Bending the Curve in Cardiovascular Disease Mortality: Bethesda + 40 and Beyond.

ABSTRACT: More than 40 years after the 1978 Bethesda Conference on the Declining Mortality from Coronary Heart Disease provided the scientific community with a blueprint for systematic analysis to understand declining rates of coronary heart disease, there are indications the decline has ended or even reversed despite advances in our knowledge about the condition and treatment. Recent data show a more complex situation, with mortality rates for overall cardiovascular disease, including coronary heart disease and stroke, decelerating, whereas those for heart failure are increasing. To mark the 40th anniversary of the Bethesda Conference, the National Heart, Lung, and Blood Institute and the American Heart Association cosponsored the "Bending the Curve in Cardiovascular Disease Mortality: Bethesda + 40" symposium. The objective was to examine the immediate and long-term outcomes of the 1978 conference and understand the current environment. Symposium themes included trends and future projections in cardiovascular disease (in the United States and internationally), the evolving obesity and diabetes epidemics, and harnessing emerging and innovative opportunities to preserve and promote cardiovascular health and prevent cardiovascular disease. In addition, participant-led discussion explored the challenges and barriers in promoting cardiovascular health across the lifespan and established a potential framework for observational research and interventions that would begin in early childhood (or ideally in utero). This report summarizes the relevant research, policy, and practice opportunities discussed at the symposium.

Project description:Objective: To investigate the independent and interactive effects of circulating folate and methylmalonic acid (MMA) status on cardiovascular disease (CVD) mortality. Methods: This study included 22,487 individuals from the U.S. National Health and Nutrition Examination Survey (NHANES) between 1999 and 2014. Kaplan-Meier and weighted Cox proportional hazards models were used to assess the associations of folate and MMA with CVD mortality. Restricted cubic splines (RCS) evaluated nonlinear relationships. Additive interaction method explored interactive effects. Sensitivity analysis tested the robustness of the findings. In vitro, CCK-8 assays were performed to assess the combined effects of folate and MMA on H9c2 cardiomyocyte viability. Transcriptomic analysis identified differentially expressed genes (DEGs) and their enriched pathways. Results: During 12.1 years of median follow-up, 1,510 cardiovascular deaths (4.4%) were recorded. RCS analysis demonstrated a U-shaped association between folate levels and CVD mortality, and a linear association for MMA. When MMA ≤ 182 nmol/L, folate levels were not associated with CVD mortality. However, among individuals with MMA > 182 nmol/L, both low and high folate levels were significantly associated with higher CVD mortality, with adjusted hazard ratios (aHRs) of 1.43 (95% CI: 1.19-1.71) and 2.36 (95% CI: 1.85-3.00), respectively (both P < 0.001). An additive interaction between folate and MMA on CVD mortality (RERI: 0.82 [95% CI: 0.06-1.58], API: 0.35 [95% CI: 0.03-0.67]; SI: 1.57 [95% CI: 1.27-2.00]). Sensitivity analyses confirmed these findings. In vitro, MMA (8 mM) significantly enhanced folate-induced (1.6 mM) cytotoxicity, reducing cell viability to 49.26% (P < 0.001). Transcriptomic analysis revealed 53 DEGs under combined treatment, enriched in inflammation-related pathways. Conclusions: Both population-based and experimental evidence suggest that elevated folate and MMA levels independently increase CVD mortality risk, with their interaction synergistically exacerbating cardiovascular outcomes.

Project description:PurposeConduct a systematic umbrella review to evaluate the relationship of physical activity (PA) with all-cause mortality, cardiovascular mortality, and incident cardiovascular disease (CVD); to evaluate the shape of the dose-response relationships; and to evaluate these relationships relative to the 2008 Physical Activity Guidelines Advisory Committee Report.MethodsPrimary search encompassing 2006 to March, 2018 for existing systematic reviews, meta-analyses, and pooled analyses reporting on these relationships. Graded the strength of evidence using a matrix developed for the Physical Activity Guidelines Advisory Committee.ResultsThe association of self-reported moderate-to-vigorous physical activity (MVPA) on all-cause mortality, CVD mortality, and atherosclerotic CVD-including incident coronary heart disease, ischemic stroke and heart failure-are very similar. Increasing MVPA to guidelines amounts in the inactive US population has the potential to have an important and substantial positive impact on these outcomes in the adult population. The following points are clear: the associations of PA with beneficial health outcomes begin when adopting very modest (one-third of guidelines) amounts; any MVPA is better than none; meeting the 2008 PA guidelines reduces mortality and CVD risk to about 75% of the maximal benefit obtained by physical activity alone; PA amounts beyond guidelines recommendations amount reduces risk even more, but greater amounts of PA are required to obtain smaller health benefits; and there is no evidence of excess risk over the maximal effect observed at about three to five times the amounts associated with current guidelines. When PA is quantified in terms of energy expenditure (MET·h·wk), these relationships hold for walking, running, and biking.ConclusionsTo avoid the risks associated with premature mortality and the development of ischemic heart disease, ischemic stroke, and all-cause heart failure, all adults should strive to reach the 2008 Physical Activity Guidelines for Americans.

Project description:ImportanceCardiovascular deaths and influenza epidemics peak during winter in temperate regions.ObjectivesTo quantify the temporal association between population increases in seasonal influenza infections and mortality due to cardiovascular causes and to test if influenza incidence indicators are predictive of cardiovascular mortality during the influenza season.Design, setting, and participantsTime-series analysis of vital statistics records and emergency department visits in New York City, among cardiovascular deaths that occurred during influenza seasons between January 1, 2006, and December 31, 2012. The 2009 novel influenza A(H1N1) pandemic period was excluded from temporal analyses.ExposuresEmergency department visits for influenza-like illness, grouped by age (≥0 years and ≥65 years) and scaled by laboratory surveillance data for viral types and subtypes, in the previous 28 days.Main outcomes and measuresMortality due to cardiovascular disease, ischemic heart disease, and myocardial infarction.ResultsAmong adults 65 years and older, who accounted for 83.0% (73 363 deaths) of nonpandemic cardiovascular mortality during influenza seasons, seasonal average influenza incidence was correlated year to year with excess cardiovascular mortality (Pearson correlation coefficients ≥0.75, P ≤ .05 for 4 different influenza indicators). In daily time-series analyses using 4 different influenza metrics, interquartile range increases in influenza incidence during the previous 21 days were associated with an increase between 2.3% (95% CI, 0.7%-3.9%) and 6.3% (95% CI, 3.7%-8.9%) for cardiovascular disease mortality and between 2.4% (95% CI, 1.1%-3.6%) and 6.9% (95% CI, 4.0%-9.9%) for ischemic heart disease mortality among adults 65 years and older. The associations were most acute and strongest for myocardial infarction mortality, with each interquartile range increase in influenza incidence during the previous 14 days associated with mortality increases between 5.8% (95% CI, 2.5%-9.1%) and 13.1% (95% CI, 5.3%-20.9%). Out-of-sample prediction of cardiovascular mortality among adults 65 years and older during the 2009-2010 influenza season yielded average estimates with 94.0% accuracy using 4 different influenza metrics.Conclusions and relevanceEmergency department visits for influenza-like illness were associated with and predictive of cardiovascular disease mortality. Retrospective estimation of influenza-attributable cardiovascular mortality burden combined with accurate and reliable influenza forecasts could predict the timing and burden of seasonal increases in cardiovascular mortality.
