Project description:Countries around the world have had to respond to the COVID-19 outbreak with limited information and confronting many uncertainties. Their ability to be agile and adaptive has been stressed, particularly in regard to the timing of policy measures, the level of decision centralization, the autonomy of decisions and the balance between change and stability. In this contribution we use our observations of responses to COVID-19 to reflect on agility and adaptive governance and provide tools to evaluate it after the dust has settled. Whereas agility relates mainly to the speed of response within given structures, adaptivity implies system-level changes throughout government. Existing institutional structures and tools can enable adaptivity and agility, which can be complimentary approaches. However, agility sometimes conflicts with adaptability. Our analysis points to the paradoxical nature of adaptive governance. Indeed, successful adaptive governance calls for both decision speed and sound analysis, for both centralized and decentralized decision-making, for both innovation and bureaucracy, and both science and politics.

Project description:The human monkeypox virus (MPV), a zoonotic illness that was hitherto solely prevalent in Central and West Africa, has lately been discovered to infect people all over the world and has become a major threat to global health. Humans unintentionally contract this zoonotic orthopoxvirus, which resembles smallpox, when they come into contact with infected animals. Studies show that the illness can also be transferred through frequent proximity, respiratory droplets, and household linens such as towels and bedding. However, MPV infection does not presently have a specified therapy. Smallpox vaccinations provide cross-protection against MPV because of antigenic similarities. Despite scant knowledge of the genesis, epidemiology, and ecology of the illness, the incidence and geographic distribution of monkeypox outbreaks have grown recently. Polymerase chain reaction technique on lesion specimens can be used to detect MPV. Vaccines like ACAM2000, vaccinia immune globulin intravenous (VIG-IV), and JYNNEOS (brand name: Imvamune or Imvanex) as well as FDA-approved antiviral medications such as brincidofovir (brand name: Tembexa), tecovirimat (brand name: TPOXX or ST-246), and cidofovir (brand name: Vistide) are used as therapeutic medications against MPV. In this overview, we provide an outline of the MPV's morphology, evolution, mechanism, transmission, diagnosis, preventative measures, and therapeutic approaches. This study offers the fundamental information required to prevent and manage any further spread of this emerging virus.

Project description:Abstract Does clear and transparent communication of risks, benefits, and uncertainties increase or undermine public trust in scientific information that people use to guide their decision-making? We examined the impact of reframing messages written in traditional persuasive style to align instead with recent “evidence communication” principles, aiming to inform decision-making: communicating a balance of risks and benefits, disclosing uncertainties and evidence quality, and prebunking misperceptions. In two pre-registered experiments, UK participants read either a persuasive message or a balanced and informative message adhering to evidence communication recommendations about COVID-19 vaccines (Study 1) or nuclear power plants (Study 2). We find that balanced messages are either perceived as trustworthy as persuasive messages (Study 1), or more so (Study 2). However, we note a moderating role of prior beliefs such that balanced messages were consistently perceived as more trustworthy among those with negative or neutral prior beliefs about the message content. We furthermore note that participants who had read the persuasive message on nuclear power plants voiced significantly stronger support for nuclear power than those who had read the balanced message, despite rating the information as less trustworthy. There was no difference in vaccination intentions between groups reading the different vaccine messages.

Project description:ObjectiveTo analyze the Swiss public perception of COVID-19 communication by the government and its impact on behavior and trust.MethodsA cross-sectional representative online survey was conducted in January 2022 in a sample of 2587 Swiss residents.ResultsOverall the survey showed moderate satisfaction with communication, with relevance rated highly. Perceptions of communication varied across demographics. Also, differences were noted in trust in institutions, risk perception, and attitudes towards public health measures based on perceived communication quality.ConclusionThis study highlights the dynamic nature of public health communication and underscores the significance of continuous adaptation and evaluation of strategies to effectively reach and influence diverse audiences. As misinformation persists, the study underscores the need for informative, empathetic, and honest communication, as well as tailored approaches to build public trust-an essential asset for managing health crises successfully.InnovationThis study provides innovative concrete insights into how governmental communication could be designed more strategically to effectively communicate with the public in contexts characterized by disinformation, emphasizing the role of quality, honest, and empathetic communication in public health messaging.

