Ondine's curse: myth meets reality.

ABSTRACT: Ondine's curse is one of the most enchanting mythical tales in the field of Medicine. The nymph Ondine was an immortal water spirit who became human after falling in love for a man, marrying him, and having a baby. In one of the versions of the tale, when she caught her husband sleeping with another woman, she cursed him to remain awake in order to control his own breathing. During the 19th century, the rare syndrome characterized by loss of autonomic breath control, while voluntary respiration remains intact, was cleverly named "Ondine's curse". Nowadays, the term Ondine's curse is usually associated with congenital central hypoventilation syndrome; however, in medical literature, it also designates several respiratory disorders. Here, we present a review of the myth focused on history, arts and medicine.

Global bioethics -- myth or reality?

Project description:BackgroundThere has been debate on whether a global or unified field of bioethics exists. If bioethics is a unified global field, or at the very least a closely shared way of thinking, then we should expect bioethicists to behave the same way in their academic activities anywhere in the world. This paper investigates whether there is a 'global bioethics' in the sense of a unified academic community.MethodsTo address this question, we study the web-linking patterns of bioethics institutions, the citation patterns of bioethics papers and the buying patterns of bioethics books.ResultsAll three analyses indicate that there are geographical and institutional differences in the academic behavior of bioethicists and bioethics institutions.ConclusionThese exploratory studies support the position that there is no unified global field of bioethics. This is a problem if the only reason is parochialism. But these regional differences are probably of less concern if one notices that bioethics comes in many not always mutually understandable dialects.

Genetic Basis of Myocarditis: Myth or Reality?

Project description:The genetic basis of myocarditis remains an intriguing concept, at least as long as the definition of myocarditis constitutes the definitive presence of myocardial inflammation sufficient to cause the observed ventricular dysfunction in the setting of cardiotropic infections. Autoimmune or immune-mediated myocardial inflammation constitutes a complex area of clinical interest, wherein numerous and not yet fully understood role of hereditary auto-inflammatory diseases can result in inflammation of the pericardium and myocardium. Finally, myocardial involvement in hereditary immunodeficiency diseases, cellular and humoral, is a possible trigger for infections which may complicate the diseases themselves. Whether the role of constitutional genetics can make the patient susceptible to myocardial inflammation remains yet to be explored.

Cellular Therapy in NSCLC: Between Myth and Reality.

Project description:Purpose of reviewIn this paper, we review the current state and modalities of adoptive cell therapies (ACT) in non-small cell lung carcinoma (NSCLC). We also discuss the challenges hampering the use of ACT and the approaches to overcome these barriers.Recent findingsSeveral trials are ongoing investigating the three main modalities of T cell-based ACT: tumor-infiltrating lymphocytes (TILs), genetically engineered T-cell receptors (TCRs), and chimeric antigen receptor (CAR) T cells. The latter, in particular, has revolutionized the treatment of hematologic malignancies. However, the efficacy against solid tumor is still sparse. Major limitations include the following: severe toxicities, restricted infiltration and activation within the tumors, antigen escape and heterogeneity, and manufacturing issues. ACT is a promising tool to improve the outcome of metastatic NSCLC, but significant translational and clinical research is needed to improve its application and expand the use in NSCLC.

Corticosteroids in Pediatric Heart Surgery: Myth or Reality.

Project description:Background: Corticosteroids have been administered prophylactically for more than 60 years in pediatric heart surgery, however, their use remains a matter of debate. There are three main indications for corticosteroid use in pediatric heart surgery with the use of cardiopulmonary bypass (CPB): (1) to blunt the systemic inflammatory response (SIRS) induced by the extracorporeal circuit; (2) to provide perioperative supplementation for presumed relative adrenal insufficiency; (3) for the presumed neuroprotective effect during deep hypothermic circulatory arrest operations. This review discusses the current evidence behind the use of corticosteroids in these three overlapping areas. Materials and Methods: We conducted a structured research of the literature using PubMed and MEDLINE databases to November 2017 and additional articles were identified by cross-referencing. Results: The evidence suggests that there is no correlation between the effect of corticosteroids on inflammation and their effect on clinical outcome. Due to the limitations of the available evidence, it remains unclear if corticosteroids have an impact on early post-operative outcomes or if there are any long-term effects. There is a limited understanding of the hypothalamic-pituitary-adrenal axis function during cardiac surgery in children. The neuroprotective effect of corticosteroids during deep hypothermic circulatory arrest surgery is controversial. Conclusions: The utility of steroid administration for pediatric heart surgery with the use of CPB remains a matter of debate. The effect on early and late outcomes requires clarification with a large multicenter randomized trial. More research into the understanding of the adrenal response to surgery in children and the effect of corticosteroids on brain injury is warranted.

