Lateral Patellar Retinaculum Z-Lengthening.

ABSTRACT: The lateral retinaculum is a 2-layered structure. The plane between the superficial oblique fibers and the deep transverse fibers allows for coronal plane Z-lengthening of the lateral retinaculum. The lengthening procedure can be used for treatment of lateral patellar hypercompression syndrome or as an adjunct to surgical procedures undertaken to address patellar instability. This article describes the surgical technique for lateral retinacular lengthening.

Deep Transverse Lateral Retinaculum Reconstruction for Medial Patellar Instability.

Project description:Medial patellar instability can be a disabling complication of an extensive lateral retinaculum release. It is often overlooked, and for the diagnosis, it is necessary to have a high index of suspicion. Typically, the patient feels a new pain and new instability after the lateral retinaculum release that are distinct from, and much worse than, those before surgery. All of our patients had significant relief from their pain with "reverse" McConnell taping. If there is a significant improvement in symptoms after this taping and stress radiographs or stress axial computed tomography scans show an objective pathologic medial patellar displacement, reconstruction of the lateral retinaculum should be considered. This article details our technique for reconstruction of the deep transverse layer of the lateral retinaculum using an anterior strip of the iliotibial band. This strip is detached from its insertion onto the Gerdy tubercle and then reflected proximally beyond the level of the lateral femoral epicondyle. Finally, it is attached either by direct suture to the remaining prepatellar and peripatellar retinaculum if there is adequate tissue present or by a suture anchor.

Dynamic Ultrasonography for Imaging Pediatric Fat Pad Herniation through the Lateral Patellar Retinaculum.

Project description:A 3-year-old boy was found to have a painless mass over his right anterior lateral knee for the previous 6 months. The mass was hardly identified during knee extension and became visible upon squatting. There was no specific finding on ultrasound imaging over his right knee at the supine extended position. During squatting, ultrasound imaging showed an ill-demarcated hyperechoic mass protruding next to the cartilage overlying the distal femur towards the subcutaneous layer through a slit over the lateral patellar retinaculum. Herniation of the fat pad through a defect in the lateral patellar retinaculum was diagnosed. Our case highlights the usefulness of ultrasound examination as regards the lateral patellar retinaculum defect in pediatric knees, as well as its capability for dynamic scanning to capture the exact "pathological moment".

Patellar Tendon Lengthening: Rescue Procedure for Patella Baja.

Project description:Patella baja is a challenging pathologic condition that causes pain and functional restrictions and can even lead to premature osteoarthritis-even more so in cases of patella infera or cases associated with degeneration of the patellar tendon in which simple conservative treatment frequently is not resolutive. Several surgical options have been described for symptomatic patella baja: excision of the lower third of the patella, lengthening of the patellar tendon, reconstruction of the patellar tendon with allograft, and proximalization of the tibial tubercle. A combination of 2 or more of these treatments may be recommended in cases of significant patella baja. We present a simple and reproducible technique to address patella baja that combines a partial transposition of the tibial tubercle and patellar tendon lengthening using a subperiosteal patellar flap in continuity with the patellar tendon.

Trochleoplasty, Medial Patellofemoral Ligament Reconstruction, and Open Lateral Lengthening for Patellar Instability in the Setting of High-Grade Trochlear Dysplasia.

Project description:Trochlear dysplasia is the most commonly encountered pathoanatomy in patients who present with patellar instability. Outcomes of trochleoplasty procedures have shown low rates of recurrent instability and high patient-reported outcome scores. This article describes a "thin-flap" groove-deepening trochleoplasty combined with medial patellofemoral ligament reconstruction with a gracilis allograft and lateral retinacular lengthening to treat recurrent patellar instability due to high-grade trochlear dysplasia. This technique can obviate tibial tubercle osteotomy by normalizing the position of the trochlear groove and, subsequently, decreasing the tibial tubercle-to-trochlear groove distance.

