Hourly fetal urine production rate in isolated oligohydramnios at term.

ABSTRACT:

Objective

The aim of this study was to evaluate the hourly fetal urine production rate (HFUPR) via three-dimensional ultrasonography in women with isolated oligohydramnios and compare with normal pregnant women at term.

Materials and methods

This was a prospective observational cohort study of 112 women from 34 to 40 6/7 weeks' gestation. They were classified into three groups according to the amniotic fluid index (AFI) and ultrasonographic estimated fetal weight (EFW) as isolated oligohydramnios (defined as AFI below 5% and appropriate EFW corresponding to gestational age) (n = 34) and IUGR (defined as EFW below 5% corresponding to gestational age irrespective amniotic fluid) (n = 17), and normal pregnancy (n = 61). HFUPR was measured using three-dimensional virtual organ computer-aided analysis. Adverse perinatal outcomes in all participants were examined.

Results

There was no significant difference in HFUPR between patients with isolated oligohydramnios and women with normal pregnancies (median, 40.0 mL/h [interquartile range [IQR] 31.0-66.5] vs. 48.6 [31.5-81.2], p = 0.224). HFUPR was significantly decreased in the IUGR group (13.8 mL/h [IQR 10.1-24.8]), compared to the normal pregnancy group (p<0.001) and the isolated oligohydramnios group (p<0.001). HFUPR was significantly decreased in neonates with adverse perinatal outcomes compared to the control (24.7 mL/h [IQR 13.4-47.4] vs. 43.6 [29.8-79.0], p = 0.016).

Conclusion

HFUPR was not decreased in patients with isolated oligohydramnios but was decreased in patients with IUGR when compared to normal controls at term.

Project description:BackgroundThe multiple causes of oligohydramnios make it challenging to study. Long noncoding RNAs (lncRNAs) are sets of RNAs that have been proven to function in multiple biological processes. The purpose of this study is to study expression level and possible role of lncRNAs in oligohydramnios.MethodsIn this study, total RNA was isolated from fetal membranes resected from oligohydramnios pregnant women (OP) and normal amount of amniotic fluid pregnant women (Normal). LncRNA microarray was used to analyze the differentially expressed lncRNAs and mRNAs. Kyoto Encyclopedia of Genes and Genomes (KEGG) was used to analyze the main enrichment pathways of differentially expressed mRNAs. Real-time quantitative PCR (qPCR) was used to validate the lncRNA expression level.ResultsLncRNA microarray analysis revealed that a total of 801 lncRNAs and 367 mRNAs were differentially expressed in OP; in these results, 638 lncRNAs and 189 mRNAs were upregulated, and 163 lncRNAs and 178 mRNAs were downregulated. Of the lncRNAs, 566 were intergenic lncRNAs, 351 were intronic antisense lncRNAs, and 300 were natural antisense lncRNAs. The differentially expressed lncRNAs were primarily located in chromosomes 2, 1, and 11. KEGG enrichment pathways revealed that the differentially expressed mRNAs were enriched in focal adhesion as well as in the signaling pathways of Ras, tumor necrosis factor (TNF), estrogen, and chemokine. The qPCR results confirmed that LINC00515 and RP11-388P9.2 were upregulated in OP. Furthermore, the constructed lncRNA-miRNA-mRNA regulatory network revealed tenascin R (TNR), cystic fibrosis transmembrane conductance regulator (CFTR), ATP-binding cassette sub-family A member 12 (ABCA12), and collagen 9A2 (COL9A2) as the candidate targets of LINC00515 and RP11-388P9.2.ConclusionsIn summary, we revealed the profiles of lncRNA and mRNA in OP. These results might offer potential targets for biological prevention for pregnant women with oligohydramnios detected before delivery and provided a reliable basis for clinical biological treatment in OP.

Project description:BackgroundOligohydramnios is a condition of abnormally low amniotic fluid volume that has been associated with poor pregnancy outcomes. To date, the prevalence of this condition and its outcomes has not been well described in low and low-middle income countries (LMIC) where ultrasound use to diagnose this condition in pregnancy is limited. As part of a prospective trial of ultrasound at antenatal care in LMICs, we sought to evaluate the incidence of and the adverse maternal, fetal and neonatal outcomes associated with oligohydramnios.MethodsWe included data in this report from all pregnant women in community settings in Guatemala, Pakistan, Zambia and the Democratic Republic of Congo (DRC) who received a third trimester ultrasound as part of the First Look Study, a randomized trial to assess the value of ultrasound at antenatal care. Using these data, we conducted a planned secondary analysis to compare pregnancy outcomes of women with to those without oligohydramnios. Oligohydramnios was defined as measurement of an Amniotic Fluid Index less than 5 cm in at least one ultrasound in the third trimester. The outcomes assessed included maternal morbidity and fetal and neonatal mortality, preterm birth and low-birthweight. We used pairwise site comparisons with Tukey-Kramer adjustment and multivariable logistic models using general estimating equations to account for the correlation of outcomes within cluster.ResultsOf 12,940 women enrolled in the clusters in Guatemala, Pakistan, Zambia and the DRC in the First Look Study who had a third trimester ultrasound examination, 87 women were diagnosed with oligohydramnios, equivalent to 0.7% of those studied. Prevalence of detected oligohydramnios varied among study sites; from the lowest of 0.2% in Zambia and the DRC to the highest of 1.5% in Pakistan. Women diagnosed with oligohydramnios had higher rates of hemorrhage, fetal malposition, and cesarean delivery than women without oligohydramnios. We also found unfavorable fetal and neonatal outcomes associated with oligohydramnios including stillbirths (OR 5.16, 95%CI 2.07, 12.85), neonatal deaths < 28 days (OR 3.18, 95% CI 1.18, 8.57), low birth weight (OR 2.10, 95% CI 1.44, 3.07) and preterm births (OR 2.73, 95%CI 1.76, 4.23). The mean birth weight was 162 g less (95% CI -288.6, - 35.9) with oligohydramnios.ConclusionsOligohydramnos was associated with worse neonatal, fetal and maternal outcomes in LMIC. Further research is needed to assess effective interventions to diagnose and ultimately to reduce poor outcomes in these settings.Trial registrationNCT01990625.