Direct enzymatic bioelectrocatalysis: differentiating between myth and reality.

Project description:Enzymatic bioelectrocatalysis is being increasingly exploited to better understand oxidoreductase enzymes, to develop minimalistic yet specific biosensor platforms, and to develop alternative energy conversion devices and bioelectrosynthetic devices for the production of energy and/or important chemical commodities. In some cases, these enzymes are able to electronically communicate with an appropriately designed electrode surface without the requirement of an electron mediator to shuttle electrons between the enzyme and electrode. This phenomenon has been termed direct electron transfer or direct bioelectrocatalysis. While many thorough studies have extensively investigated this fascinating feat, it is sometimes difficult to differentiate desirable enzymatic bioelectrocatalysis from electrocatalysis deriving from inactivated enzyme that may have also released its catalytic cofactor. This article will review direct bioelectrocatalysis of several oxidoreductases, with an emphasis on experiments that provide support for direct bioelectrocatalysis versus denatured enzyme or dissociated cofactor. Finally, this review will conclude with a series of proposed control experiments that could be adopted to discern successful direct electronic communication of an enzyme from its denatured counterpart.

Searching for the perfect goalkeeping personality. Myth or reality?

Project description:BackgroundPsychological factors such as personality characteristics are influential factors of the goalkeeping performance in football (soccer). Not only for individualized treatment in practice, also from a scientific point of view, profiling goalkeepers is a relevant part of understanding athletes. The aim of this study was to investigate personality traits of goalkeepers of different expertise, age, and sex.MethodsUsing the Five Factor Model of personality we assessed personality traits of 132 male and female football goalkeepers ranging from youth to senior and low to elite level. A series of analysis investigated differences between the groups focusing on expertise, age, and sex.ResultsSignificant differences in the personality trait agreeableness between groups of different expertise and sex could be detected. Although a significant difference in neuroticism levels of males and females could be shown.ConclusionThis study was a first step of profiling football goalkeepers of different expertise, age, and sex. The study calls for more replication in this specific field of football and goalkeeping in general to understand the influence of personality characteristics on sport performance.

Personalised Medicine with IL-23 Blockers: Myth or Reality?

Project description:Background and aimsThe medical management of inflammatory bowel disease [IBD] has become increasingly targeted, through the identification of specific immune mediators involved in its pathogenesis. IL-23 is an inflammatory cytokine involved in both innate and adaptive immunity, which has been identified as a therapeutic target in Crohn's disease [CD] and ulcerative colitis [UC] through its upstream inhibition of the T helper 17 [Th17] pathway. We sought to review available data on the efficacy of IL-23 inhibitors in the treatment of IBD and the potential for clinical and molecular predictors of response to facilitate a personalised medicine approach with these agents.MethodsWe reviewed and summarised available clinical trial data on the use of the IL-23 inhibitors risankizumab, brazikumab, mirikizumab, and guselkumab in the treatment of IBD, as well as the evidence from studies of these agents in IBD and other immune-mediated conditions which might inform prediction of response to IL-23 inhibition.ResultsEarly clinical trials have demonstrated promising results following both induction and maintenance therapy with IL-23 inhibitors in CD and UC. Pre- and post-treatment levels of IL-22 and post-treatment levels of IL-17 have been identified as potential molecular predictors of response to therapy, in several studies. No significant clinical predictors of response have been identified thus far.ConclusionsIL-23 antagonism is a promising therapeutic approach in IBD. Further exploration of molecular and clinical predictors of response may identify patients most likely to benefit from these medications.

Reinstating verbal memories with virtual contexts: Myth or reality?

Project description:When learning new information, contextual information about the encoding situation is stored in addition to the focal memory content. Later, these strings of extra information can help retrieve the learned content as demonstrated by experiments where contextual cues from an encoding situation facilitate remembering and improve memory performance when reinstated during retrieval. This context-dependent memory effect has been investigated over the course of several decades and has been demonstrated with many different types of contexts. Based on this, the widely held belief is that context-dependent memory is a strong and robust effect, with transferable substance for everyday learning and potential clinical applications. Here we report the results of a multi-study design investigating the influence of reinstated visual contexts on memory performance. Data from 120 participants were included in three studies comprising a variety of visual cues. We show convincingly that even rich, salient and fully surrounding visual contexts provided by virtual reality are not sufficient to induce effects of context-dependency in a free recall memory task. We also investigated contextual modulation of oscillatory brain activity in order to test the effect of reinstated neural contexts, which failed to evoke a robust effect when re-tested in an internal conceptual replication study. Moreover, a Bayesian sequential statistical analysis revealed moderate to strong evidence against the hypothesis that reinstatement of visual contexts benefits free recall memory tasks indicating that effects are small and may not be suitable for transfer into everyday learning.
