Organizing Audible Alarm Sounds in the Hospital: A Card-Sorting Study.

ABSTRACT: In hospitals, clinicians are presented with varied and disorganized alarm sounds from disparate devices. While there has been attention to reducing inactionable alarms to address alarm overload, little effort has focused on organizing, simplifying, or improving the informativeness of alarms. We sought to elicit nurses' tacit interpretation of alarm events to create an organizational structure to inform the design of advanced alarm sounds or integrated alert systems. We used open card sorting to evaluate nurses' perception of the relatedness of different alarm events. Seventy hospital nurses sorted 89 alarm events into groups they believed could or should be indicated by the same sound. We conducted factor analysis on a similarity matrix of frequency of alarm event pairings to interpret how strongly alarm events loaded on different alarm groups (factors). We interpreted participants' grouping rationale from their group labels and comments. Urgency of response was the most common grouping rationale. Participants also grouped: 1) monitoring-related events, 2) device-related events, and 3) events related to calls and patients. Our findings support standardization and integration of alarm sounds across devices toward a simpler and more informative hospital alarm environment.

Project description:ObjectivesThe analysis of categorization of everyday sounds is a crucial aspect of the perception of our surrounding world. However, it constitutes a poorly explored domain in developmental studies. The aim of our study was to understand the nature and the logic of the construction of auditory cognitive categories for natural sounds during development. We have developed an original approach based on a free sorting task (FST). Indeed, categorization is fundamental for structuring the world and cognitive skills related to, without having any need of the use of language. Our project explored the ability of children to structure their acoustic world, and to investigate how such structuration matures during normal development. We hypothesized that age affects the listening strategy and the category decision, as well as the number and the content of individual categories.DesignEighty-two French children (6-9 years), 20 teenagers (12-13 years), and 24 young adults participated in the study. Perception and categorization of everyday sounds was assessed based on a FST composed of 18 different sounds belonging to three a priori categories: non-linguistic human vocalizations, environmental sounds, and musical instruments.ResultsChildren listened to the sounds more times than older participants, built significantly more classes than adults, and used a different strategy of classification. We can thus conclude that there is an age effect on how the participants accomplished the task. Analysis of the auditory categorization performed by 6-year-old children showed that this age constitutes a pivotal stage, in agreement with the progressive change from a non-logical reasoning based mainly on perceptive representations to the logical reasoning used by older children. In conclusion, our results suggest that the processing of auditory object categorization develops through different stages, while the intrinsic basis of the classification of sounds is already present in childhood.

Project description:Insight problems are particularly interesting, because problems which require restructuring allow researchers to investigate the underpinnings of the Aha-experience, creativity and out of the box thinking. There is a need for new insight tasks to probe and extend the limits of existing theories and cognitive frameworks. To shed more light on this fascinating issue, we addressed the question: Is it possible to convey a well-known card sorting game into an insight task? We introduced different conditions and tested them via two online experiments (N = 546). Between the conditions we systematically varied the available perceptual features, and the existence of non-obvious rules. We found that our card sorting game elicited insight experience. In the first experiment, our data revealed that solution strategies and insight experience varied by the availability and saliency of perceptual features. The discovery of a non-obvious rule, which is not hinted at by perceptual features, was most difficult. With our new paradigm, we were able to construe ambiguous problems which allowed participants to find more than one solution strategy. Interestingly, we realized interindividual preferences for different strategies. The same problem drove strategies which either relied on feature integration or on more deliberate strategies. The second experiment varied the degree of independence of a sorting rule from the standard rules which were in accordance with prior knowledge. It was shown that the more independent the hidden rule was, the more difficult the task became. In sum, we demonstrated a new insight task which extended the available task domains and shed light on sequential and multi-step rule learning problems. Finally, we provided a first sketch of a cognitive model that should help to integrate the data within the existing literature on cognitive models and speculated about the generalizability of the interplay of prior knowledge modification and variation for problem solving.

Project description:BackgroundAdolescence represents a pivotal period for the initiation of smoking behaviours. While family, peer and social groups serve as significant reference groups influencing adolescents' decision-making process, there remains a paucity of research that examines how diversified reference groups influence their smoking decisions.ObjectiveThe objective is to compare normative and informative influences given by different reference groups on adolescents' smoking decision-making.DesignThis study applied reference group theory and used the card sorting technique. Adolescents were presented with 16 cards portraying various reference groups and 27 cards portraying different types of reference group influences. They were asked to construct sentences as smoking-elicited and smoking-inhibited cues and ranked these influences based on their relevance to their smoking decision-making.ParticipantsThe participants were recruited from four cities in the North, Central, South and East regions of Taiwan. The study sample consisted of 297 Taiwanese 11th graders drawn from high schools, vocational high schools and night schools.Primary outcome measuresThe most influential reference group among smokers, former smokers and never smokers.ResultsOur results of multivariable logistic regression show that for current smokers, having a smoking best friend (aOR=7.58, p<0.01), having a smoking colleague (aOR=3.83, p<0.01) and participating in ceremonial dance performances (aOR=4.62, p<0.01) are more likely to smoke. Peers play an important role in providing smoking-elicited cues for current smokers. Families provided the most smoking-inhibited cues for former and never smokers. Normative influences such as encouraging adolescent smoking provided more cues for adolescents than informative influences such as releasing stress by smoking.ConclusionsAdolescent smoking behaviour was influenced by peer norms, while former and non-smoking behaviours are associated with family norms. Therefore, tobacco prevention efforts should focus more on family-level interventions to discourage smoking initiation. Smoking cessation programmes for current smokers should address the reduction of peer pressure to smoke. Moreover, schools and community partners should collaborate to develop effective smoking cessation strategies, particularly for high-risk groups such as adolescents who involved in ceremonial dance performance.

Project description:Cognitive flexibility is a critical component of executive function and is strongly influenced by genetic factors. We conducted a genome-wide association study of cognitive flexibility (as measured by perseverative errors on the Wisconsin Card Sorting Test) in two sets of African American (AA) and European American (EA) subjects (Yale-Penn-1: 1,411 AAs/949 EAs; Yale-Penn-2: 1,178 AAs/1,335 EAs). We examined the association of cognitive flexibility with genotyped or imputed SNPs across the genome. In AAs, two correlated common single nucleotide polymorphisms (SNPs) (rs7165213/rs35633795) in the downstream region of the noncoding gene LOC101927286 on chromosome 15 showed genome-wide significant (GWS) associations with cognitive flexibility (Yale-Penn-1: p = 6.0 × 10-9 /1.3 × 10-8 ; Yale-Penn-2: p = .029/.010; meta-analysis: p = 4.2 × 10-7 /1.0 × 10-7 ) in the same effect direction. In EAs, no GWS associations were observed. Enriched gene sets identified by Data-driven Expression-Prioritized Integration for Complex Traits (DEPICT) analysis of the top SNPs (pmeta-analysis < 10-5 ) included the signalosome and ubiquitin-specific peptidase 9, X-linked (USP9X) subnetwork in AAs, and abnormal frontal and occipital bone morphology in EAs. We also performed polygenic risk score (PRS) analysis to examine the genetic correlation of cognition-proxy phenotypes (general cognitive function, education attainment, childhood intelligence, and infant head circumference) and cognitive flexibility in EAs. The PRS derived from general cognitive function-associated SNPs was significantly associated with cognitive flexibility. Nongenetic factors (age, education, sex, and tobacco recency) also exerted significant effects on cognitive flexibility. Our study demonstrates that both genetic and nongenetic factors impact cognitive flexibility, and variants in genes involved in protein degradation and brain development may contribute to population variation in cognitive function.
