Health effects from heat waves in France: an economic evaluation.

ABSTRACT:

Background

Scarcity of data on the health impacts and associated economic costs of heat waves may limit the will to invest in adaptation measures. We assessed the economic impact associated with mortality, morbidity, and loss of well-being during heat waves in France between 2015 and 2019.

Methods

Health indicators monitored by the French national heat wave plan were used to estimate excess visits to emergency rooms and outpatient clinics and hospitalizations for heat-related causes. Total excess mortality and years of life loss were considered, as well as the size of the population that experienced restricted activity. A cost-of-illness and willingness-to-pay approach was used to account for associated costs.

Results

Between 2015 and 2019, the economic impact of selected health effects of heat waves amounts to €25.5 billion, mainly in mortality (€23.2 billion), minor restricted activity days (€2.3 billion), and morbidity (€0.031 billion).

Conclusion

The results highlight a significant economic burden on the French health system and the population. A better understanding of the economic impacts of climate change on health is required to alert decision-makers to the urgency of mitigation and to support concrete adaptation actions.

Project description:ImportanceExtensive research has found the detrimental health effects of heat waves. However, a critical gap exists in understanding their association with adverse health events among older dually eligible individuals, who may be particularly susceptible to heat waves.ObjectiveTo assess the association between heat waves and adverse health events among dually eligible individuals 65 years and older.Design, setting, and participantsThis retrospective time-series study assessed the association between heat waves in warm months from 2016 to 2019 and zip code tabulation area (ZCTA)-day level adverse health events. Dually eligible individuals 65 years and older who were continuously enrolled in either a Medicare fee-for-service plan or a Medicare Advantage plan with full Medicaid benefits from May to September in any given year were identified. All ZCTAs in the US with at least 1 dually eligible individual in each study year were included. Data were analyzed from September 2023 to August 2024.ExposureHeat waves, defined as 3 or more consecutive extreme heat days (ie, days with a maximum temperature of at least 90 °F [32.2 °C] and in the 97th percentile of daily maximum temperatures for each ZCTA during the study period).Main outcomes and measuresDaily counts of heat-related emergency department visits and heat-related hospitalizations for each ZCTA.ResultsThe study sample included 5 448 499 beneficiaries 65 years and older in 28 404 ZCTAs across 50 states and Washington, DC; the mean (SD) proportion of female beneficiaries and beneficiaries 85 years and older in each ZCTA was 66% (7%) and 20% (8%), respectively. The incidence rate for heat-related emergency department visits was 10% higher during heat wave days compared to non-heat wave days (incidence rate ratio [IRR], 1.10; 95% CI, 1.08-1.12), and the incidence rate of heat-related hospitalizations was 7% higher during heat wave days (IRR, 1.07; 95% CI, 1.04-1.09). There were similar patterns in other adverse health events, including a 4% higher incidence rate of death during heat wave days (IRR, 1.04; 95% CI, 1.01-1.07). The magnitude of these associations varied across some subgroups. For example, the association between heat waves and heat-related emergency department visits was statistically significant only for individuals in 3 of 9 US climate regions: the Northwest, Ohio Valley, and the West.Conclusions and relevanceIn this time-series study, heat waves were associated with increased adverse health events among dually eligible individuals 65 years and older. Without adaptation strategies to address the health-related impacts of heat, dually eligible individuals are increasingly likely to face adverse outcomes.

Project description:Alpine plants are considered to be particularly vulnerable to climate change and related extreme episodes, such as heat waves. Despite growing interest in the impact of heat waves on alpine plants, knowledge about their effects on regeneration is still fragmentary. Recruitment from seeds will be crucial for the successful migration and survival of these species and will play a key role in their future adaptation to climate change. In this study, we assessed the impacts of heat waves on the seed germination of 53 high mountain plants from the Northern Apennines (Italy). The seeds were exposed to laboratory simulations of three seasonal temperature treatments, derived from real data recorded at a meteorological station near the species growing site, which included two heat wave episodes that occurred both in spring 2003 and in autumn 2011. Moreover, to consider the effect of increasing drought conditions related to heat waves, seed germination was also investigated under four different water potentials. In the absence of heat waves, seed germination mainly occurred in spring, after seeds had experienced autumn and winter seasons. However, heat waves resulted in a significant increase of spring germination in c. 30% of the species and elicited autumn germination in 50%. When heat waves were coupled with drought, seed germination decreased in all species, but did not stop completely. Our results suggest that in the future, heat waves will affect the germination phenology of alpine plants, especially conditionally dormant and strictly cold-adapted chorotypes, by shifting the emergence time from spring to autumn and by increasing the proportion of emerged seedlings. The detrimental effects of heat waves on recruitment success is less likely to be due to the inhibition of seed germination per se, but rather due to seedling survival in seasons, and temperature and water conditions that they are not used to experiencing. Changes in the proportion and timing of emergence suggest that there may be major implications for future plant population size and structure.
