Computed Tomography Radiomics Kinetics as Early Imaging Correlates of Osteoradionecrosis in Oropharyngeal Cancer Patients.

ABSTRACT: Osteoradionecrosis (ORN) is a major side-effect of radiation therapy in oropharyngeal cancer (OPC) patients. In this study, we demonstrate that early prediction of ORN is possible by analyzing the temporal evolution of mandibular subvolumes receiving radiation. For our analysis, we use computed tomography (CT) scans from 21 OPC patients treated with Intensity Modulated Radiation Therapy (IMRT) with subsequent radiographically-proven ≥ grade II ORN, at three different time points: pre-IMRT, 2-months, and 6-months post-IMRT. For each patient, radiomic features were extracted from a mandibular subvolume that developed ORN and a control subvolume that received the same dose but did not develop ORN. We used a Multivariate Functional Principal Component Analysis (MFPCA) approach to characterize the temporal trajectories of these features. The proposed MFPCA model performs the best at classifying ORN vs. Control subvolumes with an area under curve (AUC) = 0.74 [95% confidence interval (C.I.): 0.61-0.90], significantly outperforming existing approaches such as a pre-IMRT features model or a delta model based on changes at intermediate time points, i.e., at 2- and 6-month follow-up. This suggests that temporal trajectories of radiomics features derived from sequential pre- and post-RT CT scans can provide markers that are correlates of RT-induced mandibular injury, and consequently aid in earlier management of ORN.

Project description:PurposeThe aim of this study is to develop and compare performance of radiomics signatures using texture features extracted from noncontrast enhanced CT (NECT) and contrast enhanced CT (CECT) images for preoperative predicting risk categorization and clinical stage of thymomas.Materials and methodsBetween January 2010 and October 2018, 199 patients with surgical resection and histopathologically confirmed thymoma were enrolled in this retrospective study. We extracted 841 radiomics features separately from volume of interest (VOI) in NECT and CECT images. The features with poor reproducibility and highly redundancy were removed. Then a least absolute shrinkage and selection operator method (LASSO) logistic regression model with 10-fold cross validation was used for further feature selection and radiomics signatures build. The predictive performances of radiomics signatures were assessed by receiver operating characteristic (ROC) analysis. The areas under the receiver operating characteristic curve (AUC) between radiomics signatures were compared by using Delong test.ResultIn differentiating high risk thymomas from low risk thymomas, the AUC, sensitivity, and specificity were 0.801(95% CI 0.740-0.863), 0.752 and 0.767 for radiomics signature based on NECT images, and 0.827 (95% CI 0.771 -0.884), 0.798, and 0.722 for radiomics signature based on CECT images. But there was no significant difference (p=0.365) between them. In differentiating advanced stage thymomas from early stage thymomas, the AUC, sensitivity, and specificity were 0.829 (95%CI 0.757-0.900), 0.712, and 0.806 for radiomics signature based on NECT images and 0.860 (95%CI 0.803-0.917), 0.699, and 0.889 for radiomics signature based on CECT images. There was no significant difference (p=0.069) between them. The accuracy was 0.819 for radiomics signature based on NECT images, 0.869 for radiomics signature based on CECT images, and 0.779 for radiologists. Both radiomics signatures had a better performance than radiologists. But there was significant difference (p = 0.025) only between CECT radiomics signature and radiologists.ConclusionRadiomics signatures based on texture analysis from NECT and CECT images could be utilized as noninvasive biomarkers for differentiating high risk thymomas from low risk thymomas and advanced stage thymomas from early stage thymoma. As a quantitative method, radiomics signature can provide complementary diagnostic information and help to plan personalized treatment for patients with thymomas.

Project description:PurposeThe present study aimed to investigate the clinical prognostic significance of radiomics signature (R-signature) in patients with gastric neuroendocrine neoplasm (GNEN).Methods and materialsA retrospective study of 182 patients with GNEN who underwent dual-phase enhanced computed tomography (CT) scanning was conducted. LASSO-Cox regression analysis was used to screen the features and establish the arterial, venous and the arteriovenous phase combined R-signature, respectively. The association between the optimal R-signature with the best prognostic performance and overall survival (OS) was assessed in the training cohort and verified in the validation cohort. Univariate and multivariate Cox regression analysis were used to identify the significant factors of clinicopathological characteristics for OS. Furthermore, the performance of a combined radiomics-clinical nomogram integrating the R-signature and independent clinicopathological risk factors was evaluated.ResultsThe arteriovenous phase combined R-signature had the best performance in predicting OS, and its C-index value was better than the independent arterial and venous phase R-signature (0.803 vs 0.784 and 0.803 vs 0.756, P<0.001, respectively). The optimal R-signature was significantly associated with OS in the training cohort and validation cohort. GNEN patients could be successfully divided into high and low prognostic risk groups with radiomics score median. The combined radiomics-clinical nomogram combining this R-signature and independent clinicopathological risk factors (sex, age, treatment methods, T stage, N stage, M stage, tumor boundary, Ki67, CD56) exhibited significant prognostic superiority over clinical nomogram, R-signature alone, and traditional TNM staging system (C-index, 0.882 vs 0.861, 882 vs 0.803, and 0.882 vs 0.870 respectively, P<0.001). All calibration curves showed remarkable consistency between predicted and actual survival, and decision curve analysis verified the usefulness of the combined radiomics-clinical nomogram for clinical practice.ConclusionsThe R-signature could be used to stratify patients with GNEN into high and low risk groups. Furthermore, the combined radiomics-clinical nomogram provided better predictive accuracy than other predictive models and might aid clinicians with therapeutic decision-making and patient counseling.
