Project description:The performance of a newly developed, high resolution, microangiographic fluoroscope (MAF) (35 μm pixel pitch and 300 μm thick CsI phosphor) was evaluated using a generalized linear system analysis and compared with that of a standard amorphous Si thin film transistor flat panel detector (FPD) (194 μm pixel pitch and 600 μm thick CsI phosphor). The linear system metrics such as modulation transfer function (MTF), noise power spectrum, and detection quantum efficiency (DQE) are commonly used to gauge the intrinsic detector performance in the detector plane. However, these linear system metrics do not provide information about the image receptor performance in a real system since they do not include the effects of other parameters such as focal spot distribution, scatter radiation, and geometric unsharpness, which may compromise detector performance characteristics. Use of generalized linear system metrics [generalized modulation transfer function (GMTF), generalized normalized noise power spectrum (GNNPS), and generalized detection quantum efficiency (GDQE)] that include these effects gives a more meaningful, complete, and appropriate evaluation of detector performance as part of the imaging system.A uniform head equivalent phantom was used to simulate realistic clinical parameters and x-ray spectra. The detector MTFs were measured using the slanted edge method and the focal spot MTFs were measured using a pinhole assembly. The scatter MTF was simulated and the scatter fraction was measured for a head-equivalent phantom. The generalized system metrics were calculated for different combinations of three choices of focal spots and three different magnifications with two different air-gaps. The performance of the MAF was also illustrated using stent images obtained with different focal spots under similar conditions.Results for the generalized metrics provide a quantitative description of the performance of the imaging system for both detectors. This generalized analysis demonstrated that both detectors have similar imaging capabilities at lower spatial frequencies, but that the MAF has superior performance over the FPD at higher frequencies even when considering focal spot blurring and scatter.This generalized performance analysis demonstrates the significance of focal spot size, magnification, and scatter on the system performance metrics (GMTF, GNNPS, and GDQE). Although the ideal detector performance characteristics of the MAF are not fully realized due to these other system factors, it still retains an advantage in DQE at high spatial frequencies over the FPD. Similar studies based on the generalized linear system metrics can serve as an efficient tool to evaluate total system capabilities under different realistic conditions to enable optimal design for specific imaging tasks.

Project description:Background(a) To evaluate the clinical performance of endocrine analytes using the sigma metrics (σ) model. (b) To redesign quality control strategies for performance improvement.MethodsThe sigma values of the analytes were initially evaluated based on the allowable total error (TEa), bias, and coefficient of variation (CV) at QC materials level 1 and 2 in March 2018. And then, the normalized QC performance decision charts, personalized QC rules, quality goal index (QGI) analysis, and root causes analysis (RCA) were performed based on the sigma values of the analytes. Finally, the sigma values were re-evaluated in September 2018 after a series of targeted corrective actions.ResultsBased on the initial sigma values, two analytes (FT3 and TSH) with σ > 6, only needed one QC rule (13S ) with N2 and R500 for QC management. On the other hand, seven analytes (FT4, TT4, CROT, E2, PRL, TESTO, and INS) with σ < 4 at one QC material level or both needed multiple rules (13S /22S /R4S /41S /10X ) with N6 and R10-500 depending on different sigma values for QC management. Subsequently, detailed and comprehensive RCA and timely corrective actions were performed on all the analytes base on the QGI analysis. Compared with the initial sigma values, the re-evaluated sigma metrics of all the analytes increased significantly.ConclusionsIt was demonstrated that the combination of sigma metrics, QGI analysis, and RCA provided a useful evaluation system for the analytical performance of endocrine analytes.

