Scheimpflug Corneal Densitometry Changes After Trabeculectomy.

ABSTRACT:

Purpose

To study the possible changes in Scheimpflug corneal densitometry 6 months after mitomycin C-augmented trabeculectomy and to compare these measurements with healthy controls.

Methods

Corneal densitometry was monitored with the Pentacam HR3 before and 6 months after first-time uncomplicated mitomycin C-augmented trabeculectomy in 42 eyes of 42 white patients with open-angle glaucoma and in 22 healthy age-matched controls. Preoperative intraocular pressure (IOP), central corneal thickness, known duration of the disease, gender, the type and number of substances, applications and amount of benzalkonium chloride per day, and postoperative topical cortisone use were tested for possible correlations in the trabeculectomy group.

Results

There was a statistically significant reduction of mean diurnal IOP from 19.0 ± 7.7 to 11.1 ± 7.7 mm Hg (P = 0.003) and the amount of pressure-lowering substances from 3.7 ± 1.0 to 0.1 ± 0.5 (P < 0.001). Densitometry measurements decreased in the entire cornea from 25.5 ± 5.7 to 23.1 ± 5.8 grayscale units (P = 0.001) with emphasis in the anterior layer. They returned close to normal 6 months after trabeculectomy and were not statistically significantly different compared with a healthy control group (22.8 ± 3.4 grayscale unit; P = 0.824). No correlations could be found with these observations and possible causing factors studied.

Conclusions

Corneal densitometry, an objective and sensitive measure of corneal transparency, returned close to normal 6 months after trabeculectomy. Although the observations cannot be associated with any causing factor in this study, the significant IOP reduction and the nearly complete cessation of topical antiglaucomatous substances including benzalkonium chloride seem to be the most plausible reasons for this finding.

Project description:BackgroundObjective measurement of corneal densitometry (CD) values can be used to assess corneal transparency and health status, to investigate corneal diseases, and to review anterior segment surgeries. However, literature regarding the association between CD and corneal parameters in healthy adolescent and older individuals is limited. This study investigated age-related changes in Scheimpflug CD values and their correlations with age, sex, and corneal topographic parameters.MethodsThis retrospective cross-sectional observational study included 347 eyes from 181 consecutive healthy Chinese participants aged between 5 and 90 years. They were divided into 9 age groups: 5-9, 10-19, 20-29, 30-39, 40-49, 50-59, 60-69, 70-79, and 80-90 years. CD and corneal topographic measurements were measured using the Oculus Pentacam. To evaluate CD, the cornea was divided into 3 layers according to depth (anterior 120 µm, central, and posterior 60 µm), and into 4 annular regions according to diameter (0-2, 2-6, 6-10, and 10-12 mm).ResultsCD across different depths and regions was positively correlated with age (all P values <0.001). Of the 3 layers of corneal thickness, CD was highest in the anterior 120 µm and lowest in the posterior 60 µm (all P values <0.05). Among the 4 annular regions spanning the corneal diameter, the lowest CD values were 6-10, 2-6, and 0-2 mm at 5-29, 30-69, and 70-89 years, respectively. The highest CD values were 10-12 mm at 5-79 years, and 6-10 mm at 80-90 years (all P values <0.05). CD values of 10-12 mm in the anterior 120 µm corneal layer were significantly lower in men than in women (Z=-2.353; P=0.019). CD of 0-10 mm in each layer was not significantly different between sexes (all P values >0.05). Corneal topographic measurements, including flat-axis keratometry (K1), steep-axis keratometry (K2), and spherical aberration, were slightly positively correlated with age and CD (all P values <0.05). However, central corneal thickness (CCT) and thinnest corneal thickness (TCT), and age or CD showed no correlation (all P values >0.05).ConclusionsWith age, CD, keratometry, and spherical aberration gradually increased, while the corneal thickness did not change significantly.

Project description:Purpose: To investigate the relationship between corneal biomechanical and ocular biometric parameters, and to explore biomechanical asymmetry between anisometropic eyes using the corneal visualization Scheimpflug technology device (Corvis ST). Methods: 180 anisometropic participants were included. Participants were divided into low (1.00≤△Spherical equivalent (SE) < 2.00D), moderate (2.00D≤△SE < 3.00D) and high (△SE ≥ 3.00D) anisometropic groups. Axial length (AL), keratometry, anterior chamber depth (ACD) and corneal biomechanical parameters were assessed using the OA-2000 biometer, Pentacam HR and Corvis ST, respectively. Results: The mean age of participants was 16.09 ± 5.64 years. Stress-Strain Index (SSI) was positively correlated with SE (r = 0.501, p < 0.001) and negatively correlated with AL (r = -0.436, p < 0.001). Some other Corvis ST parameters had weak correlation with SE or AL. Corneal biomechanical parameters except for time of first applanation (A1T), length of second applanation (A2L), deformation amplitude (DA), first applanation stiffness parameter (SPA1) and ambrosia relational thickness-horizontal (ARTh) were correlated with ametropic parameters (SE or AL) in multiple regression analyses. A1T, velocity of first applanation (A1V), time of second applanation (A2T), A2L, velocity of second applanation (A2V), corneal curvature radius at highest concavity (HCR), peak distance (PD), DA, deformation amplitude ratio max (2 mm) (DAR), SPA1, integrated radius (IR), and SSI showed significant differences between fellow eyes (p < 0.05). There was no significant difference in asymmetry of corneal biomechanics among the three groups (p > 0.05). Asymmetry of some biomechanical parameters had weak correlation with asymmetry of mean corneal curvatures and ACD. However, asymmetry of corneal biomechanical parameters was not correlated with asymmetry of SE or AL (p > 0.05). Conclusion: More myopic eyes had weaker biomechanical properties than the contralateral eye in anisometropia. However, a certain linear relationship between anisometropia and biomechanical asymmetry was not found.
