A dataset of lung sounds recorded from the chest wall using an electronic stethoscope.

ABSTRACT: The advancement of stethoscope technology has enabled high quality recording of patient sounds. We used an electronic stethoscope to record lung sounds from healthy and unhealthy subjects. The dataset includes sounds from seven ailments (i.e., asthma, heart failure, pneumonia, bronchitis, pleural effusion, lung fibrosis, and chronic obstructive pulmonary disease (COPD)) as well as normal breathing sounds. The dataset presented in this article contains the audio recordings from the examination of the chest wall at various vantage points. The stethoscope placement on the subject was determined by the specialist physician performing the diagnosis. Each recording was replicated three times corresponding to various frequency filters that emphasize certain bodily sounds. The dataset can be used for the development of automated methods that detect pulmonary diseases from lung sounds or identify the correct type of lung sound. The same methods can also be applied to the study of heart sounds.

Project description:Most heart diseases are associated with and reflected by the sounds that the heart produces. Heart auscultation, defined as listening to the heart sound, has been a very important method for the early diagnosis of cardiac dysfunction. Traditional auscultation requires substantial clinical experience and good listening skills. The emergence of the electronic stethoscope has paved the way for a new field of computer-aided auscultation. This article provides an in-depth study of (1) the electronic stethoscope technology, and (2) the methodology for diagnosis of cardiac disorders based on computer-aided auscultation. The paper is based on a comprehensive review of (1) literature articles, (2) market (state-of-the-art) products, and (3) smartphone stethoscope apps. It covers in depth every key component of the computer-aided system with electronic stethoscope, from sensor design, front-end circuitry, denoising algorithm, heart sound segmentation, to the final machine learning techniques. Our intent is to provide an informative and illustrative presentation of the electronic stethoscope, which is valuable and beneficial to academics, researchers and engineers in the technical field, as well as to medical professionals to facilitate its use clinically. The paper provides the technological and medical basis for the development and commercialization of a real-time integrated heart sound detection, acquisition and quantification system.

A dataset of radar-recorded heart sounds and vital signs including synchronised reference sensor signals.

Project description:Radar systems allow for contactless measurements of vital signs such as heart sounds, the pulse signal, and respiration. This approach is able to tackle crucial disadvantages of state-of-the-art monitoring devices such as the need for permanent wiring and skin contact. Potential applications include the employment in a hospital environment but also in home care or passenger vehicles. This dataset consists of synchronised data which are acquired using a Six-Port-based radar system operating at 24?GHz, a digital stethoscope, an ECG, and a respiration sensor. 11 test subjects were measured in different defined scenarios and at several measurement positions such as at the carotid, the back, and several frontal positions on the thorax. Overall, around 223?minutes of data were acquired at scenarios such as breath-holding, post-exercise measurements, and while speaking. The presented dataset contains reference-labeled ECG signals and can therefore easily be used to either test algorithms for monitoring the heart rate, but also to gain insights about characteristic effects of radar-based vital sign monitoring.

Evidence of the Sequential Changes of Lung Sounds in COVID-19 Pneumonia Using a Novel Wireless Stethoscope with the Telemedicine System.

Project description:A 60-year-old woman was admitted to our hospital due to coronavirus disease 2019 (COVID-19) pneumonia with a chief complaint of persistent low-grade fever and dry cough for two weeks. Thoracic computed tomography demonstrated a crazy paving pattern in the bilateral lower lobes. In a COVID-19 ward, we used a novel wireless stethoscope with a telemedicine system and successfully recorded and shared the lung sounds in real-time between the red and green zones. The fine crackles at the posterior right lower lung fields changed from mid-to-late (day 1) to late inspiratory crackles (day 3), which disappeared at day 5 along with an improvement in both the clinical symptoms and thoracic CT findings.

Electronic Stethoscope Filtering Mimics the Perceived Sound Characteristics of Acoustic Stethoscope.

