Arthroscopic Excision of Bipartite Patella.

ABSTRACT: A bipartite patella usually presents as an incidental finding on radiographs because most cases are asymptomatic. However, some patients may present with pain and functional limitations. Conservative treatment is sufficient to resolve symptoms in most cases; however, a small minority of patients may require surgical management. Recent studies have reported excellent results with an arthroscopic approach. This Technical Note details our procedure for treating a symptomatic bipartite patella that has not resolved with conservative care.

Project description:Bipartite patella (BP) is uncommon and is usually an incidental finding. Painful BP occurs rarely and is usually associated with post-traumatic anterior knee pain. When nonoperative treatment fails, fragment resection is indicated. A systematic review shows a potentially increased risk of complications with open surgery. The purpose of this Technical Note is to demonstrate a simple arthroscopic technique for treating painful BP through resection of the bone fragment.

Arthroscopic Bony Resection for Treatment of Symptomatic Bipartite Patella.

Project description:Bipartite patella is a common pathology, affecting 2% to 3% of the population. Usually these cases are bilateral and asymptomatic. However, a patient with a bipartite patella may complain of pain, which is most likely related to fragment mobility. Conservative treatment with physiotherapy, immobilization, and nonsteroidal drugs are encouraged during the initial 6 months following diagnosis. This option is effective in most patients. For patients who do not improve with these conservative measures, surgical intervention is indicated. Many surgical treatment options have been described including excision of the fragment, as well as fixation. Fragment excision has demonstrated positive outcomes and is associated with an asymptomatic return to sport activities. The purpose of this Technical Note is to describe our preferred arthroscopic technique for the treatment of symptomatic bipartite patella.

Excision of painful bipartite patella: good long-term outcome in young adults.

Project description:UNLABELLED: Excision of the accessory bipartite fragment is widely used, but its long-term outcome is not known. We evaluated the outcome after surgical excision of a symptomatic accessory bipartite or multipartite patella fragment in young adult men performing their compulsory military service and determined the incidence of painful bipartite patellae in this group of skeletally mature adults. We followed 25 of 32 patients for a minimum of 10 years (mean, 15 years; range, 10-22 years). The incidence of painful, surgically treated bipartite patella was 9.2 per 100,000 recruits. Patients' median age at surgery was 20 years. There were 19 superolateral and six lateral bipartite fragments. Other radiographic findings were rare. At followup, the Kujala score mean was 95 points (range, 75-100 points), and osteoarthrotic changes (Kellgren-Lawrence Grade 1) were seen in two knees. No reoperations related to bipartite patella occurred during the followup. Symptomatic bipartite patella is rare and does not seem primarily associated with anatomic deviations, but when incapacitating pain persists despite nonoperative treatment, surgical excision seems to yield reasonable functional outcome and quick recovery with no apparent adverse sequelae. Our data suggest there is no reason to avoid this technically undemanding procedure for treating persistent symptoms of bipartite patella in young adults. Bipartite patella separation and partial quadriceps tendon rupture in the setting of trauma.

Project description:Normal development of the patella typically involves fusion of secondary ossification centers into a single bone during adolescence, with failure of fusion resulting in bipartite and tripartite patellae. In such variants, injury to incomplete ossification center fusion, though uncommon, has been reported to occur in the setting of traumatic quadriceps tendon rupture. The authors present a rare and complex case of traumatic bipartite fragment separation, patellar avulsion, and a complex partial quadriceps tendon tear confirmed surgically in a 36-year-old male. In this case, a tear in the lateral aspect of the quadriceps tendon attached to the nonfused patellar ossification center resulted in retraction of the band containing the bipartite fragment and separation of the patellar fragments, with superior displacement of the smaller bony avulsion likely due to complex attachments from the medial aspect of the quadriceps tendon. Knowledge of the classical locations of a bipartite and tripartite patella can aid in the differentiation of the anatomic variant versus patellar avulsion. Additionally, knowledge of the variable and complex nature of the quadriceps tendon aids in understanding the process of patellar avulsions and various tears, leading to the appropriate orthopedic management.

