A Systematic Approach to Arthroscopic Femoroplasty With Conservative Management of the Hip Capsule.

ABSTRACT: As hip arthroscopy has become increasingly used to treat femoroacetabular impingement, the importance of a complete femoroplasty to properly address cam impingement has been demonstrated. In doing so, different capsulotomy techniques have been described for gaining access to the hip joint as well as the peripheral compartment for cam resection. The periportal capsulotomy technique allows joint access while preserving the structural integrity of the iliofemoral ligament, obviating the need for capsular closure. We present a systematic approach and surgical technique for performing a complete arthroscopic femoroplasty while maintaining conservative hip capsule management through a periportal capsulotomy.

The Circumferential Femoroplasty: An All-Arthroscopic Technique for Addressing a Challenging Hip Deformity.

Project description:Femoroacetabular impingement is recognized as a common cause of hip pain. Cam-type femoroacetabular impingement results from abnormal contact between an aspherical femoral head and the acetabular rim during hip range of motion, leading to labral tearing, cartilage damage, and, eventually, osteoarthritis. Arthroscopic correction of this bony deformity has been well described, particularly in the anterolateral quadrant of the femoral neck. Some deformities extend well beyond this quadrant, involving most or all of the circumference of the femoral neck, making arthroscopic decompression a challenge. We present a post-less, all-arthroscopic technique for performing a circumferential cam decompression using 3-dimensional preoperative planning software and interactive fluoroscopy-integrated computer vision interface.

Arthroscopic Hip Capsule Reconstruction for Anterior Hip Capsule Insufficiency in the Revision Setting.

Project description:Iatrogenic hip instability is increasingly recognized as a cause of persistent pain and disability after hip arthroscopy. Many authors currently advocate capsular repair to reduce postoperative instability. However, anatomic deficiencies in the anterosuperior capsule can prevent a functional capsular repair, particularly in the revision setting. Capsular reconstruction has been shown to restore biomechanical stabilization in cadaveric models and improve short-term patient outcomes in patients with primary hip arthroscopy failure. Arthroscopic hip capsular reconstruction is technically challenging, largely owing to complex suture management and difficulties with graft placement and sizing. This article describes the capsular reconstruction technique, detailing the technical aspects of anterosuperior capsular defect identification; capsular preparation; suture management; and dermal allograft sizing, preparation, and positioning.

Arthroscopic capsule reconstruction in the hip using iliotibial band allograft.

Project description:The hip capsule has been identified as an important static stabilizer of the hip joint. Despite the intrinsic bony stability of the hip socket, the capsule plays a key role in hip stability, particularly at the extremes of motion, and the iliofemoral ligament is the most important stabilizer in extension and external rotation. Patients who do not undergo capsular closure or plication may continue to complain of hip pain and dysfunction postoperatively, likely because of microinstability or muscle invagination into the capsular defect, and high-resolution magnetic resonance imaging or magnetic resonance arthrography will identify the capsular defect. Seen primarily in the revision setting, capsular defects can cause recurrent stress at the chondrolabral junction. An attempt at secondary closure can be challenging because of capsular limb adherence to the surrounding soft tissues. Therefore reconstruction may be the only possible surgical solution for this problem. We describe our new surgical technique for arthroscopic hip capsular reconstruction using iliotibial band allograft.

The Pull String Method: Optimizing Visualization for Arthroscopic Femoroplasty.

Project description:Capsular management during hip arthroscopy represents an important component to access, visualization, and restoration of hip stability. In the setting of femoroacetabular impingement, the peripheral compartment represents access to the femoral neck where abnormal morphology is commonly addressed with arthroscopic femoroplasty. While various techniques have been described for peripheral compartment visualization, difficulty can arise in creating a wide display of the femoral neck while minimizing subsequent impairment of the capsule. In order to obtain an unobstructed view of the femoral neck, the hip capsule must be managed skillfully. The use of hip flexion to relax the capsule for visualization of the peripheral compartment is a nearly universal practice for hip surgeons. However, differences in management of the capsule have previously included traction sutures and further capsulotomies. In this technical note, we describe our Pull String method for peripheral compartment access using an interportal capsulotomy with strategic traction suture placement and choreography of suture tensioning. This enables full view of the femoral neck for successful femoroplasty, while still allowing for complete capsule repair to help restore stability.

