computed tomography (FHFBCT) scans for ventilation measurement.MethodsTen patients were scanned 25 times in alternating directions using a FHFBCT protocol. Simultaneously, an abdominal pneumatic bellows was used as a real-time breathing surrogate. Regions-of-interest (ROIs) were selected from the upper right lungs of each patient for analysis. The ROIs were first registered using a published registration technique (pTV). A subsequent follow-up registration employed an objective function with two terms, a ventilation-adjusted Hounsfield Unit difference and a conservation-of-mass term labeled ΔΓ that denoted the difference between the deformation Jacobian and the tissue density ratio. The ventilations were calculated voxel-by-voxel as the slope of a first-order fit of the Jacobian as a function of the breathing amplitude.ResultsThe ventilations of the 10 patients showed different patterns and magnitudes. The average ventilation calculated from the deformation vector fields (DVFs) of the pTV and secondary registration was nearly identical, but the standard deviation of the voxel-to-voxel differences was approximately 0.1. The mean of the 90th percentile values of ΔΓ was reduced from 0.153 to 0.079 between the pTV and secondary registration, implying first that the secondary registration improved the conservation-of-mass criterion by almost 50% and that on average the correspondence between the Jacobian and density ratios as demonstrated by ΔΓ was less than 0.1. This improvement occurred in spite of the average of the 90th percentile changes in the DVF magnitudes being only 0.58 mm.ConclusionsThis work introduces the use of multiple free-breathing CT scans for free-breathing ventilation measurements. The approach has some benefits over the traditional use of 4-dimensional CT (4DCT) or breath-hold scans. The benefit over 4DCT is that FHFBCT does not have sorting artifacts. The benefits over breath-hold scans include the relatively small motion induced by quiet respiration versus deep-inspiration breath hold and the potential for characterizing dynamic breathing processes that disappear during breath hold.

Project description:BackgroundThe European Working Group on Sarcopenia in Older People (EWGSOP) recommends SARC-F as a tool for identifying sarcopenia among older adults. However, the role of SARC-F among older adults with cancer remains unexplored. We aimed to evaluate the diagnostic utility of SARC-F to identify those with sarcopenia, or low muscle mass (using skeletal muscle index [SMI]), and myosteatosis (using skeletal muscle density [SMD]) from computed tomography (CT) imaging and the association of SARC-F with all-cause mortality.MethodsOlder adults (≥60 years) presenting for initial consultation at UAB medical oncology clinic who underwent geriatric assessment were enrolled in a prospective cohort study. We identified study participants who completed SARC-F screening and had available CT imaging within 60 days of study enrollment. Using single-slice CT images at the L3 vertebral level, we computed SMI and SMD using published methods. Sarcopenia and myosteatosis were defined using published cutpoints. We calculated the sensitivity and specificity of SARC-F for detecting low muscle mass and low muscle density using published thresholds. Finally, we computed the impact of SARC-F and CT measures on overall survival using Kaplan-Meier curves and Cox regression models, after adjusting for age, sex, cancer type, and cancer stage.ResultsWe identified 212 older adults with a median age of 68.8 years; with 60.8% males, 76.6% whites, and pancreatic cancer (21.2%) being the most common malignancy. In the overall cohort, 30.7% had abnormal SARC-F using published cutpoints. SARC-F ≥ 4 had a sensitivity of 35% and a specificity of 76% to identify low muscle mass. SARC-F ≥ 4 had a sensitivity of 38% and a specificity of 74% to identify low muscle density. Those with SARC-F ≥ 4 and low SMI/SMD had worse survival compared to those with low SMI/SMD alone. Incorporating SARC-F improved survival prognostication beyond SMI and SMD (HR = 3.1; p < 0.001; Harrel's C from 0.73 to 0.76).ConclusionsSARC-F as a screening tool has limited diagnostic utility for identifying older adults with low muscle mass and/or density. However, SARC-F retains prognostic value independent of CT-based muscle measures in predicting mortality among older adults with cancer.
