Despite the large number of data published in Italy on WEPs, there is no database providing a complete knowledge framework. Hence the need to design a database of the Italian alimurgic flora: AlimurgITA. Only strictly alimurgic taxa were chosen, excluding casual alien and cultivated ones. The collected data come from an archive of 358 texts (books and scientific articles) from 1918 to date, chosen with appropriate criteria. Project description:Introduction: Within a wider international research project aimed at operationalize the psychodynamic construct of capacity to love (Kernberg, 2011), the Capacity to Love Inventory (CTL-I) is a 41-items self-report questionnaire assessing six dimensions: interest in the life project of the other, basic trust, gratitude, common ego ideal, permanence of sexual passion, loss, and mourning. Objectives: The study is aimed at validating the Italian version of the CTL-I. Method: A total sample of 736 Italian non-clinical adults was administered a checklist assessing socio-demographic variables, and the CTL-I. A Confirmatory Factor Analyses (CFA) was conducted to examine the construct validity of the Italian version of the CTL-I. Only a part of the total sample (320 participants) was administered an additional series of concurrent measures in order to investigate the convergent validity of the CTL-I. Correlations with measures of socio-sexual orientation, quality of romance relations, and psychopathological questionnaires were examined through Pearson's correlation coefficients. Results: CFA results suggested that the Italian CTL-I fully replicated the six-factor structure of the original CTL-I. Cronbach's alpha index provided satisfactory results for all subscales and the correlations with concurrent measures were in expected direction. Conclusion: The results showed promising psychometric characteristics of the Italian version of CTL-I. Implications of the feasibility of the instrument in clinical and psychotherapeutic settings are discussed.

Project description:Background: The aging population has increased concerns about the affordability, quality, and nature of long-term care for older people, emphasizing the role of nursing homes. Unlike acute hospital and primary care, there is a lack of drug consumption data in long-term care to understand regional or national healthcare policies. Objectives: This study aimed to describe medication consumption by older adults and expenditure in Italian nursing homes (NHs). Methods: Data on drug consumption and costs from the administrative medicine informational flows that detect medicines packages supplied to patients in health facilities and NHs were used. Data on the characteristics of the healthcare residence were from the Italian Health Ministry. Records for the year 2019, selecting the nursing homes exclusively providing elderly or mixed (elderly and disabled) were used. Results: In 2019, the total expenditure on medicines in NHs amounted to 25.38 million euros, the average cost to 1.30 and the expenditure per bed to 436.18 euros. Cardiovascular drugs were the highest-consuming therapeutic class (177.0 defined daily doses-DDDs/100 days of NH stay; 22.2% of total) followed by drugs acting on the alimentary tract and metabolism (167.6% and 21.0%) and blood drugs (160.4% and 20.1%). The treatment of hypertension and heart failure was widely the most frequently used, with the consumption being driven mainly by furosemide and ramipril. Antiulcer drugs were used on average in more than half of the days of NH stay (58.5 DDDs/100 days of NH stay), representing a therapeutic category for which deprescribing initiatives are recommended. On average, almost all patients received a dose of benzodiazepines, antipsychotics and antidepressants (37.6, 35.9, and 17.7 DDDs/100 days of NH stay, respectively), confirming the high prevalence of use for these medicines. Antibiotics reached 6.8 DDDs/100 days of NH stay. Conclusion: The availability of data in this specific setting allows the identification of the main interventions toward improving appropriateness and represents a challenge for drug utilization research. Data from this study suggest that proton pump inhibitors (PPIs), benzodiazepines and antibacterials can be areas of improving prescribing appropriateness.

Project description:Backgroundin Occidental languages, no widely accepted questionnaire is available which deals with health related quality of life from the specific point of view of Traditional Chinese Medicine (TCM). Some psychometric tools of this kind are available in Chinese. One of them is the Chinese Quality of Life questionnaire (ChQoL). It comprises 50 items, subdivided in 3 Domains and 13 Facets. The ChQoL was built from scratch on the basis of TCM theory. It is therefore specifically valuable for the TCM practitioner. This paper describes our translation into Italian of the ChQoL, its first application to Occidental oncological patients, and some of its psychometric properties.Methodsa translation scheme, originally inspired by the TRAPD procedure, is developed. This scheme focuses on comprehensibility and clinical usefulness more than on linguistic issues alone. The translated questionnaire is tested on a sample of 203 consecutive female patients with breast cancer. Shapiro-Wilk normality tests, Fligner-Killeen median tests, exploratory Two-step Cluster Analysis, and Tukey's test for non-additivity are applied to study the outcomes.Resultsan Italian translation is proposed. It retains the TCM characteristics of the original ChQoL, it is intelligible to Occidental patients who have no previous knowledge of TCM, and it is useful for daily clinical practice. The score distribution is not Normal, and there are floor and ceiling effects. A Visual Analogue Scale is identified as a suitable choice. A 3-point Likert scale can also efficiently describe the data pattern. The original scales show non-additivity, but an Anscombe-Tukey transformation with γ = 1.5 recovers additivity at the Domain level. Additivity is enhanced if different γ are adopted for different Facets, except in one case.Conclusionsthe translated questionnaire can be adopted both as a filing system based on TCM and as a source of outcomes for clinical trials. A Visual Analogue Scale is recommended, but a simpler 3-point Likert scale also suitably fits data. When estimating missing data, and when grouping items within Domain in order to build a summary Domain index, an Anscombe-Tukey transformation should be applied to the raw scores.

