Validity of EQ-5D-5L health-related quality of life questionnaire in self-reported diabetes: evidence from a general population survey.

ABSTRACT:

Background

This study aimed to assess the validity of the EQ-5D-5L in respondents with self-reported diabetes coming from a representative general population survey.

Methods

2974 respondents from the general adult population of Poland, chosen with multi-stage random sampling, were surveyed with HRQoL instruments (EQ-5D-5L, EQ VAS, SF-12, EQ-5D-3L) and a screening question about diabetes. To obtain EQ-5D index values, we used country-specific Polish value sets. We compared the instruments in terms of the ceiling effect, discriminatory power and frequency of individual health states. We evaluated construct validity in terms of known-groups validity and convergent validity of EQ-5D-5L dimensions and index values with other HRQoL measures.

Results

In respondents with diabetes (n = 247), the percentage reporting 'no problems' with EQ-5D-3L was reduced by 34.5% with the use of EQ-5D-5L (from 14.2% to 9.3%, respectively). A significant improvement in informativity was noticed in mobility and pain/discomfort dimensions (a relative increase of 23.1% and 22.7%, respectively). Known-groups construct validity analysis confirmed prior hypotheses-index scores were higher in the following groups: younger respondents, males, those taking no medication or oral antidiabetic drugs, and respondents with higher levels of education. The convergence between related EQ-5D-5L and EQ-5D-3L or SF-6D dimensions was stronger than between unrelated dimensions. The Bland-Altman analysis showed a mean difference between EQ-5D-5L and EQ-5D-3L, SF-6D, EQ VAS/100 index scores of 0.047, 0.165 and 0.231 respectively.

Conclusions

Our results support the validity of the EQ-5D-5L descriptive system and EQ-5D-5L index, based on the directly measured value set in respondents with self-reported diabetes coming from the general population.

Project description:IntroductionThe new, five-level EQ-5D generic questionnaire (EQ-5D-5L) has never been used among diabetes patients in Poland.ObjectivesTo develop health-related quality of life (HRQoL) norms for patients with self-reported diabetes, based on a large representative sample of the general Polish population, using the EQ-5D-5L.Materials and methodsMembers of the general public, selected via multistage stratified sampling, filled in the EQ-5D-5L questionnaire and answered a question about the presence of diabetes. We estimated three types of EQ-5D-5L outcomes: limitations within domains, EQ VAS and EQ-5D-5L index. Multiple linear regression was used to examine the relationship between sociodemographic characteristics and HRQoL, both in patients with diabetes and the general population sample.ResultsAmong 2,973 respondents having complete EQ-5D-5L data, 255 subjects (8.6%) self-reported diabetes. Treatment with insulin, other drugs, combination therapy or lack of drug treatment was declared by 22.0%, 48.6%, 5.1% and 24.3% of patients, respectively. Respondents with diabetes had a lower EQ VAS score (18.5 points difference on a 100-points scale) and a lower EQ-5D-5L index score (0.135 difference; scale range: 1.59). The multivariate analysis showed that the factors independently improving the HRQoL in the general population were secondary or higher education, and factors reducing HRQoL were female sex, belonging to an older age group, being treated because of diabetes with insulin, other drugs or combination treatment. Respondents diagnosed with diabetes but not treated with drugs showed a decrease in EQ VAS scores, but not in the EQ-5D-5L index.ConclusionsDiabetes leads to HRQoL deterioration in all age groups when compared to matched general population respondents without diabetes. The most significant HRQoL reduction experience older patients with a basic level of education. Obtained EQ-5D-5L normative data may be used in the clinical care of patients with diabetes and health technology assessment of new anti-diabetic drugs.

Project description:Background and Objectives: Despite the critical importance of effective healthcare management for patients with multimorbidity, robust and reliable tools for assessing health-related quality of life in Lithuania remain scarce. We aim to identify trends in the quality of life of patients with multimorbidity and to evaluate the effectiveness of the Lithuanian version of the EuroQol EQ-5D-5L questionnaire. Materials and Methods: The study included patients between the ages of 40 and 85 (N = 498) who had at least two chronic conditions, arterial hypertension being a prerequisite. The participants completed a comprehensive set of questionnaires specifically prepared for the TELELISPA "Improved healthcare quality for patients with multimorbidity in Lithuania" project which included the translated EQ-5D-5L questionnaire. The predictive validity of the EQ-5D-5L questionnaire was assessed using correlations with the SF-36 and EQ-VAS scores, a random forest regression model. Reliability was evaluated using Cronbach's alpha and inter-item correlations. Trends in the quality of life in different patient groups were assessed with Chi-square tests. Results: The EQ-5D-5L questionnaire demonstrated high reliability and validity with a Cronbach's alpha value of 0.737, EQ-5D-5L random forest machine learning regression model RMSE value of 0.1396, and adequate scores from other measures. Lower quality of life was found in patients with multimorbidity who had chronic conditions such as angina pectoris, heart failure, atrial fibrillation, or joint diseases, as well as the patients who were older than 60 years of age, women, or unemployed. Different aspects of quality of life were also significantly negatively impacted by diabetes, asthma, and chronic kidney disease. Heart failure, joint diseases, and older age had the biggest negative effect on quality of life. Conclusions: It is found that the Lithuanian EQ-5D-5L questionnaire is suitable for the assessment of the quality of life in patients with multimorbidity and indicates lower quality of life among those with specific cardiovascular and joint disorder chronic conditions and, in particular, demographic groups.