Arthroscopic Hip Surgery versus Conservative Therapy on Femoroacetabular Impingement Syndrome: A Meta-Analysis of RCTs.

Project description:To determine the outcome and differences between arthroscopic hip surgery and conservative therapy in patients suffering from femoroacetabular impingement syndrome, we searched articles from PubMed, Embase, Cochrane, Web of Science and Clinicaltrials.gov using a Boolean search algorithm. Only randomized controlled trials comparing arthroscopic hip surgery and conservative therapy were included in this meta-analysis of femoroacetabular impingement syndrome management. Two authors determined eligibility, extracted the needed data and assessed the risk of bias of eligible studies independently. Then we meta-analyzed three articles to assess pooled estimate size (ES) and 95% confidence interval for Hip Outcome Score of activities of daily living (HOS ADL subscale), Hip Outcome Score sport (HOS sports subscale) and International Hip Outcome Tool (iHOT-33) analyses were performed by using STATA version 14.0 MP (STATA, College Station, TX, USA) with the principal summary measures are mean between group difference, sample size, and standard deviation. We collected 52 articles in total after removing duplicates and screened by titles and abstracts. A total of three RCTs were included finally. There was definite evidence of additional benefit of arthroscopic hip surgery against conservative therapy in the field of improving quality of life (three trials, 575 participants, ES = 2.109, 95% CI: 1.373 to 2.845, I2 = 42.8%, P = 0.000) and activity of daily living (two trials, 262 participants, ES = 9.220, 95% CI: 5.931 to 12.508, I2 = 16.5%, P = 0.000). However, no significant difference could be seen in sports function improvement (two trials, ES = 7.562, 95% CI: -2.957 to 18.082, I2 = 60.1%, P = 0.159). In conclusion, this meta-analysis suggests that arthroscopic hip surgery provided essential benefit compared with conservative therapy in improving activity of daily living and quality of life.

Project description:Hip arthroscopy techniques have evolved to treat a wide range of extra-articular pathologies. Subspine impingement commonly occurs in femoroacetabular impingement, particularly in athletes with a high range of motion, patients with low degrees of femoral version, and those with large subspine deformities. A reliable technique that preserves the hip capsule would be helpful for hip arthroscopy surgeons. This note details our technique using pericapsular windows proximal to the interportal capsulotomy to access and decompress a subspine deformity in the setting of hip femoroacetabular impingement.

Arthroscopic Approach to Preservation of the Hip with Avascular Necrosis.

Project description:Avascular necrosis (AVN) of the hip is a devastating disease that affects middle-aged adults with poor outcomes if not treated in its early stages. In recent years, subchondroplasty with calcium phosphate solution has shown promising results. Concomitant intra-articular pathologies, including femoroacetabular impingement and chondral lesions, have been described in hips affected by AVN. These should be addressed at the time of surgery to lower the risk of failure. In this Technical Note, we describe an arthroscopic approach to femoral head subchondroplasty with precollapse lesion in AVN affected hip, combined with labral reconstruction and acetabular chondral treatment.

Arthroscopic Identification and Management of Recurrent Iliopsoas Impingement After Total Hip Arthroplasty.

Project description:Arthroscopic release of the iliopsoas tendon for iliopsoas impingement (IPI) after total hip arthroplasty (THA) at the lesser trochanter gives good results. However, where IPI then recurs, due to adhesions between the healing iliopsoas tendon and the surrounding soft tissue, and nonoperative measures have failed, a revision THA procedure is usually considered. We propose a technique of arthroscopic visualization of the recurrent IPI and a subsequent psoas tenotomy at the level of the hip joint using an outside-in capsulotomy approach. This secondary tenotomy, located proximally directly at the level of the recurrent impingement, allows relief of the painful symptoms without compromising the muscle function of the iliopsoas and precludes the need for a complex THA revision.
