A case report of pregnancy in untreated alkaptonuria - Focus on urinary tissue remodelling markers.

ABSTRACT: A 34-year old woman with alkaptonuria had an elective pregnancy, during which she collected urine samples over the duration of her pregnancy until parturition. She had been attending the National Alkaptonuria Centre from the age of 31 years and continued to attend after delivery for a further three annual visits. Data from her NAC visits as well as urine samples collected during pregnancy were analysed. Urine CTX-1/urine creatinine, urine αCTX-I/ urine creatinine, urine CTX-II/ urine creatinine, and urine C3M/urine creatinine all showed a rapid increase early in pregnancy, returning to baseline before increasing in late pregnancy, indicating significant remodelling of bone, subchondral bone, cartilage and other organs and connective tissue rich in collagens I, II and III. The pattern of tissue remodelling in AKU pregnancy has been described for the very first time. Further research is needed to understand pregnancy in AKU.

Nitisinone Treatment Affects Biomarkers of Bone and Cartilage Remodelling in Alkaptonuria Patients.

Project description:Nitisinone has been approved for treatment of alkaptonuria (AKU). Non-invasive biomarkers of joint tissue remodelling could aid in understanding the molecular changes in AKU pathogenesis and how these can be affected by treatment. Serological and urinary biomarkers of type I collagen and II collagen in AKU were investigated in patients enrolled in the randomized SONIA 2 (NCT01916382) clinical study at baseline and yearly until the end of the study (Year 4). The trajectories of the biomarkers over time were observed. After treatment with nitisinone, the biomarkers of type I collagen remodelling increased at Year 1 (19% and 40% increase in CTX-I and PRO-C1, respectively), which was potentially reflected in the higher degree of mobility seen following treatment. The biomarkers of type II collagen remodelling decreased over time in the nitisinone group: C2M showed a 9.7% decline at Year 1, and levels then remained stable over the following visits; CTX-II showed a 26% decline at Year 3 and 4 in the nitisinone-treated patients. Nitisinone treatment induced changes in biomarkers of bone and cartilage remodelling. These biomarkers can aid patient management and deepen our knowledge of the molecular mechanisms of this rare disease.

Case Report of Untreated Pediatric Femoral Neck Fracture with Osteopenia.

Project description:Pediatric hip fractures are rare. This type of injury often requires surgery within 24 hours to prevent complications such as avascular necrosis, bone non-union, and premature physeal closure. Moreover, chronic disuse from untreated fracture causes severe osteopenia. Untreated pediatric hip fracture is uncommon in the developing world due to access to surgical care. Here, we report a pediatric patient who traveled from Honduras with untreated right femoral neck fracture. The delay in her initial surgical intervention left her with chronic non-union of her right hip joint and significant osteopenia of her right leg secondary to chronic disuse. This patient required a total right hip arthroplasty rather than the conventional internal fixation. It is crucial for emergency physicians to identify the causes of pediatric hip pain. Early detection of these pathologies allows for early orthopedics involvement and surgical intervention to avoid long-term complications.TopicsPediatric orthopedic, femoral neck fracture, osteopenia.

Chromatin remodelling comes into focus.

Project description:ATP-dependent chromatin remodelling enzymes are molecular machines that act to reconfigure the structure of nucleosomes. Until recently, little was known about the structure of these enzymes. Recent progress has revealed that their interaction with chromatin is dominated by ATPase domains that contact DNA at favoured locations on the nucleosome surface. Contacts with histones are limited but play important roles in modulating activity. The ATPase domains do not act in isolation but are flanked by diverse accessory domains and subunits. New structures indicate how these subunits are arranged in multi-subunit complexes providing a framework from which to understand how a common motor is applied to distinct functions.

Investigating the Robustness and Diagnostic Potential of Extracellular Matrix Remodelling Biomarkers in Alkaptonuria.