Project description:BackgroundThe EQ-5D-3L and EQ-5D-5L are two generic health-related quality of life measures, which may be used in clinical and health economic research. They measure impairment in 5 aspects of health: mobility, self-care, usual activities, pain/discomfort and anxiety/depression. The aim of this study was to assess the performance of the EQ-5D-3L and EQ-5D-5L in measuring the self-reported health status of older patients with substantial multimorbidity and associated polypharmacy.MethodsBetween 2017 and 2019, we administered EQ-5D-3L and EQ-5D-5L to a subset of patients participating in the OPERAM trial at 6 months and 12 months after enrolment. The OPERAM trial is a two-arm multinational cluster randomised controlled trial of structured medication review assisted by a software-based decision support system versus usual pharmaceutical care, for older people (aged ≥ 70 years) with multimorbidity and polypharmacy. In the psychometric analyses, we only included participants who completed the measures in full at 6 and 12 months. We assessed whether responses to the measures were consistent by assessing the proportion of EQ-5D-5L responses, which were 2 or more levels away from that person's EQ-5D-3L response. We also compared the measures in terms of informativity, and discriminant validity and responsiveness relative to the Barthel Index, which measures independence in activities of daily living.Results224 patients (mean age of 77 years; 56% male) were included in the psychometric analyses. Ceiling effects reported with the EQ-5D-5L (22%) were lower than with the EQ-5D-3L (29%). For the mobility item, the EQ-5D-5L demonstrated better informativity (Shannon's evenness index score of 0.86) than the EQ-5D-3L (Shannon's evenness index score of 0.69). Both the 3L and 5L versions of EQ-5D demonstrated good performance in terms of discriminant validity, i.e. (out of all items of the EQ-5D-3L and EQ-5D-5L, the pain/discomfort and anxiety/depression items had the weakest correlation with the Barthel Index. Both the 3L and 5L versions of EQ-5D demonstrated good responsiveness to changes in the Barthel Index.ConclusionBoth EQ-5D-3L and EQ-5D-5L demonstrated validity and responsiveness when administered to older adults with substantial multimorbidity and polypharmacy who were able to complete the measures.

Project description:BACKGROUND:The aim was to evaluate the impact of asthma on patients' Health-Related Quality of Life (HRQoL) by comparing asthmatic women and men with reference norms, to examine the factors which contributed to an impaired HRQoL, and to identify groups at higher risk. METHODS:Cross-sectional evaluation of 222 primary care patients with persistent asthma (18-40 years old). HRQoL impact was estimated with the EuroQol-5 Dimensions (EQ-5D), which allows calculating Quality-Adjusted Life-Years (QALYs) by applying society preferences. Participants self-completed the EQ-5D questionnaire online. Telephonic interviews collected information on medication and adherence, and administered the Asthma Control Questionnaire. Severity markers included asthma-related comorbidity, previous oral corticosteroids course prescription, and inhaled corticosteroids daily dose. After bivariate analyses, multiple linear regression models were constructed to examine the relations between HRQoL asthma impact and socio-demographic and clinical variables, using as dependent variable the deviation from general population-based EQ-5D reference norms. RESULTS:Deviation from the EQ-5D index norms was moderate in most age/gender groups (-0.1, which corresponds to 0.6 standard deviations), while it was large in women aged 18-24 years (-0.18, corresponding to 1.1 standard deviations). In regression models, a poor asthma control was the only factor independently associated to HRQoL impact in both women and men: β -0.18 (p<0.001) and -0.15 (p = 0.01) respectively. Translating these β coefficients to QALYs, they are interpretable as 66 fewer days of full health per year in women with uncontrolled asthma and 55 for men, compared with those with controlled asthma. CONCLUSION:Persistent asthma has a moderately negative HRQoL impact on patients of both genders, and the youngest women have been identified as a high risk group which merits further research. We identified asthma control as the major contributor to impaired HRQoL in patients, regardless of their gender, suggesting that asthma HRQoL impact could be alleviated by achieving a good control of symptoms.

Project description:BackgroundThe EQ-5D has been frequently used in national health surveys. This study is a head-to-head comparison to assess how expanding the number of levels from three (EQ-5D-3L) to five in the new EQ-5D-5L version has improved its distribution, discriminatory power, and validity in the general population.MethodsA representative sample (N = 7554) from the Catalan Health Interview Survey 2011-2012, aged ≥18, answered both EQ-5D versions, and we evaluated the response redistribution and inconsistencies between them. To assess validity of this redistribution, we calculated the mean of the Visual Analogue Scale (VAS), which measures perceived health. The discriminatory power was examined with Shannon Indices, calculated for each dimension separately. Spanish preference value sets were applied to obtain utility indices, examining their distribution with statistics of central tendency and dispersion. We estimated the proportion of individuals reporting the best health state in EQ-5D-5L and EQ-5D-3L within groups of specific chronic conditions and their VAS mean.ResultsA very small reduction in the percentage of individuals with the best health state was observed, from 61.8% in EQ-5D-3L to 60.8% in EQ-5D-5L. In contrast, a large proportion of individuals reporting extreme problems in the 3 L version moved to severe problems (level 4) in the 5 L version, particularly for pain/discomfort (75.5%) and anxiety/depression (66.4%). The average proportion of inconsistencies was 0.9%. The pattern of the perceived health VAS mean confirmed the hypothesis established a priori, supporting the validity of the observed redistribution. Shannon index showed that absolute informativity was higher in the 5 L version for all dimensions. The means (SD) of the Spanish EQ-5D-3L and EQ-5D-5L indices were 0.87 (0.25) and 0.89 (0.22). The proportion of individuals with the best health state within each specific chronic condition was very similar, regardless of the EQ-5D version (≤ 30% in half of the 28 chronic conditions).ConclusionAlthough the proportion of individuals with the best possible health state is still very high, our findings support that the increase of levels provided by the EQ-5D-5L contributed to the validity and discriminatory power of this new version to measure health in general population, as in the national health surveys.
