Insecticide-treated nets (ITNs) are one of the most widespread and impactful malaria interventions in Africa, yet a spatially-resolved time series of ITN coverage has never been published. Using data from multiple sources, we generate high-resolution maps of ITN access, use, and nets-per-capita annually from 2000 to 2020 across the 40 highest-burden African countries. Project description:BackgroundAs insecticide-treated nets (ITNs) wear out and are disposed, some household members are prioritized to use remaining ITNs. This study assessed how nets are allocated within households to individuals of different age categories as ITNs are lost or damaged and as new ITNs are obtained. The study also explored how ITN allocation affects ITN durability.MethodsA cross-sectional household survey and ITN durability study was conducted among 2,875 households across Tanzania to determine the proportion of nets that remain protective (serviceable) 22 months after net distribution aiming for universal coverage. Allocation of study nets within houses, and re-allocation of ITNs when new universal replacement campaign (URC) nets arrived in study households in Musoma District, was also assessed.ResultsSome 57.0% (95% CI 53.9-60.1%) of households had sufficient ITNs for every household member, while 84.4% (95% CI 82.4-86.4%) of the population had access to an ITN within their household (assuming 1 net covers every 2 members). In households with sufficient nets, 77.5% of members slept under ITNs. In households without sufficient nets, pregnant women (54.6%), children < 5 years (45.8%) and adults (42.1%) were prioritized, with fewer school-age children 5-14 years (35.9%), youths 15-24 years (28.1%) and seniors > 65 years (32.6%) sleeping under ITNs. Crowding ([Formula: see text] 3 people sleeping under nets) was twice as common among people residing in houses without sufficient nets for all age groups, apart from children < 5. Nets were less likely to be serviceable if: [Formula: see text] 3 people slept under them (OR 0.50 (95% CI 0.40-0.63)), or if nets were used by school-age children (OR 0.72 (95% CI 0.56-0.93)), or if the net product was Olyset®. One month after the URC, only 23.6% (95% CI 16.7-30.6%) of the population had access to a URC ITN in Musoma district. Householders in Musoma district continued the use of old ITNs even with the arrival of new URC nets.ConclusionUsers determined the useful life of ITNs and prioritized pregnant women and children < 5 to serviceable ITNs. When household net access declines, users adjust by crowding under remaining nets, which further reduces ITN lifespan. School-age children that commonly harbour gametocytes that mediate malaria transmission are compelled to sleep under unserviceable nets, crowd under nets or remain uncovered. However, they were accommodated by the arrival of new nets. More frequent ITN delivery through the school net programme in combination with mass distribution campaigns is essential to maximize ITN effectiveness.

Project description:Insecticide-treated nets (ITNs) for malaria control are widespread but coverage remains inadequate. We developed a Bayesian model using data from 102 national surveys, triangulated against delivery data and distribution reports, to generate year-by-year estimates of four ITN coverage indicators. We explored the impact of two potential 'inefficiencies': uneven net distribution among households and rapid rates of net loss from households. We estimated that, in 2013, 21% (17%-26%) of ITNs were over-allocated and this has worsened over time as overall net provision has increased. We estimated that rates of ITN loss from households are more rapid than previously thought, with 50% lost after 23 (20-28) months. We predict that the current estimate of 920 million additional ITNs required to achieve universal coverage would in reality yield a lower level of coverage (77% population access). By improving efficiency, however, the 920 million ITNs could yield population access as high as 95%.