Project description:ImportanceSurvivors of preterm birth often present with medical morbidities; however, variation in their long-term educational performance has not been well described.ObjectiveTo estimate the association between gestational age and 4 outcomes in school-aged children: readiness to enter kindergarten, scores on standardized tests in elementary and middle school, gifted status, and low performance.Design, setting, and participantsIn a retrospective cohort study, children born in Florida between 1992 and 2002 at 23 to 41 weeks' gestation who entered Florida's public schools between 1995 and 2012 were assessed for kindergarten readiness and tested in mathematics and reading in grades 3 through 8. Data analysis was performed from January 12, 2016, to March 1, 2017.ExposuresGestational age at birth.Main outcomes and measuresKindergarten readiness, scores on the Florida Comprehensive Achievement Test (FCAT), classified as gifted, and classified as low performance.ResultsA total of 1 527 113 singleton infants with gestational ages of 23 to 41 weeks born between 1992 and 2002 were matched to Florida public school records. Of these, 1 301 497 children were included in the analysis; 641 479 (49.3%) were girls. A total of 301 (65.0%) Florida children born at 23 to 24 weeks' gestation were designated as ready to start kindergarten. When the FCAT test scores were adjusted for potentially confounding maternal and infant variables, children born at 23 to 24 weeks' gestation performed 0.66 SD (95% CI, -0.73 to -0.59) lower compared with those born at full term. A total of 123 554 (9.5%) of all Florida-born public school students were considered gifted, including 17 (1.8%) of those born at 23 to 24 weeks' gestation. In comparison, 75 458 (5.8%) of all Florida-born public school students were low performing; 310 (33.5%) of these children had been born at 23 to 24 weeks' gestation. Kindergarten readiness, FCAT scores, and gifted status were positively related to gestational age, whereas low performance was inversely related to gestational age.Conclusions and relevanceAlthough gestational age has long been associated with poor educational performance, a sufficient proportion of children born near the limits of viability performed within expected school norms, warranting further investigation into how and why certain children are able to overcome the educational burdens that may follow preterm birth.

Project description:Forest understory vegetation is an important characteristic of the forest. Predicting and mapping understory is a critical need for forest management and conservation planning, but it has proved difficult with available methods to date. LiDAR has the potential to generate remotely sensed forest understory structure data, but this potential has yet to be fully validated. Our objective was to examine the capacity of LiDAR point cloud data to predict forest understory cover. We modeled ground-based observations of understory structure in three vertical strata (0.5 m to < 1.5 m, 1.5 m to < 2.5 m, 2.5 m to < 3.5 m) as a function of a variety of LiDAR metrics using both mixed-effects and Random Forest models. We compared four understory LiDAR metrics designed to control for the spatial heterogeneity of sampling density. The four metrics were highly correlated and they all produced high values of variance explained in mixed-effects models. The top-ranked model used a voxel-based understory metric along with vertical stratum (Akaike weight = 1, explained variance = 87%, cross-validation error = 15.6%). We found evidence of occlusion of LiDAR pulses in the lowest stratum but no evidence that the occlusion influenced the predictability of understory structure. The Random Forest model results were consistent with those of the mixed-effects models, in that all four understory LiDAR metrics were identified as important, along with vertical stratum. The Random Forest model explained 74.4% of the variance, but had a lower cross-validation error of 12.9%. We conclude that the best approach to predict understory structure is using the mixed-effects model with the voxel-based understory LiDAR metric along with vertical stratum, because it yielded the highest explained variance with the fewest number of variables. However, results show that other understory LiDAR metrics (fractional cover, normalized cover and leaf area density) would still be effective in mixed-effects and Random Forest modelling approaches.