Project description:Electronic stethoscopes offer several advantages over conventional acoustic stethoscopes, including noise reduction, increased amplification, and ability to store and transmit sounds. However, the acoustical characteristics of electronic and acoustic stethoscopes can differ significantly, introducing a barrier for clinicians to transition to electronic stethoscopes. This work proposes a method to process lung sounds recorded by an electronic stethoscope, such that the sounds are perceived to have been captured by an acoustic stethoscope. The proposed method calculates an electronic-to-acoustic stethoscope filter by measuring the difference between the average frequency responses of an acoustic and an electronic stethoscope to multiple lung sounds. To validate the method, a change detection experiment was conducted with 51 medical professionals to compare filtered electronic, unfiltered electronic, and acoustic stethoscope lung sounds. Participants were asked to detect when transitions occurred in sounds comprising several sections of the three types of recordings. Transitions between the filtered electronic and acoustic stethoscope sections were detected, on average, by chance (sensitivity index equal to zero) and also detected significantly less than transitions between the unfiltered electronic and acoustic stethoscope sections ( ), demonstrating the effectiveness of the method to filter electronic stethoscopes to mimic an acoustic stethoscope. This processing could incentivize clinicians to adopt electronic stethoscopes by providing a means to shift between the sound characteristics of acoustic and electronic stethoscopes in a single device, allowing for a faster transition to new technology and greater appreciation for the electronic sound quality.

Safety of mechanical lung vibrator and high-frequency chest wall oscillation in patients with cardiac implantable electronic device.

Project description:BackgroundChest physiotherapy (CPT) is a non-pharmacological therapy to facilitate airway secretion removal. There have been concerns about potential electromagnetic interference (EMI) and lead integrity problems during the use of vibrating CPT devices in patients with cardiac implantable electronic devices (CIEDs).HypothesisTwo CPT devices can be used safely in patients with CIED.MethodsVolunteer patients with CIED underwent device interrogation to check lead integrity and device function before and after application of CPT devices. Mechanical lung vibrator and high-frequency chest wall oscillation (HFCWO) vests were used while monitoring surface electrocardiograms and intra-cardiac electrograms.ResultsWe prospectively enrolled 46 patients with CIEDs (25 pacemakers, 15 implantable cardioverter-defibrillators, and six cardiac resynchronization therapy-defibrillators). There was no noise detection or EMI during CPT in any patient. None of the patients showed clinically significant changes in lead integrity parameters. HFCWO inappropriately accelerated the pacing rate up to the maximal programmed value in five patients with pacemakers and two with cardiac resynchronization therapy-defibrillators.ConclusionCPT may be safely applied to patients with CIED without compromising lead integrity or device function, except for unwanted increase in pacing rate caused by misdetection of chest wall vibration as patients' activity while using HFCWO. Deactivation of the accelerometer-based activity sensor may be needed when HFCWO is planned for CPT.

Lung Volume Reduction in Emphysema Improves Chest Wall Asynchrony.

Project description:BackgroundLung volume reduction (LVR) techniques improve lung function in selected patients with emphysema, but the impact of LVR procedures on the asynchronous movement of different chest wall compartments, which is a feature of emphysema, is not known.MethodsWe used optoelectronic plethysmography to assess the effect of surgical and bronchoscopic LVR on chest wall asynchrony. Twenty-six patients were assessed before and 3 months after LVR (surgical [n = 9] or bronchoscopic [n = 7]) or a sham/unsuccessful bronchoscopic treatment (control subjects, n = 10). Chest wall volumes were divided into six compartments (left and right of each of pulmonary ribcage [Vrc,p], abdominal ribcage [Vrc,a], and abdomen [Vab]) and phase shift angles (θ) calculated for the asynchrony between Vrc,p and Vrc,a (θRC), and between Vrc,a and Vab (θDIA).ResultsParticipants had an FEV₁ of 34.6 ± 18% predicted and a residual volume of 217.8 ± 46.0% predicted with significant chest wall asynchrony during quiet breathing at baseline (θRC, 31.3° ± 38.4°; and θDIA, -38.7° ± 36.3°). Between-group difference in the change in θRC and θDIA during quiet breathing following treatment was 44.3° (95% CI, -78 to -10.6; P = .003) and 34.5° (95% CI, 1.4 to 67.5; P = .007) toward 0° (representing perfect synchrony), respectively, favoring the LVR group. Changes in θRC and θDIA were statistically significant on the treated but not the untreated sides.ConclusionsSuccessful LVR significantly reduces chest wall asynchrony in patients with emphysema.

Robotic-Assisted Muscle-Sparing Chest Wall Resection Without Thoracotomy: Tips and Benefits for Lung Cancer Patients With Chest Wall Invasion.

Project description:This report describes our technical methods of robotic-assisted chest wall resection concomitant with lobectomy, which did not require the division of major extrathoracic muscles and spreading of the ribs. In both cases, the patients were able to raise their arms on the affected side immediately after surgery, without any shoulder dysfunction. This robotic-assisted muscle-sparing chest wall resection without thoracotomy technique enables preservation of the extrathoracic muscles and allows patients to maintain their quality of life, thus making it possible for patients to promptly receive adjuvant therapy.