Project description:BackgroundMass distribution of long-lasting insecticide treated bed nets (LLINs) has led to large increases in LLIN coverage in many African countries. As LLIN ownership levels increase, planners of future mass distributions face the challenge of deciding whether to ignore the nets already owned by households or to take these into account and attempt to target individuals or households without nets. Taking existing nets into account would reduce commodity costs but require more sophisticated, and potentially more costly, distribution procedures. The decision may also have implications for the average age of nets in use and therefore on the maintenance of universal LLIN coverage over time.MethodsA stochastic simulation model based on the NetCALC algorithm was used to determine the scenarios under which it would be cost saving to take existing nets into account, and the potential effects of doing so on the age profile of LLINs owned. The model accounted for variability in timing of distributions, concomitant use of continuous distribution systems, population growth, sampling error in pre-campaign coverage surveys, variable net 'decay' parameters and other factors including the feasibility and accuracy of identifying existing nets in the field.ResultsResults indicate that (i) where pre-campaign coverage is around 40% (of households owning at least 1 LLIN), accounting for existing nets in the campaign will have little effect on the mean age of the net population and (ii) even at pre-campaign coverage levels above 40%, an approach that reduces LLIN distribution requirements by taking existing nets into account may have only a small chance of being cost-saving overall, depending largely on the feasibility of identifying nets in the field. Based on existing literature the epidemiological implications of such a strategy is likely to vary by transmission setting, and the risks of leaving older nets in the field when accounting for existing nets must be considered.ConclusionsWhere pre-campaign coverage levels established by a household survey are below 40% we recommend that planners do not take such LLINs into account and instead plan a blanket mass distribution. At pre-campaign coverage levels above 40%, campaign planners should make explicit consideration of the cost and feasibility of accounting for existing LLINs before planning blanket mass distributions. Planners should also consider restricting the coverage estimates used for this decision to only include nets under two years of age in order to ensure that old and damaged nets do not compose too large a fraction of existing net coverage.

Project description:Globally equitable access to safely managed drinking water and sanitation is one of the major aims of the United Nations' Sustainable Development Goals, specifically SDG goal 6. We assessed global-scale progress toward this goal from 2000 to 2020 with access rates to improved drinking water and sanitation services that are adjusted for socioeconomic, political, and hydrological conditions. We found that the adjusted access rates in 2020 were lower than 2000, although not-adjusted access rates had increased. Access rates improved more slowly in higher-GDP countries than in lower-GDP ones during this period. These show that access rates in lower-income countries improved, but were still lower, compared with those in countries with the same social conditions in 2000. This suggests the recent progress toward this goal has not aligned well with social development, emphasizing need for reflecting national management, international cooperation, and investment in water-related infrastructures to achieve the goal by 2030.

Project description:Food supply has an impact on the prevalence of diet-related non-communicable diseases. We aimed to analyze the protein, fat (g/capita/day) and calorie (kcal/capita/day) supply from 2000 to 2019 as derived from the OECD Health Statistics database. A joinpoint regression was used to examine the number and location of breakpoints in the time series. The annual percent change (APC) was calculated using Joinpoint 4.9.0.0. The per capita daily kcal per nutrient was calculated for each country and the resulting percentage distributions were compared to the acceptable macronutrient distribution ranges. Protein, fat and calorie supplies have increased significantly between 2000 and 2019. Each started to show a much steeper, positive change between 2012 and 2014 (APCfat: 1.0; 95%CI: 0.8-1.1; APCprotein: 0.5; 95%CI: 0.3-0.6; APCkcal: 0.4; 95%CI: 0.3-0.5). In terms of the composition of the daily calorie intake per capita, the overall share of fat (+4.9%) and protein (+1.0%) increased between 2000 and 2019. We found significant differences among countries and also an increasing and optimal proportion of consumed protein per total calorie in all countries over the last two decades. We concluded that several countries have access to fat availability above the optimal level, which deserves particular attention from health policy makers in the fight against obesity and diet-related diseases.

Project description:From bacteria to metazoans, higher density populations have lower per capita metabolic rates than lower density populations. The negative covariance between population density and metabolic rate is thought to represent a form of adaptive metabolic plasticity. A relationship between density and metabolism was actually first noted 100 years ago, and was focused on spermatozoa; even then, it was postulated that adaptive plasticity drove this pattern. Since then, contemporary studies of sperm metabolism specifically assume that sperm concentration has no effect on metabolism and that sperm metabolic rates show no adaptive plasticity. We did a systematic review to estimate the relationship between sperm aerobic metabolism and sperm concentration, for 198 estimates spanning 49 species, from protostomes to humans from 88 studies. We found strong evidence that per capita metabolic rates are concentration dependent: both within and among species, sperm have lower metabolisms in dense ejaculates, but increase their metabolism when diluted. On average, a 10-fold decrease in sperm concentration increased per capita metabolic rate by 35%. Metabolic plasticity in sperm appears to be an adaptive response, whereby sperm maximize their chances of encountering eggs.