Arthroscopic Patellar Lateral Facetectomy.

Project description:Isolated patellofemoral osteoarthritis is relatively prevalent, with the lateral facet of the patella being the most commonly affected portion. This pathology can be a result of a patellar maltracking syndrome, patella instability, or idiopathic degenerative changes. A thorough diagnostic work-up with a physical examination and imaging studies are mandatory for a proper diagnosis and to rule out other causes of patellofemoral knee pain. These patients are often treated nonoperatively with exercises for patella mobility, intra-articular injections, braces, patellar tracking, quadriceps balance and strength, and activity modification. Patients with lateral patellar pain that is refractory to nonoperative management, and who have a clear bony deformity on the patella overriding the lateral aspect of the trochlea, can benefit from surgical intervention. We recommend an arthroscopic lateral patellar facetectomy because the joint can be dynamically assessed, treated, and re-evaluated intraoperatively to ensure that normal bony contact has been restored.

Arthroscopic Lateral Retinacular-Lengthening Procedure.

Project description:Anterior knee pain is a common musculoskeletal complaint that is often due to an excessively tight lateral retinaculum, which normally plays a role in patellar tracking and stabilization. Several etiologies underlie lateral soft-tissue tightness in the knee, including lateral patellar compression syndrome, patellofemoral arthritis, patellofemoral instability, and patellofemoral pain syndrome. Stretching the lateral retinaculum through conservative treatment may be helpful, but lateral retinacular lengthening may be indicated. Since this surgical procedure has classically been performed in an open fashion, the purpose of this Technical Note is to describe an arthroscopic technique designed to limit complications, improve patient outcomes, and reduce operative and recovery times.

Quadriceps Tendon Lengthening for Obligatory (Habitual) Patellar Dislocation in Flexion.

Project description:Obligatory patella dislocation in flexion is an uncommon form of patellar instability, where the patella is located in extension and dislocates with every episode of knee flexion. This results in dramatically altered patellofemoral kinematics and can be extremely debilitating due to extensor strength deficits and lack of knee confidence in flexion. Concomitant pathology, which is often seen, includes a tight lateral retinaculum and a shortened extensor mechanism. Lengthening the extensor mechanism is a critical part of successful patellar stabilization, and has not been well-reported. Herein, we present a technique of quadriceps lengthening for the treatment of obligatory patellar dislocation.

Partial Patellar Lateral Facetectomy: Arthroscopic Technique.

Project description:Lateral patellofemoral osteoarthritis is a relatively common condition in young, active patients. Partial lateral patellar facetectomy is a relatively minimally invasive option to restore knee function and return to sport when conservative treatments are insufficient. We describe the arthroscopic technique of partial lateral patellar facetectomy , which has been historically performed as an open procedure. We describe preoperative planning, arthroscopic portals, landmarks, intra-articular evaluation of bone resection, and surgical difficulties. The goal of the preoperative planning was a target bone resection of 10 to 15 mm including osteophytes and the lateral facet of the patella. Arthroscopic anterolateral and anterior superolateral portals were used to achieve the desired resection. An arthroscopic technique allows a direct view of the osteoarthritic bone for removal and permits intraoperative dynamic evaluation, which allows the surgeon to finetune the bone resection and obtain optimal patellar tracking. Avoiding arthrotomy could reduce morbidity and allows a rapid postoperative rehabilitation.

All-Arthroscopic Management of Lateral Patellar Instability.

Project description:Patellar dislocations are common injuries that can lead to recurrent dislocations and instability. Patellar instability is a complex problem and requires a multifactorial approach. Over the years, many different repair and reconstruction techniques has been developed. The variety of techniques proves that there is no best procedure, and different options must be taken into consideration in every case. Many of these techniques are complicated and require graft harvesting or drilling through the patella. We present a technique of arthroscopic patellar stabilization with a single suture anchor and lateral release that is easy to perform, cost-effective, and does not require drilling tunnels through patella or graft harvesting.