Project description:The digitalisation of healthcare, fueled by advances in technology and the COVID-19 pandemic can not only empower equitable access to global expert-level healthcare but also make healthcare more patient-centric. Every digital health solution has one common fundamental component: they all run on computing platforms and are powered by complex software. Traditional software development life cycles have often failed in designing complex software; consequently, the agile approach was introduced. To assess the role of agile in digital public health transformation, we used the synthetic knowledge synthesis, a triangulation of bibliometric mapping, and thematic analysis to analyse the available literature harvested from PubMed. The analysis showed that the use of the agile approach is underutilised in developing digital health software. Moreover, the study revealed that health organisations did not yet embrace the agile culture and should adapt using innovative agile solutions to deliver clinical value to patients and public health systems. Following the software industry, where agile software development is becoming the mainstream approach also for sensitive and regulated software, it is becoming even more essential that the digital health software development process should be modernised. Furthermore, a shift to agile collaboration, agile decision-making, trial tolerance, active engagement, purposeful technology adoption, knowledge sharing, and an open agile innovation ecosystem must be achieved.

Project description:Abstract The COVID?19 pandemic has caught many nations by surprise and has already caused millions of infections and hundreds of thousands of deaths worldwide. It has also exposed a deep crisis in modeling and exposed a lack of systems thinking by focusing mainly on only the short term and thinking of this event as only a health crisis. In this paper, authors from several of the key countries involved in COVID?19 propose a holistic systems model that views the problem from a perspective of human society including the natural environment, human population, health system, and economic system. We model the crisis theoretically as a feedback control problem with delay, and partial controllability and observability. Using a quantitative model of the human population allows us to test different assumptions such as detection threshold, delay to take action, fraction of the population infected, effectiveness and length of confinement strategies, and impact of earlier lifting of social distancing restrictions. Each conceptual scenario is subject to 1000+ Monte?Carlo simulations and yields both expected and surprising results. For example, we demonstrate through computational experiments that maintaining strict confinement policies for longer than 60 days may indeed be able to suppress lethality below 1% and yield the best health outcomes, but cause economic damages due to lost work that could turn out to be counterproductive in the long term. We conclude by proposing a hierarchical Computerized, Command, Control, and Communications (C4) information system and enterprise architecture for COVID?19 with real?time measurements and control actions taken at each level.

Project description:BackgroundFollowing the SARS outbreak, the World Health Organization revised the International Health Regulations to include risk communication as one of the core capacity areas. In 2006, the U.S. Centers for Disease Control and Prevention's Global Disease Detection [GDD] program began collaborating with China to enhance China's risk communication capacity to address gaps in the SARS communication response. This article describes tangible improvements in China's public health emergency risk communication capacity between the SARS and H7N9 outbreaks; documents U.S. CDC GDD cooperative technical assistance during 2006-2017; and shares lessons learnt to benefit other countries and contribute to enhance global health security.MethodA questionnaire based on the WHO Joint External Evaluation tool [Risk Communication section] was developed. A key communications official from the China National Health Commission [NHC] completed the questionnaire retrospectively to reflect China's capacity to manage communication response before, during and after the outbreaks of SARS in 2003, influenza H1N1 in 2009, and influenza H7N9 in 2013. A literature search was also conducted in English and Chinese to further substantiate the results of the questionnaire completed by NHC.ResultsChina demonstrated significantly improved risk communication capacities of pre-event, during event and post event responses to H7N9 when compared to the SARS response. China NHC improved its response through preparedness, availability of dedicated staff and resources for risk communication, internal clearance mechanisms, standard operating procedures with national response parties external to NHC, rumor management, communication with international agencies and consistent messaging with healthcare and private sectors. Correspondingly, the perceived level of trust that the public had in the NHC following outbreaks rose between the SARS and H7N9 response.ConclusionRisk communication capacities in China have increased during the ten years between the SARS outbreak of 2003 and the H7N9 outbreak of 2013. Long-term risk communication capacity building efforts in bilateral collaborations are uncommon. The U.S. CDC GDD project was one of the first such collaborations worldwide. The lessons learned from this project may benefit lower and middle-income countries as they build their national emergency risk communication capacity.

