Association between domain-specific physical activity and diabetes in Korean adults.

ABSTRACT: This study aimed to investigate the association between domain-specific physical activity (PA) and diabetes in Korean adults. We analyzed 26,653 men and women (aged > 18 years) from the Korea National Health and Nutrition Examination Survey (2014-2018). PA was measured using a validated Global PA Questionnaire. Multivariable logistic regression was used to estimate odds ratios (ORs) and 95% confidence intervals (CIs) after adjustment for various confounders. Transport PA accounted for the majority of total PA (46%, men; 58%, women), followed by leisure-time PA (30%; 22%) and work PA (24%; 20%). In men, ORs (95% CI) of diabetes comparing ≥ 600 metabolic task of equivalent (MET)-min/week vs. no activity were 0.82 (0.71-0.95) for leisure-time PA, 0.85 (0.75-0.96) for transport PA, and 0.88 (0.78-0.99) for leisure-time + transport PA. In women, ORs (95% CI) of diabetes comparing the same groups were 0.73 (0.60-0.89) for leisure-time PA, 0.97 (0.85-1.10) for transport PA, and 0.88 (0.78-1.00) for leisure-time + transport PA. However, work PA showed no association with diabetes. In conclusion, leisure-time PA was inversely associated with diabetes in both men and women, while transport PA was inversely associated only in men. But work PA was not associated with diabetes in Korean adults.

Project description:BackgroundMetabolic syndrome (MetS) increases the risk of cardiovascular disease morbidity and mortality. Physical activity (PA) reduces the likelihood of MetS, but it is a complex behavior and is accumulated in multiple domains.PurposeTo cross-sectionally investigate the total and domain-specific relationships between PA and MetS in U.S. adults.MethodsData from 3,408 adults participating in the National Health and Nutrition Examination Survey (2013-2018) were analyzed. Blood pressure (BP), waist circumference (WC), fasting blood glucose (GLU), triglycerides (TRIG), and high-density lipoprotein (HDL) were measured. MetS and its risk factors were the primary and secondary outcomes, respectively. Weekly minutes of total PA and domain-specific PA (i.e., leisure-time (LTPA), transportation (TPA), occupational & household (OHPA)), were self-reported. For each exposure, participants were grouped into weekly PA: (1) 0 min, (2) 1-149 min, (3) 150-299 min, (4) 300-599 min, and (5) 600 + minutes. Logistic regression estimated the odds of having MetS, and its risk factors from PA.ResultsTotal PA was associated with lower odds of most MetS risk factors. Compared to no LTPA, and independent of TPA and OHPA, engaging in 150-299 and 300-599 min/week of LTPA was associated with 30% (OR = 0.70 [95%CI: 0.50, 0.98]) and 43% (OR = 0.57 [95%CI: 0.35, 0.92]) lower odds of MetS, respectively. LTPA was also associated with lower odds of having high WC, GLU, TRIG, and low HDL (ORs = 0.52-0.68). Compared to no TPA, and independent of LTPA and OHPA, engaging in 300-599 min/week of TPA was associated with 54% lower odds of MetS (OR = 0.46 [95%CI: 0.25, 0.84]) and 40% lower odds of having a high WC (OR = 0.40 [95%CI: 0.21, 0.76]). Engaging in OHPA was not associated with MetS but was associated with greater odds of having a high WC (OR = 1.44 [95%CI: 1.03, 2.01]), and GLU (ORs = 1.52-1.83), independent of LTPA and TPA.ConclusionTotal PA, seemingly driven by LTPA, was inversely associated with cardiometabolic health. TPA also showcases some protective associations, while OHPA appears to not confer cardiometabolic health benefits. Longitudinal data should confirm these associations using more robust PA measurement tools.

Project description:BackgroundPhysical activity is associated with depression. However, benefits of physical activity on depression may differ for specific domains of physical activity (i.e., leisure-time, work, and travel). Moreover, the relationship between physical activity and depression could also differ for people in different Body Mass Index (BMI) categories. This study investigated the relationship between domain-specific physical activity and BMI with depression, and the moderation effects of BMI on the relationship between domain physical activity and depression.MethodsComplex survey data from the NHANES 2011-2014 was used (N=10,047). Depression was measured using the Patient Health Questionnaire (PHQ-9). Participants reported physical activity minutes in each domain using the Global Physical Activity Questionnaire. Demographic characteristics were self-reported. Weight and height were objectively measured and used for calculating BMI. Survey procedures were used to account for complex survey design. As two survey cycles were used, sampling weights were re-calculated and used for analyses. Taylor series linearisation was chosen as a variance estimation method.ResultsParticipants who engaged in ≥150 minutes/week of total moderate-vigorous physical activity (MVPA) (adjusted B = 0.83, 95% CI [0.50, 1.16]) and leisure-time MVPA (adjusted B = 0.84, 95% CI [0.57, 1.11]) experienced lower levels of depression compared to those engaging in <150 MVPA minutes/week. Work and travel-related physical activity were not associated with depression. Overweight (adjusted B = -0.40, 95% CI [-0.76, -0.04]) and underweight/normal weight participants (adjusted B = -0.60, 95%CI [-0.96, -0.25]) experienced less depressive symptoms compared to obese participants. BMI did not moderate the relationship between domain-specific physical activity and depression.ConclusionsInterventions that focus on leisure-time physical activity appear to be best suited to improve depression, however, this needs to be confirmed in purposefully designed intervention studies. Future studies may also examine ways to improve the effectiveness of work and travel physical activity for reducing depression.