Project description:Technical evaluation of swimming performance is an essential factor in preparing elite swimmers for their competitions. Inertial measurement units (IMUs) have attracted much attention recently because they can provide coaches with a detailed analysis of swimmers' performance during training. A coach can obtain a quantitative and objective evaluation from IMU. The purpose of this study was to validate the use of a new phase-based performance assessment with a single IMU worn on the sacrum during training sessions. Sixteen competitive swimmers performed five one-way front crawl trials at their maximum speed wearing an IMU on the sacrum. The coach recorded the lap time for each trial, as it remains the gold standard for swimmer's performance in competition. The measurement was carried out once a week for 10 consecutive weeks to monitor the improvement in the swimmers' performance. Meaningful progress was defined as a time decrease of at least 0.5 s over a 25 m lap. Using validated algorithms, we estimated five goal metrics from the IMU signals representing the swimmer's performance in the swimming phases (wall push-off, glide, stroke preparation, free-swimming) and in the entire lap. The results showed that the goal metrics for free-swimming phase and the entire lap predicted the swimmer's progress well (e.g., accuracy, precision, sensitivity, and specificity of 0.91, 0.89, 0.94, and 0.95 for the lap goal metric, respectively). As the goal metrics for initial phases (wall push-off, glide, stroke preparation) achieved high precision and specificity (≥0.79) in progress detection, the coach can use them for swimmers with satisfactory free-swimming phase performance and make further improvements in initial phases. Changes in the values of the goal metrics have been shown to be correlated with changes in lap time when there is meaningful progress. The results of this study show that goal metrics provided by the phase-based performance evaluation with a single IMU can help monitoring swimming progress. Average velocity of the lap can replace traditional lap time measurement, while phase-based goal metrics provide more information about the swimmer's performance in each phase. This evaluation can help the coach quantitatively monitor the swimmer's performance and train them more efficiently.

Project description:Accurate measurement of malaria parasite clearance rates (CRs) following artemisinin (ART) treatment is critical for resistance surveillance and research, and various CR metrics are currently used. We measured 13 CR metrics in 1472 ART-treated hyperparasitemia infections for which 6-hour parasite counts and parasite genotypes (93 single nucleotide polymorphisms [SNPs]) were available. We used heritability to evaluate the performance of each metric. Heritability ranged from 0.06 ± 0.06 (SD) for 50% parasite clearance times to 0.67 ± 0.04 (SD) for clearance half-lives estimated from 6-hour parasite counts. These results identify the measures that should be avoided and show that reliable clearance measures can be obtained with abbreviated monitoring protocols.

Project description:BackgroundSpurred by the Coronavirus infectious disease 2019 pandemic, aerosol containment devices (ACDs) were developed to capture infectious respiratory aerosols generated by patients at their source. Prior reviews indicated that such devices had low evidence of effectiveness, but did not address how ACDs should be evaluated, how well they should perform, nor have clearly defined performance standards. Towards developing design criteria for ACDs, two questions were posed: 1) What characteristics have guided the design of ACDs? 2) How have these characteristics been evaluated?MethodsA scoping review was performed consistent with PRISMA guidelines. Data were extracted with respect to general study information, intended use of the device, device design characteristics and evaluation.ResultsFifty-four articles were included. Evaluation was most commonly performed with respect to device aerosol containment (n = 31, 61%), with only 5 (9%), 3 (6%) and 8 (15%) formally assessing providing experience, patient experience and procedure impact, respectively. Nearly all of the studies that explored provider experience and procedure impact studied intubation. Few studies provided a priori performance criteria for any evaluation metric, or referenced any external guidelines by which to bench mark performance.ConclusionWith respect to aerosol containment, ACDs should reduce exposure among HCP with the device compared with the absence of the device, and provide ≥90% reduction in respirable aerosols, equivalent in performance to N95 filtering facepiece respirators, if the goal is to reduce reliance on personal protective equipment. The ACD should not increase awkward or uncomfortable postures, or adversely impact biomechanics of the procedure itself as this could have implications for procedure outcomes. A variety of standardized instruments exist to assess the experience of patients and healthcare personnel. Integration of ACDs into routine clinical practice requires rigorous studies of aerosol containment and the user experience.