Project description:BackgroundThe demographic changes in Brazil as a result of population aging is one of the fastest in the world. The far-reaching new challenges that come with a large older population are particularly disquieting in low- and middle-income countries (LMICs). Longitudinal studies must be completed in LMICs to investigate the social and biological determinants of aging and the consequences of such demographic changes in their context. Therefore, we designed the Prospective GERiatric Observational (ProGERO) study, a longitudinal study of outpatient older adults in São Paulo, Brazil, to collect data both on aging and chronic diseases, and investigate characteristics associated with adverse outcomes in this population.MethodsThe ProGERO study takes place in a geriatric outpatient clinic in the largest academic medical center in Latin America. We performed baseline health examinations in 2017 and will complete subsequent in-person visits every 3 years when new participants will also be recruited. We will use periodic telephone interviews to collect information on the outcomes of interest between in-person visits. The baseline evaluation included data on demographics, medical history, physical examination, and comprehensive geriatric assessment (CGA; including multimorbidity, medications, social support, functional status, cognition, depressive symptoms, nutritional status, pain assessment, frailty, gait speed, handgrip strength, and chair-stands test). We used a previously validated CGA-based model to rank participants according to mortality risk (low, medium, high). Our selected outcomes were falls, disability, health services utilization (emergency room visits and hospital admissions), institutionalization, and death. We will follow participants for at least 10 years.ResultsWe included 1336 participants with a mean age of 82 ± 8 years old. Overall, 70% were women, 31% were frail, and 43% had a Charlson comorbidity index score ≥ 3. According to our CGA-based model, the incidence of death in 1 year varied significantly across categories (low-risk = 0.6%; medium-risk = 7.4%; high-risk = 17.5%; P < 0.001).ConclusionThe ProGERO study will provide detailed clinical data and explore the late-life trajectories of outpatient older patients during a follow-up period of at least 10 years. Moreover, the study will substantially contribute to new information on the predictors of aging, senescence, and senility, particularly in frail and pre-frail outpatients from an LMIC city.

Project description:Kenya, an African country with major higher plant diversity, has a corresponding diversity of plant associations, because of the wide geographic distribution, diverse climatic conditions and soil types. In this article, all vascular plants of Kenya were counted based on the completed "Flora of Tropical East Africa (FTEA)", and all families and genera were revised using recent molecular systematics research, forming a "Synoptic List of Families and Genera of Kenyan Vascular Plants (SLFGKVP)". In total, there are 225 families, 1538 genera and 6293 indigenous species and and 62 families, 302 genera and 588 exotic species in Kenya. The Fabaceae with 98 genera and 576 Species is the largest family. Two of the seven plant distribution regions of Kenya, K4 and K7 are the most species-richest areas with regard to both total and endemic species, with 3375 and 3191 total species and 174 and 185 endemic species in K4 and K7 respectively. While, K3 and K5 have the highest density of both total and endemic species. K1 has the lowest density of total species, and K2 has the lowest density of endemic species.

Project description:ObjectiveThe research aims to investigate family communication regarding sexuality and the possible link between insecure attachment, violence in relationships, and the tendency toward sexual sensation-seeking in a sample of Italian sexual offenders.Design and methodWe evaluated 29 male sexual offenders in two correctional facilities of Southern Lazio (Italy) (mean age = 40.76; SD = 11.16). The participants completed general questions about their family and sexual education and fulfilled the following questionnaires: Compulsive Sexual Behavior Inventory (CSBI), Sexual Sensation-seeking Scale (SSSS), and the High-Risk Situation Checklist, adapted in Italian, as well as the Attachment Style Questionnaire (ASQ), validated in Italian.ResultsMost of the participants had never talked about sex within their family and perceived a severe or abusive education during childhood. In addition, positive correlations emerged between SSSS and the two scales of the CSBI, as well as between insecure attachment style, CSBI, and sexual sensation-seeking. The participants also reported some critical issues regarding the personal perception of high-risk situations linked to sexual relapse.ConclusionsThe data suggest factors to investigate, such as family education and relationships and the personal perception of sexual recidivism. The results might be effective in treatment and prevention programs among sex offenders.