Project description:Background and aimAlkaptonuria (AKU) clinical manifestations resemble severe arthritis. The Suitability of Nitisinone in Alkaptonuria 1 (SONIA 1) study is a dose-finding trial for nitisinone treatment of AKU patients. We tested a panel of serum and urinary biomarkers reflecting extracellular matrix remodelling (ECMR) of cartilage, bone and connective tissue in SONIA 1 patients to identify non-invasive and diagnostic biomarkers of tissue turnover in AKU.MethodsFasted serum and urine were retrieved from 40 SONIA 1 patients and 44 healthy controls. Established biomarkers of bone remodelling (CTX-I, P1NP, OC), cartilage remodelling (CTX-II, C2M, AGNx1) and inflammation (CRPM) as well as exploratory biomarkers of ECMR (C6M, VCANM, MIM, TIM) were measured at baseline in serum and urine by means of enzyme-linked immunosorbent assays (ELISAs) or automated systems (Elecsys 2010).ResultsThe levels of bone resorption (CTX-I) and cartilage degradation (C2M) were elevated in AKU patients as compared to controls (p > 0.0001 and p = 0.03, respectively). Also tissue inflammation (CRPM) was elevated in AKU patients (p = 0.01). In addition all four exploratory biomarkers of ECMR (C6M, VCANM, MIM, TIM) were elevated in AKU patients compared to healthy controls. CTX-II was the only biomarker to be reduced in AKU patients. TIM was the only marker that showed a higher concentration than the normal assay range in AKU patients.ConclusionsWe have identified new potential biomarkers for assessment of cartilage, bone and cardiovascular remodelling in AKU and demonstrated the robustness of the assays used to measure the biomarker concentration in biological fluids.

Multiple chromoanasynthesis in a rare case of sporadic renal leiomyosarcoma: case report

Project description:Renal leiomyosarcomas (LMS) are extremely rare neoplasms with aggressive behaviour and poor survival prognosis. The most frequent somatic events in leiomyosarcomas are mutations in TP53, RB1, ATRX and PTEN genes, chromosomal instability and chromoanagenesis. By using chromosomal microarray analysis we identified monosomy of chromosomes 3 and 11, gain of Xp (ATRX) arm and three chromoanasynthesis regions (6q21-q27, 7p22.3-p12.1 and 12q13.11-q21.2), with MDM2 and CDK4 oncogenes copy number gains, whereas no CNVs or tumor specific SNVs in TP53, RB1 and PTEN genes were observed.

Brucellosis in pregnancy: a case report.

Project description:BackgroundBrucella spp., a gram-negative bacterium, is one of the most prevalent zoonotic illnesses worldwide and is more commonly seen in animals; however, the disease may be present in humans. Clinical manifestations of brucellosis are variable and can range from asymptomatic to severe disease. In women who are pregnant, potential obstetrics complications are possible. The purpose of this report is to present a case of brucellosis in a pregnant patient and discuss the potential complications and treatment recommendations.Case descriptionWe present a case of a 17-year-old gravida 2, parity 1 (G2P1) at 35 weeks and 2 days (35w2d) with brucellosis after she assisted in the delivery of puppies. All puppies were stillborn and the dog was confirmed positive for B. canis on serological testing. Our patient was also found to have B. canis, which is a particularly rare cause of human brucellosis. She was treated with ceftriaxone, rifampin, and gentamicin before delivery and switched to doxycycline and ceftriaxone postpartum until negative. The patient was instructed not to breastfeed. After treatment, the patient felt well and the baby was healthy.ConclusionsWhen evaluating patients, it is necessary to obtain a social history including animal exposures to rule in or rule out zoonotic infections such as brucellosis. It is important to establish a suspected brucellosis infection in pregnancy and begin antibiotic treatment as soon as possible to prevent maternal and fetal complications. The treatment regimen we administered was an effective strategy, particularly for pregnancy, when typical treatments are contraindicated.
