A fully biodegradable polydioxanone occluder for ventricle septal defect closure.

Project description:Ventricular septal defect (VSD) is one of the commonest congenital heart diseases (CHDs). Current occluders for VSD treatment are mainly made of nitinol, which has the risk of nickel allergy, persistent myocardial abrasion and fatal arrythmia. Herein, a fully biodegradable polydioxanone (PDO) occluder equipped with a shape line and poly-l-lactic acid PLLA membranes is developed for VSD closure without the addition of metal marker. PDO occluder showed great mechanical strength, fatigue resistance, geometry fitness, biocompatibility and degradability. In a rat subcutaneous implantation model, PDO filaments significantly alleviated inflammation response, mitigated fibrosis and promoted endothelialization compared with nitinol. The safety and efficacy of PDO occluder were confirmed in a canine VSD model with 3-year follow-up, demonstrating the biodegradable PDO occluder could not only effectively repair VSD, induce cardiac remodeling but also address the complications associated with metal occluders. Furthermore, a pilot clinical trial with five VSD patients indicated that all the occluders were successfully implanted under the guidance of echocardiography and no adverse events occurred during the 3-month follow-up. Collectively, the fully bioresorbable PDO occluder is safe and effective for clinical VSD closure and holds great promise for the treatment of structural CHDs.

Endoscopic closure of esophageal, gastric, jejunal, and rectopelvic fistulas with cardiac septal occluder devices: a case series.

Project description:Background and aimsTreating GI fistulas and anastomotic leaks is a challenge. Traditionally, these adverse events have been treated with conservative measures or surgical interventions, both of which have been associated with high rates of morbidity. Although endoscopic techniques are available and commonly minimally invasive, their success can be variable and oftentimes require repeat interventions. Cardiac septal occluder (CSO) devices have emerged as an alternative for managing these complex GI fistulas and anastomotic leaks after the more conservative endoscopic options fail.MethodsBefore the introduction of the CSO device into the body, a CSO delivery system must first be created. After integrating this system with an endoscope, argon plasma coagulation is used on each of the fistulas, and the CSOs are then deployed under fluoroscopy.CasesWe present 4 cases of CSO device placement throughout the GI tract: in the esophagus, jejunum, stomach, and rectosigmoid colon. Each of these cases presents unique challenges in maneuvering the anatomy, ensuring the CSOs fully occlude the fistulas, and are successful in causing tissue in-growth postplacement.ConclusionsThis study presents a case series of 4 successful closures of GI fistulas using CSO devices after the failure of more conservative therapies. Each of these cases takes place in a different part of the GI tract-the esophagus, stomach, small intestines, and large intestines-indicating the broad application of CSO devices. CSOs offer a promising alternative for managing complex GI fistulas and postsurgical anastomotic leaks endoscopically throughout the entire GI tract.

Percutaneous closure of aortic pseudoaneurysm in a young female with atrial septal occluder.

Project description:This case report represents a novel technique for the treatment of a pseudoaneurysm of the aorta. Pseudoaneurysm of the aorta has been reported in patients post heart surgery. This case report is about a patient who had a pseudoaneurysm most probably following tuberculosis. Traditionally, the treatment of choice is surgical correction; however, in the current era, there are case reports describing the use of either stent grafts or Amplatzer occluders for occlusion of the pseudoaneurysm in high-risk surgical cases. We performed successful closure of the aortic pseudoaneurysm using atrial septal occluder. <Learning objective: Tuberculous arteritis can result in pseudoaneurysm rarely. Our patient had a large aneurysm compressing adjoining vital structures and was very high surgical risk case. This is the first case report of closure of tuberculous pseudoaneurysm by using atrial septal occluder in a young patient. In this case we have demonstrated that large pseudoaneurysms with narrow neck can be closed very safely using atrial septal occluder with immediate symptom relief and thrombosis of pseudoaneurysm at follow-up of three months.>.