Project description:BackgroundContinuous intrapartum fetal monitoring is challenging and its clinical benefits are debated. The project evaluated whether short-term-variation (STV) and other computerised fetal heart rate (FHR) parameters (baseline FHR, long-term-variation, accelerations and decelerations) predicted acidaemia at birth. The aims of the study were to assess the changes in FHR pattern during labour and determine the feasibility of undertaking a definitive trial by reporting the practicalities of using the monitoring device, participant recruitment, data collection and staff training.Methods200 high-risk women carrying a term singleton, non-anomalous fetus, requiring continuous FHR monitoring in labour were consented to participate from the Jessop Wing maternity unit, Sheffield, UK. The trans-abdominal fetal ECG monitor was placed as per clinical protocol. During the monitoring session, clinicians were blinded to the computerised FHR parameters. We analysed the last hour of the FHR and its ability to predict umbilical arterial blood pH <7.20 using receiver operator characteristics (ROC) curves.ResultsOf 200 women, 137 cases were excluded as either the monitor did not work from the onset of labour (n = 30), clinical staff did not return or used the monitor on another patient (n = 37), umbilical cord blood not obtained (n = 25), FHR data not recorded within an hour of birth (n = 34) and other reasons (n = 11). In 63 cases included in the final analysis, the computer-derived FHR parameters did not show significant correlation with umbilical artery cord pH <7.20. Labour was associated with a significant increase in short and long term variation of FHR and number of deceleration (P<0.001). However, baseline FHR decreased significantly before delivery (P<0.001).ConclusionsThe project encountered a number of challenges, with learning points crucial to informing the design of a large study to evaluate the potential place of intrapartum computerised FHR parameters, using abdominal fetal ECG monitor before its clinical utility and more widespread adoption can be ascertained.

Project description:ObjectivesTo assess the effectiveness of the Dawes-Redman algorithm in identifying fetal wellbeing at term by analyzing 30 years of retrospective clinical data, comparing normal and adverse pregnancy outcomes, evaluating key metrics and testing its performance when used 0-48 h before delivery.MethodsAntepartum fetal heart rate (FHR) traces from term singleton pregnancies at 37 + 0 to 41 + 6 weeks' gestation obtained between 1991 and 2024 were extracted from the Oxford University Hospitals database. Traces with > 30% of their signal information missing or with incomplete Dawes-Redman analyses were excluded. Only traces performed within 48 h prior to delivery were considered. A cohort of pregnancies with subsequent normal pregnancy outcome (NPO) was established using rigorous inclusion and exclusion criteria. Another cohort of pregnancies with adverse pregnancy outcome (APO) was developed if the neonate experienced at least one of seven APOs after delivery. Propensity score matching (PSM) facilitated a balanced comparison between NPO and APO cohorts using six factors: gestational age at FHR monitoring, fetal sex, maternal body mass index at presentation, maternal age at delivery, parity and time interval between FHR trace and delivery. FHR traces were categorized as either 'criteria met' (indicating fetal wellbeing) or 'criteria not met' (indicating a need for further evaluation) according to the Dawes-Redman algorithm, which informed the evaluation of predictive performance metrics. Performance was assessed using accuracy, sensitivity, specificity, positive predictive value, and negative predictive value (NPV) adjusted for various population risk prevalences of APO.ResultsA balanced dataset of 3316 antepartum FHR traces was developed with PSM (standardized mean difference < 0.10). The Dawes-Redman algorithm showed a high specificity of 90.7% (95% CI, 89.2-92.0%) for ruling out APO. Sensitivity was 18.2% (95% CI, 16.3-20.0%). The NPV varied with the population prevalence of APO and was high in very-low-risk settings (NPV, 99.1% (95% CI, 98.9-99.3%) at 1% APO prevalence) and decreased with increasing risk of APO (NPV, 72.1% (95% CI, 67.7-76.1%) at 30% APO prevalence). Temporal proximity of FHR assessment to delivery indicated robust specificity, which was similar for assessments performed at 0-24 h and 24-48 h prior to delivery (specificity at 0-24 h, 90.8% (95% CI, 88.8-92.7%); specificity at 24-48 h, 90.3% (95% CI, 88.2-92.3%); P = 0.898). Across the different adverse outcomes comprising the APO cohort, the performance of the Dawes-Redman algorithm remained consistent, with high specificity (ranging from 87.7% to 94.7%) and NPVs (ranging from 95.4% to 96.0%), confirming its utility in identifying fetal wellbeing.ConclusionThese findings indicate that the Dawes-Redman algorithm is effective for its intended purpose: identifying a state of fetal wellbeing. This is evidenced by its high specificity. However, its low sensitivity suggests limitations in its ability to identify fetuses at risk of APO. The predictive accuracy of the algorithm is affected significantly by the prevalence of healthy pregnancies within the population. Clinical interpretation of FHR traces that do not satisfy the 10 Dawes-Redman criteria warrant further expert clinical evaluation. While the algorithm proves reliable for its primary objective, the development of an algorithm optimized for high-risk pregnancy scenarios remains an area of interest for future study. © 2025 The Author(s).