Project description:Seasonal variation in the proportion of the population using an insecticide-treated net (ITN) is well documented and is widely believed to be dependent on mosquito abundance and heat, driven by rainfall and temperature. However, seasonal variation in ITN use has not been quantified controlling for ITN access. Demographic and Health Survey and Malaria Indicator Survey datasets, their georeferenced data, and public rainfall and climate layers were pooled for 21 countries. Nine rainfall typologies were developed from rainfall patterns in Köppen climate zones. For each typology, the odds of ITN use among individuals with access to an ITN within their households ("ITN use given access") were estimated for each month of the year, controlling for region, wealth quintile, residence, year, temperature, and malaria parasitemia level. Seasonality of ITN use given access was observed over all nine rainfall typologies and was most pronounced in arid climates and less pronounced where rainfall was relatively constant throughout the year. Peak ITN use occurred 1-3 months after peak rainfall and corresponded with peak malaria incidence and average malaria transmission season. The observed lags between peak rainfall and peak ITN use given access suggest that net use is triggered by mosquito density. In equatorial areas, ITN use is likely to be high year-round, given the presence of mosquitoes and an associated year-round perceived malaria risk. These results can be used to inform behavior change interventions to improve ITN use in specific times of the year and to inform geospatial models of the impact of ITNs on transmission.

Project description:BackgroundPopulation access to insecticide-treated nets (ITN) is usually determined from survey data. However, for planning purposes it is necessary to estimate this indicator between surveys. Two different approaches are currently recommended for such estimates from administrative data, multiplying the number of ITN delivered either by 2.0 or 1.8 before dividing by the population. However, the validity of such estimates has not previously been investigated.MethodsThirty-five datasets from household surveys in sub-Saharan Africa were selected from ten different countries. The number of ITN and de-facto population from the samples was used as proxy administrative data and estimates of population access to ITN were calculated using the recommended formulae. Administrative estimates were compared to the access indicator from the survey data. Regression analysis was used to further define the relationship between administrative and survey population access. Mean number of ITN users was determined for each data set separately for households with and without enough ITN.ResultsAnalysis of users per ITN showed that the assumption of two users per net is valid overall (median 2.00) but that it was consistently lower in households with at least one ITN for every two people (median 1.66). Using the formula number of ITN times 2.0 divided by the population to estimate population access to ITN from administrative data generally overestimated the survey access indicator. This was particularly the case at higher coverage levels, resulting in a 30 percentage-point overestimate at survey access above 80%. Using 1.8 as the multiplier for the number of ITN from administrative data improved the results but still showed a 19 percentage-point overestimate at access coverage above 80%. Regression analysis found that a factor of 1.64 provides the best prediction of the access indicator with slight underestimation at low access levels but good fit at levels above 55%.ConclusionsA factor of 1.6 rather than 2.0 or 1.8 as the mean number of users per ITN provides a more accurate estimation of population access to ITN from administrative data accounting for discordant ITN-person pairs and a reduced number of ITN users when sufficient ITN are available.

Project description:Following well-established practices in demography, this article discusses several measures based on the number of CoViD-19 deaths to facilitate comparisons over time and across populations. National populations in 186 UN countries and territories and populations in first-level sub-national administrative entities in Brazil, China, Italy, Mexico, Peru, Spain, and the USA. None (death statistics only). An unstandardized occurrence/exposure rate comparable to the Crude Death Rate; an indirectly age-and-sex standardized rate that can be derived even when the breakdown of CoViD-19 deaths by age and sex required for direct standardization is unavailable; the reduction in life expectancy at birth corresponding to the 2020 number of CoViD-19 deaths. To date, the highest unstandardized rate has been in New York, at its peak exceeding the state 2017 Crude Death Rate. Populations compare differently after standardization: while parts of Italy, Spain and the USA have the highest unstandardized rates, parts of Mexico and Peru have the highest standardized rates. For several populations with the necessary data by age and sex for direct standardization, we show that direct and indirect standardization yield similar results. US life expectancy is estimated to have declined this year by more than a year (-1.26 years), far more than during the worst year of the HIV epidemic, or the worst three years of the opioid crisis, and to reach its lowest level since 2008. Substantially larger reductions, exceeding two years, are estimated for Panama, Peru, and parts of Italy, Spain, the USA, and especially, Mexico. With lesser demand on data than direct standardization, indirect standardization is a valid alternative to adjust international comparisons for differences in population distribution by sex and age-groups. A number of populations have experienced reductions in 2020 life expectancies that are substantial by recent historical standards.