Project description:The coronavirus pandemic caused a state of panic worldwide. Mixed messages were given about its risk and how to contain it, when trust in authorities and reliable scientific information are essential to reduce unnecessary scares and inappropriate risk perceptions. We know little about public concerns and opinions in health authorities in the Arab world. Thus, an attempt was made to generate such information through a web-based survey. A total of 1074 subjects from Lebanon, Jordan, and Tunisia were recruited to explore their perception of food and non-food risks of infection and the influence of the source of information, trust, and attitudes towards the local authorities' communication of risk. Seventy percent of the respondents were concerned that COVID-19 may be transmitted through food. The perception of risk from touching contaminated surfaces and food packaging and being exposed to infected people during food shopping was even higher. For only less than half of the respondents, the information from local authorities was considered trustworthy and the associated risk communication and response to false rumors were timely, effective, and clear. But the satisfaction level among the Jordanians was remarkably stronger than for the Lebanese and Tunisian respondents. The demographic factors, trust in information, and attitudes towards authorities' performance in risk communication did not influence risk perceptions. Respondents' knowledge was limited based on their chief sources of information, such as social media, local news media broadcasts, and announcements by the World Health Organization. Our conclusion is that unnecessary fear increases among the public when risks that impact heath are unknown. More research in the Arab region is needed to understand the determinants of risk perceptions considering psychological factors on the risk to health. Unfortunately, it is difficult to restrict or contain misleading information from various forms of social media. We recommend that for reducing fear and building confidence with the public for appropriate action during the pandemic, local authorities should enhance the quality and level of details of the information that they share during such crises.

Project description:This study shows that social and political trust may diverge in the face of shared threats, and that this pattern is driven by negative information about crisis management. Leveraging a three-wave panel survey and an information-provision experiment in the USA during the COVID-19 crisis, our research reveals that negative perceptions of pandemic management lead to a decline in political trust and a parallel increase in social trust. This dynamic is pronounced among government supporters, who, confronted with COVID-19 challenges, experience a substantial erosion of political trust. Simultaneously, there is a notable rise in social trust within this group. Our analysis suggests that, as government supporters attributed more responsibility for the crisis to their political leader, political trust was supplanted by social trust. Disenchanted voters, feeling let down by institutions, sought support in society. Both the survey and the experiment underscore that societal shocks can prompt individuals to shift from relying on formal institutions to informal ones as a coping strategy. This research contributes a generalizable framework explaining how negative perceptions of crisis management can lead societies to substitute political trust with social trust, advancing our understanding of societal responses to shared threats and adaptive strategies during crises.

Project description:Rising COVID-19 cases in Canada in early 2021, coupled with pervasive mis- and disinformation, demonstrate the critical relationship between effective crisis communication, trust, and risk protective measure adherence by the public. Trust in crisis communication is affected by the communication's characteristics including transparency, timeliness, empathy, and clarity, as well as the source and communication channels used. Crisis communication occurs in a rhetorical arena where various actors, including public health, news media, and the public, are co-producing and responding to messages. Rhetorical arenas must be monitored to assess the acceptance of messaging. The quality and content of Canadian public health and news media crisis communication on Facebook were evaluated to understand the use of key guiding principles of effective crisis communication, the focus of the communication, and subsequent public emotional response to included posts. Four hundred and thirty-eight posts and 26,774 anonymized comments were collected and analyzed. Overall, the guiding principles for effective crisis communication were inconsistently applied and combined. A limited combination of guiding principles, especially those that demonstrate trustworthiness, was likely driving the negative sentiment uncovered in the comments. Public health and news media should use the guiding principles consistently to increase positive sentiment and build trust among followers.