Project description:BackgroundGiven the diagnostic accuracy of HPV-DNA tests in terms of self-collected samples, in order to implement self-sampling in cervical screening programs, the standardization of the pre-analytical phase, including decisions concerning the choice of medium, the volume of elution, and storage conditions, are necessary, in addition to understanding the potential factors involved in acceptability by women. On this basis, we carried out a cross-sectional study to assess (i) the stability of dry vaginal self-collected samples stored at room temperature for up to 4 weeks after elution in 2 mL of eNat® (Copan) medium, and (ii) the acceptability of self-collection in enrolled women.Methods185 women were enrolled in the LILT (Italian League Against Tumors) regional project. A self-sampling kit, including a dry FLOQSwab® (Copan), instructions for use, and a satisfaction questionnaire, were supplied for each woman and sent by mail to the laboratory. The HPV-DNA test was carried out using the Anyplex™ II HPV HR (Seegene) kit. To evaluate the specimen's stability, 185 dry vaginal swabs were eluted in eNat®, a lyses-based molecular medium and tested for HPV detection at two different time points (<6 days and 1 month after elution). The Cohen's Kappa coefficients and McNemar test were used to assess the agreement of HPV-DNA at different times.ResultsWe found high agreement in terms of HPV-DNA results among the samples tested at two different time points (Cohen K = 0.98; p < 0.0001). Moreover, most of the women found it easy to use self-collection devices and the pictorial instructions clear to understand. Approximately half of the enrolled women declared preferring self-sampling to clinician-collected methods.ConclusionOur results display the high reliability and accuracy of HPV-DNA tests using dry vaginal self-collection FLOQSwabs® devices eluted in 2 mL of molecular medium. The analysis of the questionnaire showed a high acceptability of self-collection among women, although a high percentage preferred standard collection devices. Overall, our preliminary results support the adoption of self-collection in screening programs, even though further analyses should be performed to optimize and standardize protocols for HPV tests on self-samples, and educational campaigns are needed to adequately inform and increase responsiveness in a target population.

Project description:Over- and undercoagulation with warfarin are associated with hemorrhagic and thromboembolic events, respectively. Genetic and clinical factors affect warfarin response, and the causes of this variability remain unclear. We present descriptive statistics and test for predictors of poor anticoagulation control. The Quebec Warfarin Cohort (QWC) comprises 1059 new warfarin users, with prospective follow-up using telephone questionnaires every 3 months for 1 year, and using healthcare administrative databases (RAMQ and Med-Echo) for 5 years prior to cohort entry and up to 10 years following active patient participation. Genetic material was collected, and genotyping of CYP2C9 and VKORC1 genes was conducted. Measured outcomes included the percentage of time patients spent within therapeutic range, anticoagulation control, warfarin dose, bleeding, and thromboembolic events. We report baseline characteristics and outcomes after 1 year of follow-up. Poor anticoagulation control was defined as time in therapeutic range <60% in the 3- to 12-month interval. Participants had a mean age of 71 years, and 62% were men. The most common indication for warfarin was atrial fibrillation (87%). Mean time in therapeutic range was 56% (±25%) in the 3 months following warfarin initiation, and 70% (±21%) in the 3- to 12-month interval. During follow-up, the rate of stroke or systemic embolism was 1.8 events per 100 person-years; for major bleeding events, 3.3 events per 100 person-years. Independent predictors of poor anticoagulation control were chronic kidney disease, heart failure, dyslipidemia, and age. The QWC represents a good research cohort to investigate clinical and genetic factors in a warfarin-anticoagulated population.

Project description:Our understanding of the richness and uniqueness of the flora growing on gypsum substrates in Italy has grown significantly since the 19th century and, even today, new plant species are still being discovered. However, the plants and plant communities, growing on gypsum substrates in Italy, are still a relatively unknown subject. The main aim of this paper was to elaborate a checklist of the Italian gypsophilous flora, to increase knowledge about this peculiar flora and for which conservation efforts need to be addressed. Through a structured group communication process of experts (application of the Delphi technique), a remarkable number of experienced Italian botanists have joined together to select focal plant species linked to gypsum substrates. From the results obtained, 31 plant species behave as absolute or preferent taxa (gypsophytes and gypsoclines) and form the 'core' Italian gypsophilous flora. The most abundant life forms were chamaephytes and hemicryptophytes, belonging to Poaceae and Brassicaceae; as for chorotypes, the most represented are Mediterranean and narrow endemics. By improving on previously available information about the flora with a clear preference for gypsum in Italy, this undertaking represents an important contribution to the knowledge of a habitat which is today considered a priority for conservation.

Project description:BackgroundRheumatic Heart Disease (RHD) remains a major cause of valvular heart disease related mortality and morbidity in low- and middle-income countries, with significant variation in characteristics and course of the disease across different regions. However, despite the high disease burden, there is sparse region-specific data on demographics, disease characteristics and course in treated and untreated patients to guide policy.MethodsThe ARGI database is a hospital-based registry in a tertiary referral national centre (Aswan Heart Centre, AHC) in which all patients with the diagnosis of RHD are being included. The mode of presentation, including baseline clinical and echocardiographic characteristics (as well as other imaging modalities), biomarkers and genetics are being documented. Treatment modalities and adherence to treatment is being recorded and patients are followed up regularly every 6 and/or 12 months, or more frequently if needed.DiscussionThis study shows for the first time an in-depth analysis of the severity and phenotype of disease in Egyptian patients presenting with RHD as well as the progression with time and provides a platform for further comparisons of regional differences in these details as well as their causes. The ARGI database will be of help in achieving the objectives of the Cairo Accord aiming at eradication of RF and RHD.