Arthroscopic Excision of the Sternoclavicular Joint.

Project description:Osteoarthritis changes at the sternoclavicular joint (SCJ) have been shown to be present on computed tomography in more than 90% of people over the age of 60 years. Although usually asymptomatic, when symptoms do occur, they can be very debilitating. Most patients respond favorably to conservative treatment, but there is a small cohort of patients who continue to be symptomatic despite adequate conservative treatment. Surgical management with an open SCJ excision has been shown to give satisfactory results. However, probably due to concerns with regard to damage to the mediastinal structures, instability, and scarring, there is a high threshold for surgery. Arthroscopic SCJ excision has been shown to achieve similar results to an open procedure while avoiding some of the risks. In this Technical Note, we describe the indications, imaging, and the technique of an arthroscopic excision of the SCJ.

Case report: bipartite tibial epiphysis: radiologic and arthroscopic presentation.

Project description:We report the case of a bilateral bipartite ossification center of the proximal tibia in a 15-year-old boy with a history of knee injury. The presence of the duplicated ossific centers was an incidental finding on radiography and computed tomography performed after injury. Arthroscopy showed continuous hyaline cartilage between the two ossific centers confirming the diagnosis of a developmental abnormality; however, the articular surface was depressed in the region overlying the smaller ossific center. We presume the presence of an accessory center in the posteromedial aspect of the tibial plate could be a source of instability for the corresponding capsular and meniscal structures. This is, to our knowledge, the first report of tibial bipartite epiphysis.

Arthroscopic Excision of a Symptomatic Meso-acromiale.

Project description:The surgical technique for the management of a symptomatic os acromiale remains unclear. Several operative techniques have been described including open excision, open reduction-internal fixation (ORIF), arthroscopic acromioplasty or subacromial decompression, and arthroscopic excision. There are 4 types of os acromiale, with the meso-acromion being the most common and difficult to treat. The excision of a pre-acromion arthroscopically or in an open manner usually produces satisfactory results. However, the open excision of a meso-acromion can lead to persistent pain and deltoid weakness and atrophy. The management of a meso-acromial fragment with ORIF can also result in persistent pain and deltoid weakness and atrophy with nonunion of the fragments. The purpose of this article is to describe an alternative surgical technique to open excision or ORIF when presented with a symptomatic meso-acromiale.

Dry Arthroscopic Excision of Dorsal Wrist Ganglion.

Project description:Ganglions are common soft tissue masses of the hand. High recurrence rates are associated with nonsurgical treatment; thus, excision is often indicated. Arthroscopic excision and open excision have similar recurrence rates; however, the latter is associated with prolonged healing time and increased scarring. Recently, dry wrist arthroscopic techniques have been used. This technique allows easier confirmation of complete ganglion removal, easier conversion to open surgery, earlier return of motion, and stitch-less closure when compared with traditional "wet" arthroscopic excision.

Arthroscopic Excision of Osteoid Osteoma of the Elbow.

Project description:Osteoid osteoma has been reported infrequently around the elbow joint, and is usually treated with radiofrequency ablation or open excision. Elbow arthroscopy is useful for excision of accessible lesions, and the accompanying elbow stiffness can be treated concurrently. This report describes an arthroscopic excision of an osteoid osteoma located in the juxta-articular distal humeral bone. An initial adhesiolysis and capsulectomy is performed to gain access to the region above the capitellar articular margin. A 70° arthroscope is necessary to visualize this region via the anteromedial portal. An accessory lateral portal is used to detach the capsule from the distal humerus; the scar tissue overlying the lesion is debrided and the circumferential lesion is visualized and probed for identification of its extent. A biopsy is performed, and thereafter the lesion is excised piecemeal. The sclerotic margins are curetted, and radiofrequency ablation is performed. The technique is cosmetic and minimally invasive, and can be performed in revision scenarios involving recurrences after open surgery or needle radiofrequency thermal ablation. Early rehabilitation restores range of motion and strength. Technical tips for identification and curettage of the lesion are presented.