Treatment of T3N0 non-small cell lung cancer with chest wall invasion using stereotactic body radiotherapy.

Project description:Objectives:Chest wall invasion (CWI) is observed in 5% of localized non-small cell lung cancer (NSCLC). The role of stereotactic body radiotherapy (SBRT) in these patients is unknown. We investigate the safety and efficacy of SBRT in patients with T3N0 NSCLC due to CWI. Methods:Patients with T3N0 NSCLC due to CWI were identified using a prospective registry. CWI was defined as radiographic evidence of soft tissue invasion or bony destruction. We excluded patients with recurrent or metastatic disease. All patients were treated with definitive SBRT. Prescribed dose was 50?Gy in 5 fractions for most patients. Kaplan-Meier analysis was used to estimate survival outcomes. Results:We identified 12 patients treated between 2006 and 2017. Median age was 70 (range, 58-85). Median tumor diameter was 3.0?cm (range, 0.9-7.2). Median survival was 12.0?months (range, 2.4-63). At a median follow-up of 8.9?months (range, 2.1-63), 1-year primary tumor control was 89%, involved lobar control was 89%, local-regional control was 82%, distant control was 91%, and survival was 63%. Of the 4 patients with pre-treatment chest wall pain, 3 reported improvement after SBRT. Two patients reported new grade 1-2 chest wall pain. No grade 3+ toxicity was reported, with 1 patient experiencing grade 1 skin toxicity and 3 patients experiencing grade 1-2 radiation pneumonitis. Conclusions:SBRT for CWI NSCLC is safe, with high early tumor control and low treatment-related toxicity. Most patients with pre-treatment chest wall pain experienced relief after SBRT, with no grade 3+ toxicity observed.

Chest X-ray dataset and ground truth for lung segmentation.

Project description:Chest X-ray images are a valuable tool for accurately and efficiently diagnosing Covid-19 with the assistance of computer technology. These images enable the detection of diseases in internal organs, particularly the lungs, by providing crucial information about the pathological state of the lungs and other internal organs and tissues. Segmentation plays an essential role in the earliest stages of disease detection through computer-assisted analysis of medical images. This method enables the extraction of significant elements from the image, facilitating the identification of relevant areas. In the subsequent stage, healthcare professionals might acquire more precise diagnosis outcomes. Deep learning plays a significant role in developing models to achieve exact and efficient diagnostic results in picture segmentation and image classification procedures. However, using deep learning models in the image segmentation process necessitates the availability of image datasets and ground truth that radiologists have validated to facilitate the training process. The dataset provided in this article comprises 292 chest X-ray images obtained from Airlangga University Hospital in Indonesia. These images are accompanied with ground truth data that has been meticulously verified by radiologists. The offered X-ray images encompass those of patients diagnosed with Covid-19, pneumonia and those representing normal conditions. The provided dataset exhibits potential utility in advancing artificial intelligence techniques for segmentation and classification procedures.

Robotic chest wall resection for primary benign chest wall tumors and locally advanced lung cancer: an institutional case series and national report.

Project description:BackgroundLimited data exists for robotic chest wall resection; we report institutional and national experience of robotic chest wall resection.MethodsIn this comparative retrospective case series we describe patients who underwent robotic chest wall resection at our institution and enrich this case series with data from the National Cancer Database (NCDB). We describe our preoperative workup, operative technique, and postoperative care. Outcomes included conversion to open, length of stay, readmissions, and 30- and 90-day mortality. The results are descriptively reported and compared.ResultsWe describe 6 patients institutionally and 96 NCDB patients. At our institution 66.7% were males, median age was 70.0 (range, 39-91) years, and 50% were primary chest wall tumors. Median tumor size was 5.25 (range, 2.3-8.3) cm. Outcomes were as follows: no open conversions, median length of stay 3 (range, 1-6) days, no unplanned 30-day readmissions or 90-day mortality. In the NCDB, 55.2% were males with median age of 68.5 (range, 30-89) years. Median tumor size was 3.90 (range, 2.4-6.0) cm. NCDB outcomes were as follows: 18.8% open conversion, median length of stay 7 (range, 5-10) days, 3.1% unplanned 30-day readmission, and 8.3% 90-day mortality. Our institutional case series had 18.0 months median follow-up (range, 6-54 months) with no functional deficits. Median survival in NCDB was 49.6 months.ConclusionsRobotic chest wall resection is feasible and is performed nationally with acceptable short- and long-term outcomes. Our institutional experience reports our technique, resultant short hospital stay, and excellent functional outcomes.
