Severe Acute Respiratory Syndrome Coronavirus 2 Seropositivity among Healthcare Personnel in Hospitals and Nursing Homes, Rhode Island, USA, July-August 2020.

ABSTRACT: Healthcare personnel are recognized to be at higher risk for infection with severe acute respiratory syndrome coronavirus 2. We conducted a serologic survey in 15 hospitals and 56 nursing homes across Rhode Island, USA, during July 17-August 28, 2020. Overall seropositivity among 9,863 healthcare personnel was 4.6% (95% CI 4.2%-5.0%) but varied 4-fold between hospital personnel (3.1%, 95% CI 2.7%-3.5%) and nursing home personnel (13.1%, 95% CI 11.5%-14.9%). Within nursing homes, prevalence was highest among personnel working in coronavirus disease units (24.1%; 95% CI 20.6%-27.8%). Adjusted analysis showed that in hospitals, nurses and receptionists/medical assistants had a higher likelihood of seropositivity than physicians. In nursing homes, nursing assistants and social workers/case managers had higher likelihoods of seropositivity than occupational/physical/speech therapists. Nursing home personnel in all occupations had elevated seropositivity compared with hospital counterparts. Additional mitigation strategies are needed to protect nursing home personnel from infection, regardless of occupation.

Project description:Drainage enhancement (e.g., ditch digging, open-marsh water management, runnelling) has long been used to reduce tidal marsh soil waterlogging and surface ponding to promote salt hay production and mosquito control. Now it is also being used as a tool to enhance marsh resilience to sea-level rise despite a lack of studies that evaluate its effectiveness as an intervention approach. We therefore conducted a controlled field experiment to evaluate short-term responses to drainage enhancement of a Rhode Island (USA) salt marsh. Drainage enhancement elicited rapid physical changes in portions of the marsh including declines in water levels and marsh elevation, but the biological components examined (e.g., vegetation and bird community composition) were largely unaffected. In two of the four areas monitored, marsh surface inundation duration declined from > 75% to 3-10% and low water levels dropped by 20 cm. Mean annual marsh surface elevation in monitoring plots increased 5 mm one year after drainage enhancement but dropped to 11 mm below initial conditions after three years. The decline in elevation varied among habitats, with the greatest decline (18 mm) found in areas dominated by Spartina alterniflora and/or bare ground. Vegetation community composition and % cover and heights of dominant species were unchanged, but areas that were initially bare had fully revegetated after three years. Drainage enhancement also had no effects on bird community composition or marsh sparrow (Ammodramus spp.) density. Our study provides evidence that drainage enhancement can relieve waterlogging and some of its impacts without any apparent adverse effects on the composition and abundance of existing vegetation and bird communities. At the same time, it can induce a loss of marsh platform elevation that has the potential to offset declining water levels and inhibit high marsh enhancement. Finally, unanticipated findings from our study provide evidence that the effects of larger-scale drivers such as sea-level rise may predominate over localized responses to drainage enhancement itself.

Project description:BackgroundFalls and their consequences are particularly common in older adults in hospitals and long-term care (LTC) facilities.AimTo avoid falls and their consequences, and provide nurses with an overview of all relevant research literature on fall prevention, and a practice guideline on fall prevention in older adults was developed.MethodsThe development process included a systematic literature review to identify systematic reviews and primary studies on the topic of fall prevention, an assessment of the study quality, the preparation of meta-analyses to summarize the results, and the application of the GRADE (Grading of Recommendations Assessment, Development and Evaluation) approach to grade the scientific literature. The guideline panel and an external multidisciplinary team graded the recommendations using the Delphi method. In addition, the panel and team formulated expert opinions.ResultsA total of 79 randomized controlled trials on fall prevention were identified, which formed the basis of the recommendations. Strongly recommended measures for both settings included multifactorial interventions, professionally supported body exercise interventions, and education and counselling interventions. The panel and team did not recommend the use of a specific assessment tool for fall risk assessment, low-floor beds in hospitals, or body exercise interventions in frail residents.Linking evidence to actionDuring the development of this guideline, particular attention was paid to collecting evidence-based knowledge relevant to practice. By applying the recommendations, the outlined nursing care is justified, enabling healthcare personnel to achieve the overriding goal of providing optimal care to persons at risk of falling. Evidence for several fall prevention interventions was graded as very low. Sound intervention studies are necessary to strengthen the confidence in the evidence for low-floor beds, alarm sensor systems, medication review, and staff education in hospitals.