Project description:OBJECTIVES:The aim of the study was to evaluate acid and nonacid gastroesophageal reflux in infants and school-aged children with esophageal atresia (EA) using pH-impedance (pH-MII) monitoring. METHODS:Between 2012 and 2017, all 24-hour pH-MII studies performed in infants (≤18 months) and 8-year olds with EA were included. Antiacid therapy was discontinued before study. Exclusion criteria were: isolated tracheoesophageal fistula; esophageal replacement therapy; tube feeding; and monitoring <18 hours. Automatically detected retrograde bolus movements (RBM) were manually reviewed and modified/deleted if necessary. RESULTS:We included 57 children (51% boys; 2% isolated EA; 44% thoracoscopic EA repair): 24 infants (median age 0.6 years) and 33 school-aged children (median age 8.2 years). Of the automatically detected 3313 RBM, 1292 were manually deleted from the tracings: 52% of nonacid RBM and 8% of acid RBM (mainly misinterpreted swallows or 1 event recognized as several events). In infants, median reflux index (RI; pH <4) was 2.6% (abnormal in n = 2), median RBM was 61 (62% nonacid, 58% mixed), and median of the mean BCT was 11 seconds. In older children, median RI was 0.3% (abnormal in n = 4), median RBM was 21 (64% nonacid; 75% mixed), and median of the mean BCT was 13 seconds. CONCLUSIONS:Most children with EA off medication have a normal RI, yet experience a significant number of nonacid RBM. After manual revision of the tracings, a high percentage of RBM was deleted. Our data show that automated impedance analysis software needs refinement for use in infants and children with EA and question the need for standard antiacid therapy in these patients.

Project description:This study aims to build a comprehensive dataset that enables the automatic evaluation of piano performances. In real-world piano performance, especially within the realm of classical piano music, we encounter a vast spectrum of performance variations. The challenge lies in how to effectively evaluate these performances. We must consider three critical aspects: (1) It is essential to gauge how performers express the music and how listeners perceive the performance, rather than focusing on the compositional characteristics of the musical piece. (2) Beyond fundamental elements like pitch and duration, we must also embrace higher-level features such as interpretation. (3) Such evaluation should be done by experts to discern the nuanced performances. Regrettably, there exists no dataset that addresses these challenging evaluation tasks. Therefore, we introduce a pioneering dataset PercePiano, annotated by music experts, with more extensive features capable of representing these nuanced aspects effectively. It encapsulates piano performance with a wide range of perceptual features that are recognized by musicians. Our evaluation benchmark includes a novel metric designed to accommodate the inherent subjectivity of perception. Furthermore, we propose an enhanced baseline framework that grounds performance on score data, aligning model predictions with human perception. Harnessing the aligned features enhances the baseline performance and proves to be adaptable to various model structures. In conclusion, our research opens new possibilities for comprehensive piano performance evaluation.

Project description:IntroductionDespite its high prevalence and significance, there is still no widely available method to quantify cough. In order to demonstrate agreement with the current gold standard of human annotation, emerging automated techniques require a robust, reproducible approach to annotation. We describe the extent to which a human annotator of cough sounds (a) agrees with herself (intralabeller or intrarater agreement) and (b) agrees with other independent labellers (interlabeller or inter-rater agreement); we go on to describe significant sex differences in cough sound length and epochs size.Materials and methods24 participants wore an audiorecording smartwatch to capture 6-24 hours of continuous audio. A randomly selected sample of the whole audio was labelled twice by an expert annotator and a third time by six trained annotators. We collected 400 hours of audio and analysed 40 hours. The cough counts as well as cough seconds (any 1 s of time containing at least one cough) from different annotators were compared and summary statistics from linear and Bland-Altman analyses were used to quantify intraobserver and interobserver agreement.ResultsThere was excellent intralabeller (less than two disagreements per hour monitored, Pearson's correlation 0.98) and interlabeller agreement (Pearson's correlation 0.96), using cough seconds as the unit of analysis decreased annotator discrepancies by 50% in comparison to coughs. Within this data set, it was observed that the length of cough sounds and epoch size (number of coughs per bout or attach) differed between women and men.ConclusionGiven the decreased interobserver variability in annotation when using cough seconds (vs just coughs) we propose their use for manually annotating cough when assessing of the performance of automatic cough monitoring systems. The differences in cough sound length and epochs size may have important implications for equality in the development of cough monitoring tools.Trial registration numberNCT05042063.
